Intellectual Disability - Understanding What Is It!

As parents watch their children grow, they take pleasure in seeing their kids perform simple activities like crawling, rolling, talking, walking, etc. However, a child’s intellectual development could be hampered, and they could have intellectual disability (ID). While the symptoms are obvious during infancy in some children, others show symptoms only during their schooling period. There are associated mental conditions like seizures, mood disorders, vision/hearing problems, autism, etc.

In medical terms, a child is said to be affected by ID if his/her IQ is less than 75. Normal IQ ranges between 85 and 115, with the average being 100. There are various tests that a doctor would do to test if the child’s IQ is affected. The following are some milestones that the family can observe to help in diagnosis.

  1. Rolling, sitting, crawling, and walking are all delayed
  2. Improper speech
  3. Slow to learn things like toilet training, dressing, feeding, etc.
  4. Tends to forget things easily
  5. Throwing tantrums to get what they want
  6. Lack of logical thinking, problem-solving, and connecting actions with results

Causes: While the exact aetiology is not established, in 2/3 affected children, ID is the result of when the brain development is affected. Some of the causes include the following:

  1. Genetic conditions like Down syndrome
  2. Alcohol or drugs or smoking abuse, malnutrition, and infections during pregnancy
  3. Issues during childbirth, including forceps delivery, reduced oxygen levels, or premature birth
  4. Illnesses during early stages of life, including whooping cough, meningitis, head injury, toxic substances exposure, severe abuse, and malnutrition

Diagnosis: In addition to IQ tests, other structural abnormalities can be diagnosed using magnetic resonance imaging (MRI) and an electroencephalogram (EEG) to look for evidence of seizures, etc. Observing the child, interviewing the parents, and testing the child can together help in confirming the diagnosis.

Management: The family has to work together to get the child to lead a normal a life. Speech therapy, occupational therapy, nutrition counseling, physical therapy, etc., are all available as support systems to work on improving various aspects. The parents play a critical role and in addition to above, they can do the following:

  1. Learn about the condition as much as possible and support the child.
  2. Allow the child to become independent, even if he/she is making mistakes. Allow them to make mistakes until they learn the right way of doing things. Coax them into taking feedback and learning new things.
  3. Get them involved in group activities like arts, sports, etc.
  4. Keep in touch with the teachers so that you are aware of their progress in school.
  5. Reinforcing learning at home is very essential.
  6. Form support groups with similar parents and caretakers.
  7. ID is definitely not easy to manage in a child, but there is definitely a lot of help available compared to what was available earlier.

Can Seasonal Affective Disorder (SAD) Be Dealt With?

Are you suffering from seasonal depression triggered by the change in season? This condition is known as seasonal affective disorder or SAD and primarily occurs during the winter. It usually starts when the days start getting colder and shorter. SAD requires treatment as it limits an affected person’s ability to smoothly carry out his or her everyday activities. Here are the several methods by which SAD can be treated or dealt with:

  1. Light therapy boxes: Light therapy boxes emit light which mimics sunshine in a particular way. This helps in recovery from SAD. The light from these therapy boxes are brighter than normal light bulbs and are of various wavelengths. The patient is required to sit in front of a light box for a period of 30 minutes every day. As a result, the circadian rhythms of the body get stimulated and the natural release of melatonin is suppressed. Light therapy is very effective when undertaken in the morning after waking up.   2. Dawn simulators: Dawn simulators help in the management of SAD. These are devices which work as alarm clocks. However, instead of waking you up abruptly by means of a beep or loud music, they produce light which increases in intensity gradually, just like the sun. There are several models of dawn simulators which are available in the market. You should choose a dawn simulator, which uses a full spectrum light. This form depicts natural sunlight. For people with mild SAD, dawn simulators are as effective as light therapy.
  3. Consult a doctor: SAD is a type of depression and it is recommended that you to consult a psychiatrist. Several screening questions are asked to a patient to determine his/her level of depression. A doctor will also observe whether your depression is because of SAD or any other factor.
  4. Antidepressants: In case, psychotherapy or light therapy fails to cure your SAD and the accompanied depression, you should take prescribed antidepressants. These medicines might help in overcoming your seasonal depression. Try to avoid medicines which make you feel sleepy. If you have SAD, you should take antidepressants every winter and not just during the winters when you feel the blues.

Bipolar Disorder - Things You Must Know!

We all have mood swings, but not all mood swings have the same intensity. People who have extreme mood swings ranging from depression to manic highs are said to be suffering from bipolar disorder. These extreme moods are known as episodes and usually last for a few days of even a few weeks at a time. A person suffering from bipolar disorder could have an episode several times a year. This is a very common psychological problem and can affect both children and adults. Though it cannot be cured, this disease can be managed with medication.

There are no known causes for bipolar disorder but genetics may play an important role in triggering this disorder. Hence, if a parent or sibling suffers from this condition, you have a risk of developing it as well. An imbalance in the neurotransmitters in the brain may also play a part in the development of this condition. Stress or traumatic events like the loss of a loved one can also increase your risk of suffering from this condition.
Given that mood swings affect almost each one of us, bipolar disorder can be difficult to diagnose. Some of the things to look out for are:

  1. Change in self-esteem
  2. Unusual talkativeness or quietness
  3. Being increasingly prone to distractions
  4. Decreased need for sleep
  5. Agitation
  6. Recklessness in terms of spending decisions, relationships etc

It must be kept in mind that a person must display at least 4 of the above symptoms simultaneously to be diagnosed as bipolar. Also, none of these symptoms should be a result or side effect of any kind of medication, drugs, alcohol or other intoxicants.

