I have been diagnosed with spiking of tibial spine bilaterlly, tiny marginal ostophytes formation seen at right patellar polar region. I feel some hard object while lying down at night and also burning sensation in my anus region. Pl advise?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Spiking of tibial spine with marginal osteophytes are indication of degenerative changes in your knee region. Treatment of osteoarthrosis depends upon what grade of degeneration has occurred, what is your knee range of movements and what medications like analgesics do you use on a daily basis? recommendations for early oa are: - start doing daily brisk walks for at least 2-3 km in a day in mornings. - intensive knee - quadriceps and hamstring strengthening exercises should be initiated initially in presence of a trained physical therapist and then at home on your own - avoid sitting cross legged, not to use indian style washrooms, do not do anything which eccentrically loads the knee and increases cartilage breakdown. - adequate calcium and vitamin d metabolism in terms of appropriate intake is important to keep bone metabolism ok - you can take short course of anti inflammatory medication like tab etoshine on sos basis especially when there is an episode of inflammatory pain, swelling etc. Do not hesitate to contact me if you need any further assistance.

In my opinion reading your query I will advise you to get a comprehensive body checkup done with all investigations including inflammatory profile markers also. It will be good if you can actually consult with a general physician and get all the tests done. From an orthopaedic point of view, since you have multiple joint pains, I will advise you to get: cbc, esr, crp, serum vitamin d levels, serum electrolytes, thyroid profile also. Hello start physiotherapy treatment and learn posture care/ spine exercise. Meanwhile take few precautions. 1) take hot water fomentation for 20 min. At every 3hrs. May be you lift heavy weight then your shoulder can lift. Increase your weight gradually with variety of exercises. Do exercises with medium weight followed by hold them for few min. In which muscle is loaded. Then increase your weight.

I am 17 years old After masturbation I have pain in my all joints and I become lazy I don't want to do any work and when I take breadth inside, some type of vibrate is being held inside my stomach and my skin become rough.

All these are not related to masturbation. Masturbation is not really bad; it is the addiction to it that is bad. Not even the number of years is a problem but the frequency and compulsiveness to do it is to be considered for its ill-effects. At your age you may feel it difficult and it can become addictive if you have alone time, if you obsess about it, and if you are not into relationships. The problem with addiction to this is that you may go on to prefer masturbation to heterosexual contact, even when you are married. That obviously would be an insult to your wife and will lead to a lot of conflict. Heterosexual contact is very wholesome and the physiology is made exactly for penile and vaginal contact. You will instead enjoy a poor replica of this and not know or care for the difference. The second thing that can happen with addiction is that it will become compulsive and will have no boundaries or limits, since it can be done all by yourself, whenever, wherever, and however too. The big problem a lot of children get into is the guilt around this. A useful way to discharge the energy is to get into contact games, or exercises, or even hobbies to channelize it. Also you should not spend too much time alone, and you need to keep your mind occupied with other interesting things and a lot of socializing. Even if you are not interested in marriage, for whatever reasons, it will help you to get into a friendship with a girl and enjoy the relationship where there will be a lot of excitement when you fall in love. Right now your hormones may be very active and your physiological apparatus is primed and ready for sexual stimulation. But because of your age and even otherwise it is important to develop good control and not feed into your impulses. Keeping yourself occupied and living a healthy social life are the keys to manage your sexual drives.

Dear , this condition is known as planter fascitis. Just do 1 thing at home, take lukewarm water in a small tub and put a small teaspoon of salt to it. Dip your heel inside for 30mins. After that dry it properly n put a thin filament of any ointment like moov, volini etc. N wear socks so that the heat remains for a longer period n keep your foot under the blanket. Do it before sleeping for 7 days and then tell me how it feels.

I am aged 16 and from 2 days before, I have started an ache or pain in backside. what i have to do ?

Investigations advised - hb, tlc, dlc, esr, sugar f & pp, uric acid, vit d3 urine r/e x ray of the affected part. Anyway you may try- sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomentation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Mri scan can help in headache problems to find out serious causes of headache like tumor or infection. There is no harm in getting mri scan done. Migraine diagnosis is made on detailed history about headache pattern and response to preventive treatment. Since details of your headache character is not mentioned a diagnosis cannot be made. Migraine headache is characterised by unilateral pulsating headache associated with nause/vomiting and photophobia / phonophobia. There are triggers for migraine like sun exposure, certain foods or smell, lack of sleep, excess caffeine etc. The triggers are variable for different patients. Migraine attacks can be managed by simple painkillers like paracetamol but in resistant cases drugs like sumatriptan may be needed. Follow these lifestyle changes to avoid migraine headache 1. Maintain regular sleep wake cycle, even on holiday's. 2. Don't skip meals. Take regular healthy diet with adequate fluids 3. Do not consume excess caffeine / carbohydrates / alcohol or smoke. 4. Exercise daily (aerobics and cardiovascular) 5. Identify your headache trigger and try to avoid it. 6. Do pranayam daily. You can consult a pain specialist for therapies like dry needling or botulinum injections for your problem. Consult again for further queries.

Usually it happens due to repetitive use of tools for prolonged period like scissors screwdrivers which press firmly on the tendon sheath at the base of finger or thumb and also Diabetic and Rheumatoid arthritis are at increased risk

Do hot fomentation twice daily. Postural correction- always sit with a small pillow at your low back. Sit tall, walk tall. Extension exercises like bridging and bhujang asana x twice daily. Sleep on your back with a pillow under your knees or on your sides with a pillow between your knees. And get vit d levels checked. Go for regular walks follow 30-30 rule when at work. After every 30 min of work. Do 30 sec of stretching or stand and walk a bit.
