Upper Gastrointestinal Bleeding - Common Reasons Behind It!

It is gastrointestinal bleeding in the upper gastrointestinal tract, commonly defined as bleeding arising from the esophagus, stomach, or duodenum. Blood is often observed in vomit (hematemesis) or in stool (melena). Upper gastrointestinal bleeding denotes a medical emergency and typically requires hospital care for primary diagnosis and treatment. The incidence of upper gastrointestinal bleeding is 50-150 individuals per 100,000 annually. Depending on its severity, it carries an estimated mortality risk of 11%.

The causes of upper gastrointestinal bleeding are as follows:

Esophageal causes (gastrorrhagia):

  • Esophageal varices
  • Esophagitis
  • Esophageal cancer
  • Esophageal ulcers
  • Mallory-Weiss tear

Gastric causes

Dieulafoy's lesions

  • Duodenal causes
  • Duodenal ulcer
  • Vascular malformation, including aorto-enteric fistulae
  • Hematobilia or bleeding from the biliary tree
  • Hemosuccus pancreaticus or bleeding from the pancreatic duct
  • Severe superior mesenteric artery syndrome

The signs and symptoms of upper gastrointestinal bleeding are as follows:

  • Hematemesis - Vomiting of blood
  • Melena - Blood in the stool
  • Hematochezia - Passage of fresh blood through the anus, usually in or with stools
  • Syncope - Loss of consciousness (fainting)
  • Presyncope - State of lightheadedness, muscular weakness, blurred vision, and feeling faint
  • Dyspepsia – IndigestionEpigastric painHeartburnDiffuse abdominal pain
  • Dysphagia - Difficulty in swallowing. Weight lossJaundice - Yellow discoloration of the skin, mucous membranes, and sclera

The diagnosis of upper gastrointestinal bleeding is made when hematemesis is present. In the absence of hematemesis, an upper source of GI bleeding is likely in the presence of at least two factors among - Black stool, age < 50 years or blood urea nitrogen/creatinine ratio 30 or more

If these findings are absent, consider a nasogastric aspirate to determine the source of bleeding. If the aspirate is positive, an upper GI bleed is greater than 50%, but not high enough to be certain. If the aspirate is negative, the source of a GI bleed is likely lower. The accuracy of the aspirate is improved by using the Gastroccult test. Also, the following diagnostic tests are done:

  • Orthostatic blood pressure
  • Complete blood count with differential counts
  • Hemoglobin level
  • Type and crossmatch blood
  • Basic metabolic profile, BUN,
  • Coagulation profile
  • Serum calcium
  • Serum gastrin
  • Endoscopy
  • Chest radiography
  • Nasogastric lavageAngiography (if bleeding persists and endoscopy fails to identify a bleeding site)

Upper gastrointestinal bleeding can be managed in the following ways:

  • Airway management and fluid resuscitation using either intravenous fluids and or blood
  • Medications to stop the bleeding (Proton-pump inhibitors are often given in the emergency)
  • Surgical intervention
  • Treating the consequences (like anemia) that the bleeding may have caused
  • Precautions are taken to prevent rebleeding

The development of breasts is one of the most prominent physical difference between the structure of a man and a woman. In some cases, hormonal imbalances can cause enlargement of the male breast gland, which is known as gynecomastia.  Apart from being a cause for embarrassment, it is also associated with a number of medical conditions such as kidney failure and liver cirrhosis. However, there is little need to worry if you have been diagnosed with such a condition as it can easily be treated with dietary changes, medication and surgery. Among these, surgery is the only method of treatment that guarantees a permanent and immediate solution to the problem. 

Surgical treatment for gynecomastia is of two types

The type of surgery suitable for you will depend on a number of factors including the extent and type of gynecomastia.

Liposuction involves the removal of fat from the breast, but does not touch the breast tissue itself. For this surgery, a surgeon makes a small incision on either side of the chest or along the edge of the areola. Excessive fat is then removed through these incisions and the skin and remaining tissue is then restructured. Liposuction can take anywhere between a week and 3 weeks for complete recovery. In the meanwhile, the patient must wear a compression garment to minimize the swelling and support the chest tissue as it heals. 

A mastectomy on the other hand is used to remove the breast tissue. A mastectomy is performed endoscopically and hence is less invasive. In this case, the incisions are located around the edge of the areola or in the natural crease of the chest. A mastectomy requires a shorter recovery period. In some cases, a combination of the two techniques may also be used. 

The results of both surgical approaches can be seen immediately after the surgery. Slight soreness and pain is a common side effect of this procedure and can be relieved with pain medication. Some men may also experience a temporary loss of sensation over the treated area. 

Though the results of both types of surgeries are permanent, it is important to maintain a healthy lifestyle after the surgery. Unhealthy eating that leads to significant weight gain can make the problem recur. Use of steroids or medication that affects the testosterone levels in the body and certain medical conditions can also lead to recurring gynecomastia.

Atrial fibrillation is a form of irregular heartbeat or arrhythmia, which you may experience due to a problem in the electrical system of the heart. Such a problem causes the upper parts of the heart or the atria to fibrillate. Due to this quivering, the normal rhythm between the lower parts of the heart and the atria gets disrupted. The ventricles are likely to beat faster in an irregular rhythm. This is a severe condition where the blood may get collected in the atria; this could lead to the formation of blood clots. These clots can block the blood flow and lead to a stroke.

Causes
Several conditions cause strain and damage to the heart. These include the following:

  1. High blood pressure or hypertension
  2. Heart attack
  3. Valvular diseases
  4. Coronary artery disease
  5. Medical problems like heart failure, lung diseases, high level of thyroid or pneumonia
  6. Heart surgeries

Diagnosis
Conducting several tests, physical examinations and an analysis of your health history is the first step towards diagnosing atrial fibrillation. An electrocardiogram (ECG) needs to be carried out for the detection of this condition. This test is performed for checking problems regarding the electrical activity of the heart. Other laboratory tests and an echocardiogram maybe required as well. An echocardiogram helps in observing the pumping function of the heart and to check whether the valves have been damaged.

Treatment
The treatment options for atrial fibrillation depend on the cause, symptoms and the risks of getting a stroke. Several medicines are used for treatment along with other methods. They are as follows:

  1. Blood thinning medicines for the prevention of a stroke.
  2. Heart rate control medicines which will prevent irregular beating of the heart.
  3. Rhythm control medicines for restoring the heart’s rhythm to normal.
  4. A process known as cardioversion may be used to bring the heartbeat to a normal rhythm. This can be carried out by medicines or an electric shock therapy known as electrical cardioversion.
  5. In case of severe symptoms, ablation may be carried out where the affected areas of the heart are destroyed by the creation of a scar tissue.

Hello doctors. My name is usman tanimu, I'm from Nigeria, i'm 27 years old, and also i'm a footballer, for the past two years I got an injury in my knee joint which lead to knee buckling, I was tried so hard to treated this issue but till date my knee still buckled when I play the football, I was scanned using x-ray machine, the doctor told me that my knee joints are in normal position, but the knee keeps on buckling when I play the football with less pains, please doctors how can I do to stop my knee from buckling and is there any pills or injection to stop my knee from buckling? It seems you have an ACL injury in the knee. You need to examined and you should have an MRI scan. ACL is an important ligament for stability of knee and injury to ACL or tear of ACL leads to buckling. As you are young and involved in sports. You might need a surgery if there is an ACL tear. I operated a boy who plays international hockey at under 19 level 2 years ago. He came with same symptoms. He is back to playing Hockey without any difficulty.
