Glaucoma - Factors That Put You Risk!

DOMS, MBBS
Ophthalmologist, Delhi
Glaucoma - Factors That Put You Risk!

Glaucoma is a group of eye diseases characterised by damage to optic nerve usually due to excessively high intra ocular pressure (eye pressure). If left untreated can lead to optic nerve damage resulting in progressive, permanent vision loss. Glaucoma usually affects people above the age of 40, but can affect younger people as well. This condition causes fluid to build up in the eye, which puts pressure on the optic nerve.

There are broadly two types of glaucoma - Open angle glaucoma ( more common in elderly) and angle closure glaucoma.

Women are more likely to suffer from glaucoma than men are. Ethnicity also plays an important role in determining the risk of suffering from this condition. East Asian and African Americans have a high risk of glaucoma, as they have a shallow, anterior chamber depth. Some of the other conditions that can increase a person’s risk of glaucoma are:

  1. Diabetes
  2. Hypothyroidism
  3. Myopia
  4. An earlier eye surgery
  5. Retinal detachment
  6. Tumours
  7. Injury to the eye
  8. Long term corticosteroid medication

If diagnosed early enough, glaucoma can be treated and its effects can be reduced. However, the damage already caused by glaucoma cannot be reversed. The aim of glaucoma treatment is to either reduce the production of fluid in the eye or improve its flow. This helps slow down the progression of this disease and prevents any further loss of vision. Glaucoma treatment usually takes the form of eye drops or surgery.

Eye drops are the first step towards treating glaucoma. There are many different types of eye drops. Prostaglandin analogues aim at increasing the flow of fluid out of the eye and are usually prescribed in cases of swelling around the rim of the eyes, darkening of the iris and blurred vision. Beta-blockers can also help lower the secretion of fluids in the eye.

Patients who are also suffering from diabetes or lung conditions such as bronchitis may be prescribed cholinergic agents, carbonic anhydrase inhibitors or sympathomimetic drugs. Cholinergic agents help improve the outward flow of fluid in the eyes while Carbonic anhydrase inhibitors reduce fluid production in the eyes. Sympathomimetic drugs do both reduce the production of fluids and improve outflow.

If the eye drops are not effective, surgery may be advised to lower the pressure built up in the eye. Some of the common types of surgery for glaucoma are:

  1. Trabeculoplasty: This procedure involves directing a high-energy laser beam into the eye to open clogged drainage canals. The surgery has high success rates but the problem may recur after surgery.
  2. Filtering surgery: This involves creating an opening in the eye and removing a part of the trabecular meshwork, thus allowing fluid to flow outwards.
  3. Drainage implants: Children or patients with secondary glaucoma may be treated with this surgery where a small silicone tube is placed in the eye to aid in fluid drainage.

How to Decide the Type of Eye Lens (Spherical/Monofocal/Multifocal) in Cataract Surgery?

MBBS, DOMS, MS - Ophthalmology, , Fellowship in phacoemusification
Ophthalmologist, Patna
How to Decide the Type of Eye Lens (Spherical/Monofocal/Multifocal) in Cataract Surgery?

Cataract also called 'motiyabin' in hindi. Phacoemusification also called phaco is the most common procedure done for removal of cataract in the world now.

Common public is often misguided by marketing professionals/staff in hospitals to opt for expensive lenses without thinking whether patients actually needs them or not.

Here is a complete summary of type of lens implanted in cataract surgery and it's pros and cons.

Fixed-focus monofocal lenses-these lenses can help you see at a distance, but you'll need reading glasses to see up close.

Accommodating monofocal lenses (crysta lens)
-a newer option, accommodating monofocal lenses, can shift from near to far vision in response to movements of the ciliary muscles in your eye, similar to the natural lens in a younger eye. These lenses offer excellent distance and middle vision, but they aren't as reliable for near vision. Eye exercises can help you get used to them, but about half of people who receive them still end up needing reading glasses.

Toric lenses-these specialized monofocal lenses are designed for people with astigmatism. Toric lenses offer focused vision at a single distance and correct your astigmatism, so you might not need distance glasses after surgery.

Multifocal lenses-like the bifocal or progressive lenses used in glasses, multifocal lenses have different areas designed for distance, intermediate, and near vision. The brain and eye figure out which part of the lens to use. The main drawback of multifocal lenses is they can distort bright light, creating more glare and halos at night.

Aspheric lens-designed to improve contrast sensitivity and visual clarity under certain circumstances.

Monovision-this implant technique uses a monofocal lens with a different power in each eye so that you don't have to use glasses for most of your daily activities. Your dominant eye is generally set for distance, and the other eye is set for near. Many people successfully use monovision with contact lenses. Successful monovision requires cataract surgery to each eye.

I have two problems First is regarding my eyes. I feel pain in eyes while using mobile or while driving in evening? Second one I have some fat on my tummy how to reduce it?

