Hello, Thanks for the query. It is good to know that blood glucose is in control. Ideally when in control FBG should be < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. You can take other things as mentioned in your communication for additional support. Fenugreek can be taken either as powder or roasted seeds about 2 to 3 tea spoon fulls (seeds) before a meal. Similar quantity of powder can be used. Cinnamon about 6 g twice a day may be used. Garlic I am not aware. Thanks.

You are suffering with type 1 daibetes for this insulin intake is the only treatment. You have to take insulin injcetns as per your blood sugar levels. And you have to follow restrict diabetic diet.

My wife has sugar level between 300 and 400 -fasting and post what you suggest. She is under the treatment of a specialist but it does not come down.

Either your wife is not following advices given by her specialist or your specialist is not taking proper care. Treatment of diabetes needs 1) medicines - that a doctor specialist can only advice. 2) patient's participation in treatment (70% of the treatment effectivity depends on it) by the way of dietary management, exercise and weight control if overweight. 3) monitoring for the disease control and prevention of complication. If you are sure your wife is following advices in toto about diet and exercise and weight control, you change the doctor without further trial.

1. Lose extra pounds and watch your waistline Blood pressure often increases as weight increases. Being overweight also can cause disrupted breathing while you sleep (sleep apnea), which further raises your blood pressure. Weight loss is one of the most effective lifestyle changes for controlling blood pressure. Losing just 4.5 kilograms can help reduce your blood pressure. Besides shedding weight, you generally should also keep an eye on your waistline. Carrying too much weight around your waist can put you at greater risk of high blood pressure. In general: • Men are at risk if their waist measurement is greater than 40 inches (102 centimeters). • Women are at risk if their waist measurement is greater than 35 inches (89 centimeters). These numbers vary among ethnic groups. Ask your doctor about a healthy waist measurement for you. 2. Exercise regularly Regular physical activity — at least 30 minutes most days of the week — can lower your blood pressure by 4 to 9 millimeters of mercury (mm Hg). It's important to be consistent because if you stop exercising, your blood pressure can rise again. If you have slightly high blood pressure (prehypertension), exercise can help you avoid developing full-blown hypertension. If you already have hypertension, regular physical activity can bring your blood pressure down to safer levels. The best types of exercise for lowering blood pressure include walking, jogging, cycling, swimming or dancing. Strength training also can help reduce blood pressure. Talk to your doctor about developing an exercise program. 3. Eat a healthy diet Eating a diet that is rich in whole grains, fruits, vegetables and low-fat dairy products and skimps on saturated fat and cholesterol can lower your blood pressure by up to 14 mm Hg. It isn't easy to change your eating habits, but with these tips, you can adopt a healthy diet: • Keep a food diary. Writing down what you eat, even for just a week, can shed surprising light on your true eating habits. Monitor what you eat, how much, when and why. • Consider boosting potassium. Potassium can lessen the effects of sodium on blood pressure. The best source of potassium is food, such as fruits and vegetables, rather than supplements. Talk to your doctor about the potassium level that's best for you. • Be a smart shopper. Read food labels when you shop and stick to your healthy-eating plan when you're dining out, too. 4. Reduce sodium in your diet Even a small reduction in the sodium in your diet can reduce blood pressure by 2 to 8 mm Hg. The effect of sodium intake on blood pressure varies among groups of people. In general, limit sodium to less than 2,300 milligrams (mg) a day or less. However, a lower sodium intake — 1,500 mg a day or less — is appropriate for people with greater salt sensitivity, including: Anyone age 51 or older Anyone diagnosed with high blood pressure, diabetes or chronic kidney disease To decrease sodium in your diet, consider these tips: • Read food labels. If possible, choose low-sodium alternatives of the foods and beverages you normally buy. • Eat fewer processed foods. Only a small amount of sodium occurs naturally in foods. Most sodium is added during processing. • Don't add salt. Just 1 level teaspoon of salt has 2,300 mg of sodium. Use herbs or spices to add flavor to your food. • Ease into it. If you don't feel you can drastically reduce the sodium in your diet suddenly, cut back gradually. Your palate will adjust over time. 5. Limit the amount of alcohol you drink Alcohol can be both good and bad for your health. In small amounts, it can potentially lower your blood pressure by 2 to 4 mm Hg. But that protective effect is lost if you drink too much alcohol — generally more than one drink a day for women and for men older than age 65, or more than two a day for men age 65 and younger. One drink equals 12 ounces of beer, five ounces of wine or 1.5 ounces of 80-proof liquor. Drinking more than moderate amounts of alcohol can actually raise blood pressure by several points. It can also reduce the effectiveness of blood pressure medications.

