Hi Lybtateres, this Ravi Kumar Muppidi (Endocrinologist). Today I am enlightning you about diabetes.

What is Diabetes?

Diabetes means high blood sugar above normal blood sugar. Right now 7 crores people are suffering with diabetes and another 7 crores will become diabetic in a few months or few years. Diabetes are of two varieties Type 1, type 2 and other varieties. Type 1 is when body is fully deficient of insulin and need daily insulin injections to control their blood sugars. So Type 2 is the common variety, most people will suffer with type 2 diabetes. Why this type 2 diabetes is very high in India because of their lifestyle. We have to focus on this lifestyle. We have to keep moving and maintain an ideal weight. The thing is if you maintain an ideal weight for your height then this diabetes wont occurs as common. Daily ten thousands steps you have to walk otherwise your heart rate should be above 130 per minute for 20 minutes a day then that is called exercise so daily exercise and maintaining weight through diet is important so you need to concentrate on diet restrictions, whatever necessary the amount you have to eat. You got diabetes don’t worry you need to maintain diet, exercise and medication. Medication also very important once you have got diagnose with diabetes you have to take tablets. Type 2 diabetic’s patients they need to be aware of diabetes and their blood sugar levels; they need to monitors their blood sugar levels weekly once at least. The blood sugar should below 130 before food and below 180 after food ok.

How to know if you are diabetic?

If you feel thirsty and if you are drinking lot of water then check your blood sugars, So other complaints of a diabetic are not only excess thirst also excessive urine  one or two times in the night, three or four times in the night and excess appetite. You eat a lot of food and still you will be hungry ok. You your weight is overweight and your age is above 30, it’s the right time to check blood sugars. So the next thing is complications. This high blood sugar can affect your eyes causing retinopathy and even nerves causing diabetic neuropathy and kidney causing nephropathy and these heart problems are very common in diabetic people, strokes and other are common in diabetic people. Infections are also more common in diabetic people. So these are the complications of diabetes. Coming to prevention, how can we prevent has tony before lifestyle. So you have to walk at least 20 to 30 minutes a day. Then you have maintain ideal weight, so if your height is 160 cms you should not be more than 60 kgs, if you height is 170 cms you should not be more than 70 kgs. So like this you have maintain your ideal weight and if you got diabetes’s you need to start medicines along with lifestyle modifications.

Hi Diabetes when not controlled causes sexual dysfunction. Keep your blood pressure and blood sugars under control and if your problem still persists you need some.medicines for which you need a private consultation.

1. Do regular aerobic exercise for 1 hour daily (brisk walking, jogging, running, swimming, cycling etc) 2. Reduce weight if overweight 3control diet a) avoid sweets, sugar/honey, milk, milk product excess calorie and carbohydrate rich diet like white bread, cereal, rice, pasta b) avoid all refined food like maida, pasta, starchy food like rice, potato, bakery item, processed food, c) include more fiber like pulse, green leafy vegetable, cucumber, carrot, tomato, j legume. D) avoid sweetened fruit like banana, mango, litchi, chickoo, sapota, seethaphal jack fruit, grape etc. Can have fruits like guava, pineapple, papaya, orange, mousambi, strawberry, watermelon, pomegranate, jack fruit, sweet potato e) avoid white/polished rice, take unpolished rice like brown rice/wheat/jowar/ragi, moong and chana dal. F) avoid ghee/ butter./coconut oil/excess salt intake like papad/pickle. G) avoid red meat. Take fish/chicken. H) avoid alcohol and carbonated drinks like soft drink 4. Adhere to a strict diabetic diet by consulting dietitian 5. Monitor your fasting, pp blood sugar 3 monthly and hba1c six monthly 6. Consult diabetologist for advice and treatment.

I have type 1 diabetic. I am taking human Actrapid 3 times before meal (10, 8, 6 units) and Lantus 16 units at night. But still my sugar level is not controlled. Can I replace Actrapid with Mixtard 30/70 or 50/50 3 times daily (no Lantus also ).

I am pre-diabetic patient. My fasting sugar level is remaining in the range of 123-127 & PP 156-176 for quite long time. A1c is remaining in the range of 6.3 to 6.8. Should I need to start medicine? My BP, Lipid profile, creatinine normal. Please advise.

No not now ..just follow these simple tips first 1-Losing Extra Weight: helps lower your blood pressure and cholesterol too. waist of more than 80 cm in women & 90 cm in man are at risk 2- Constant and regular food timings :improves metabolic activity and helps reducing weight. 3- Split meals and eat in moderate amount 4- No long Gaps and skipping of meals:-Never miss out breakfast. 5- No overeating:-Never eat in a bulk on one time Remember to eat food slowly & chew well. 6- Proper Exercise and work outs: regular brisk walking for 30 min. five times a week is best .Cycling, swimming, dancing can also be done. Do not eat right away after a workout . 7-constant and regular timings for medication 8-Avoid fried foods ,meat,fats and sweets.  9- Take high-fibre and complex carbohydrates:-like oats and include high-fiber carbohydrates like wheat bran, flaxseed and methi seeds to your wheat flour .Add whole grain foods in your diet. Avoid white bread, pasta, pizza , candies, and snacks . 10-Drink enough fluids:-Keep your body well hydrated.  11.Fresh vegetables and fruits: .Include sprouts in the diet as they are easily digestible

As an allopathic doctor and cardiodiabetologist, I would recommend you to take allopathic antidiabetic and antihypertensive medicines. Dont mix allopathy with other disciplines of medicine. U may send me your all current reports, blood pressure privately. I will get back to u.