I have two problems First is regarding my eyes. I feel pain in eyes while using mobile or while driving in evening?
S...
Mobile screen is made up of hundreds of tiny pixels that are constantly moving. Our eyes in turn are not naturally able to set focus on these constantly moving pixels, and hence we try to focus on an area behind the actual screen. Therefore, in order to work on a mobile, our eyes need to keep on fluctuating focus between their natural point of focus and the pixels on the screen. And this tires the eyes and causes a lot of wear and tear.As a result, we suffer from irritation, which may range from a number of things like headaches, dry eyes, blurred vision, problems in focusing and faulty color perception, to something as severe as glaucoma. The electromagnetic waves that are emitted by a mobile phone can be extremely damaging to the eyes. The solution for this problem I its reduced usage and while uing mobiles use them inproper light. There is nothing more effective than controlling usage to reduce and negate the effects. You can also reduce the intensity of the mobile’s screen by attaching a filter or by getting an eye-friendly screen. Above all, one must try to reduce the amount of mobile usage or take frequent breaks. Homoeopathic medicine CINELUBE EYEDROPS ( HAPDCO) Pour 2 drops 3 times daily. What is your weight ,so that I could guide you regarding your tummy fat accordingly.

Lasik surgery done before 3 weeks halos and glayer are disturbing so much at night and vision is not sharp and stable.

MBBS, DOMS, MS - Ophthalmology, , Fellowship in phacoemusification
Ophthalmologist, Patna
Lasik surgery done before 3 weeks halos and glayer are disturbing so much at night and vision is not sharp and stable.
LASIK is refractive surgery to correct myopia, hypermetropia & astigmatism, refractive surgery also include intraocular lend procedure. For  LASIK your eyes must be healthy, if you have persistent dry eyes, LASIK surgery may aggravate this condition resulting in burning sensation & blurring of vision after LASIK.  For LASIK cornea must have sufficient thickness because in reshaping of cornea, you loose the thickness of cornea & if your cornea is too thin, you are not fit case for LASIK.   -1D of myopic correction  consumes 35 micron of corneal thickness.  Average thickness of cornea in Indian is 450 micron to 500 micron& after LASIK there should be around 300 micron thickness of cornea for safety of eye in future.  Pupils are usually large in myopic people who undergo LASIK resulting in side effects such as halos, glares especially when driving in night.       LASIK for the treatment of high retractive errors are less predictable, because very high amount of myopia may require removal of too much corneal tissue putting at high risk of vision complications.  LASIK should not be undertaken before 21 years in male & 19 years in female because myopic power increases by that age, you must have stable vision before LASIK.   There is no upper age limit but one should keep in mind that once you are 40 years old, you may still need reading glass due to presbyopia changing every two years till you develop cataract in old age.     Contraindication for LASIK is degenerative disease, sjogren syndrome, type 1 diabetes etc. People who are taking immunosuppressive must think 10 times before undergoing LASIK.  One must go LASIK  screening before taking decision for LASIK.   One who undergoes for LASIK should be fully aware of potential LASIK complications & should not hide anything from operating surgeon about health.

Hello. There is no medication or food for improving eye sight or to get rid of glasses. Get your vision tested and wear glasses. If not interested in glasses then you can try a. Contact lens b. Lasik laser as your age is now 25 years so your number will not increase much now whether you wear glasses or not as number becomes stable at age of 18-20 yrs. Having said that I advise you to eat balanced diet including green vegetables and seasonal fruits for keeping eyes and body healthy. Lasik is a very easy and painless procedure. If you want to get rid of glasses then go for lasik.

Surgery is safe and comfortable but remember that it will only be for distance vision you will need reading glasses for near vision whether you get surgery done or not as your approaching 40 and use of reading glasses is age related.

. I am 15years old male and from my eye some time water come ourt. What should I do?
Hello, Get your eyes tested from an Opthalmologist. Include the following for getting better vision: Vitamins A, C, E, and minerals like copper and zinc are essential to eyesight. Get the antioxidants from dark leafy greens, egg yolks, yellow peppers, pumpkin, sweet potatoes, and carrots. Include garlic and onions. Blueberries, grapes as they have antioxidant and anti-inflammatory properties that can help improve your vision. DHA is a fatty acid found in coldwater fish like salmon, sardines and mackerel. Exercises for eyes: Warm your eyes. Rub your palms together to create heat, and then place them against your eyes for five seconds. Repeat this three times. Roll your eyes. Start by looking up and then slowly circle 10 times clockwise and 10 times counterclockwise. Focus. Hold a pen at arm's length, focus your eyes on it, and slowly bring the pen closer until it's about 6 inches away from your nose. Then slowly move it back, keeping your eyes focused on the pen, 10 times in all. Massage your temples. Using your thumb knuckles, massage your temples in small circles, 20 times in one direction and 20 in the other. Repeat the same actions above the mid-point of the eyebrows at the forehead, then below the eyes on both sides of the bridge of the nose. Take a mini-nap. Put your head back, close your eyes, and relax for 3 minutes. Insufficient sleep may weaken your vision, so shoot for 8 hours of sound sleep a night. Rest your eyes 10 minutes for every 50 minutes spent reading or in front of the computer. If your eyes feel overly tired, lie down and place cooling cucumber slices over your eyelids. Medication: Take homoeopathic medicine which will help you in preventing first and reducing the power of your eyes. Take Physostigma 6/ thrice daily for 1 month. Thereafter, revert back for further guidance.