I am a man of 66 years having bp and sugar problem. For the last two months I am having continuous pain on top of the right shoulder. I have applied omni gel regularly but there is not relief. What should I do now?

Dear , get an ecg done. If it is normal I think it could be' frozen shoulder" which is quite common in diabetes. You can take pain killers apply diclofenac ointment and do physiotherapy. It might take some time but in most patients the condition goes away within 2 yrs

Mr. Lybrate-user, you blood sugar levels mentioned are quite with in the expected range when on treatment. What is the hba1c%? it should be <7 or ideally closer to 6.5%. You are controlling the sugar levels really well. Keep it up. Avoiding hyperglycemia is essential as prevents or delays development of long term complications of diabetes.

U need to take 1600 kcal balnaced adiet eat more vegetables and fruits eat foods which is low glycemic index do exercise do not skip meal avoid sugar eat 15-20 mi oil/day.

I am 53 years male ,Initially I was dignosed with 350 sugar level around 3 years back took Metformin 500 BD but it is still fluctuating between 150 - 250. Do take sweet some occassions and also use sugar free tablets. Please advise.

Avoid sweetened fruit like banana, mango, litchi, chickoo, sapota, seethaphal, jackfruit, grape etc. You can have apple/pear/ papaya/guava/ jamun/ mausami.

Mr. lybrate-user, Orange gives only few grams of glucose. It is unlikely to raise blood glucose level to the extent mentioned. However, 174 mg random glucose level is high. It is better to get a proper fasting, PP glucose level and HbA1c% done. That will give a clear idea whether there is any problem. Following are the criteria to decide whether a person has diabetes: 1)Normal: FBG:70 to 110 mg, HbA1c% <5.9 2) Pre-diabetes: FBG:108 to 125 mg, PP 141 to 199 mg & HbA1c%5.9 to 6.4. 3) Diabetes:FBG: 126 mg and above, PP 200 mg and above & HbA1c%6.5 and above. yes,its typical diabetic neuropathy,mostly affecting both legs.. keep watch on your legs regularly during going to sleep..as it will help to find any injury happened to legs left unchecked due to loss of sensation.. use medicated oil for massage, use proper footware, Take some ayurvedic medications.

DIABETIC DIET ADVICE: ●Eat regular meals throughout the day at same time everyday..Avoid skipping meals ●Include high fibre foods at each meal e.g. legumes(beans/lentils ),nuts(almonds,walnut),eggs etc ●Take fruit as a healthy snack e.g. apples,pears,papaya,mausami,jamun,guava ●Try to avoid –grapes,mango,bananas,jackfruit(cheeku) ●Vegetables to be included in plenty-at least 2 servings per meal ●Reduce the fat intake,fried foods,cakes,pastries and foods containing palm oil and coconut oil ●Include healthy foods like daliya,oats ●Reduce salt intake in the diet to 1 tsp or less per day ●stay away from especially labelled diabetic foods(they are of no benefit),and saturated fats ●Limit intake of sugar and sugary foods like sweets ●If so desire can take sugarfree as sugar substitute for sweetening purposes DIABETIC DIET CHART: Morning: 4-5 Almonds soaked overnight in water or 1-2 walnuts 6:30 a.m.: Tea + 2 mariegold biscuits(only for those who can’t skip their morning tea) 8:00-9:00 a.m. : 2 Roti + subzi + daliya /upma/poha/raw cheese/sprouts + 1 glass skimmed milk 11:00-12:00 a.m.: Fruits (only those permitted), salads, guava, papaya, pomegranate, cucumber, carrot, sprouts etc 1:00-2:00 p.m.: 2 roti +dal + salads+curd/raita/buttermilk 4:00-5:00 p.m : tea + roasted chana/bhelpuri(only if hungry) 7:00 p.m : salads/coconut water/lemon water(without sugar) 8:00-9:00 p.m: 2 roti +dal/khicdi + skimmed milk Try to sleep daily on sametime till 11:00 p.m. SPECIAL PRECAUTIONS : Avoid potato/sweet potato/arbi banana/cheeku/grapes/mango/litchi papads/ chatni/pickles/fried foods groundnuts/cashew/pista/raisins sweets/honey/cold drinks/jaggery Note: For preparing chapatis, use whole wheat flour mixed with chana flour or besan in 3:1 proportion alongwith small amounts of soyabean flour Learn how to live with diabetes: It is common to feel overwhelmed, sad, or angry when you are living with diabetes. You may know the steps you should take to stay healthy, but have trouble sticking with your plan over time. Stress can raise your blood sugar. Learn ways to lower your stress. Try deep breathing, gardening, taking a walk, meditating, working on your hobby, or listening to your favourite music. Eat well: stick to a diabetes meal plan .chose foods that are lower in calories, saturated fat, trans fat, sugar, and salt.Eat foods with more fibre, such as whole grain cereals, crackers, rice, or pasta. Set a goal to be more active most days of the week. Start slow by taking 10 minute walks, 3 times a day.Twice a week, work to increase your muscle strength. Use stretch bands, do yoga, heavy gardening (digging and planting with tools), or try push-ups. Check your feet every day for cuts, blisters, red spots, and swelling. Brush your teeth and floss every day to keep your mouth, teeth, and gums healthy. Stop smoking. Ask for help to quit. Keep track of your blood sugar. Check your blood pressure if your doctor advises and keep a record of it. At each visit, be sure you have a: blood pressure check,foot check,weight check Two times each year, have an: HbA1C test. It may be checked more often if it is over 7. Once each year, be sure you have a: cholesterol test,complete foot exam,dental exam to check teeth and gums,dilated eye exam to check for eye problems,urine and a blood test to check for kidney problems

Mr. Lybrate-user, your reported blood sugar levels look good, but it is essential to see the hba1c% also, to observe long term control. Weakness and tiredness are usually seen when sugar control is not good. Regular exercise and proper diet are equally important to overcome all such symptoms. You may get a test done to check b12 and vitamin d3 level in blood.

Drink 2-3 glass of water in the morning empty stomach, and 3-4 litres of water during the day daily. Walk for minimum 5 minutes. You may also mix two spoons of isabgol in a glass of warm milk and drink it just before you go to bed. It will help. Thank You

Yes worm infestation is a common problem in children. I would suggest the following: 1. Repeat deworming dose every 2-3 weeks for 4-6 courses 2. Personal hygiene measures eg. Daily bathing, clean & soft towel, clean & loose clothes, clean socks (even if shoes are not worn), clean toys as the child has a tendency to put toys in the mouth, brushing of teeth, hand washing / sanitizer use & so on 3. Good nutritious food 4. In view of the itching, massage of the skin in general & area around the anus in particular, with oil / cream of your choice will be helpful 5. Periodic check by your paediatrician for this as well as immunizations and preventive & promotive health is a good idea.

Hello, the blood glucose levels mentioned are too high. When diabetes is in control fbs should be closer to 100 mg, pp should be 150 to 160 mg/100 ml & hba1c% <6.8. It is clear that her current treatment is totally inadequate. It needs immediate upgradation. Otherwise there is a possibility of developing any othe long term complications of diabetes. If you want specific advise regarding treatment please come in a private conversation with all details like fbs< pp & hba1c% and diet pattern as well as extent of daily exercise, then I will be able to guide.