There are four main types of bipolar disorder.

  1. Bipolar I: In such cases, the person has at least one episode of elevated moods and associated abnormal behaviour in his or her life. This may be preceded or followed by depressive episodes.
  2. Bipolar II: In these cases, the person must have suffered from at least one depressive episode. It is very similar to bipolar I except that in bipolar II the highs never reach the manic stage.
  3. Rapid Cycling: Some people can have 4 or more manic and depressive episodes a year. In such cases, they are said to be rapid cycling. This condition affects 10-20% of bipolar cases.
  4. Mixed Bipolar: Usually, bipolar patients show mood swings that alternate between manic and depressive moods. However, sometimes a person may experience both elation and depression simultaneously or in rapid succession of each other. These cases are known as mixed bipolar.

While it is normal to ruminate and most people spend some time ruminating every other day, rumination disorder is quite different and is seen as a mental condition. But what really happens when a person has rumination disorder?

With rumination disorder, sometimes known as mercycism, a person can be expected to bring up food that has been swallowed so that it can be re-chewed and then swallowed again. While this is generally the case with most people who have it, there can be some cases of rumination disorder in which the person spits out the food that has been eaten already.

But what exactly causes it? Experts have not been able to point out a single or a set of causes leading to this condition. However, it is commonly seen in babies and it is perceived that if a baby is not given the sort of attention that is required, it is his/her way of soothing himself or herself.

Since rumination disorder is usually tied to a lack of affection for a baby, it can be said that allowing someone other than the parents to feed him or her can have a good effect. If the person who is experiencing rumination disorder happens to be an adult, then whenever there is a feeling of a need to engage in rumination, a small amount of chewing gum can be provided so as to suppress this need.

When it comes to health and wellness ‘prevention is better than cure’. However, in this case, there is no one thing which can be done in order to successfully prevent rumination disorder from affecting somebody. This is probably due to the fact that there are no explicit causes known. Having said that, if a parent shares a bond that is strong with the baby, it can go a long way in possibly reducing the chance of the baby being affected by rumination disorder and also, stave off other issues which occur when a child does not get enough attention.

However, do note that it is not true that a baby who is ruminating would continue to do so for the rest of his or her life. What is scary about rumination disorder is the fact that a person who engages in it is threatened by dehydration and malnutrition, and this is exactly why an effort should be made to get it treated as soon as possible.

It is often said that a human being is a social animal. While this is true, does this mean that all degrees of social attachment are completely fine? Honestly, no. In fact, attachment displayed by children, to a very great extent, can have an unhealthy impact on the child’s psychology.

Children are sometimes affected by attachment disorders. This results in the child not being able to manage emotions to a sufficient degree. In addition to this, a child may suffer from a lack of self-worth due to not being able to connect on an emotional level with another person. Dependent personality disorder (DPD) is a frequently diagnosed personality disorder that is characterised by a strong need to be taken care of and a fear of being abandoned by important people in the patient’s life.
Symptoms include long-term dependent and submissive behaviours to elicit caregiving in others. The disorder is found equally in men and women.

Common signs

  1. Emotional dependence is one of the most common signs of DPD. Other signs are-
  2. Needy, passive, and clinging behaviour, and an intense fear of separation
  3. Inability to make everyday decisions without other’s advice and reassurance
  4. Avoidance of adult responsibilities
  5. Acting passive and helpless all the time
  6. Oversensitivity to criticism
  7. Lack of self-confidence
  8. Difficulty being alone
  9. Willingness to tolerate abuse from others


Causes of Dependent Personality Disorder ‚Äč(DPD)
Chronic physical illness or other psychological disorders like separation anxiety disorder in childhood can predispose a person to the development of dependent personality disorder. The exact cause of DPD is not known but several factors are involved like genetic, behavioural, psychological and temperamental.

Impaired parenting styles may also be to blame.
However, research suggests that no single factor is solely responsible rather, it is the complex and likely intertwined nature of all three factors that are important.

Treatment
Many people with a dependent personality disorder don’t seek treatment till it starts to significantly interfere or impact their lives. This usually happens when a person can’t cope with stress due to life events.
People with DPD are more prone to develop depression or anxiety symptoms.

Treatments for DPD are focused around psychotherapy, a type of counselling. Its goal is to help a person become more active and independent and to learn to form healthy relationships. The psychologist prefers short term therapy with specific goals when the focus is on managing behaviours that interfere with day-to-day functioning.

Assertiveness training helps the person with DPD develop self-confidence and cognitive-behavioral therapy (CBT) helps him or her develop new attitudes and perspectives about themselves that are positive. However, change in someone's personality structure usually requires long-term psychoanalytic therapy. In this type of therapy, childhood experiences are examined for formation of defence mechanisms, coping styles, and patterns of attachment in relationships.

DPD is the most difficult personality disorder to treat because of the patient’s strong need for constant reassurance and support. It is also very important for the doctor to monitor patient dependency.
Group therapy can also be helpful, although care needs to be taken so that the patient doesn’t use groups to reinforce dependent relationships in his life. Medication is also used though it’s not very effective. It’s not fun to be a sex addict. The only difference between chemical addiction and sexual addiction is that a sex addict craves sex and not a chemical.
To a sex addict, a sexual act has to be done without regard to potential dangerous consequences. Like other addicts, sex addicts also use sex as a quick fix to relieve anxiety, loneliness, pain or stress.

Understanding sexual addiction
Also called compulsive sexual behavior, hypersexual disorder, nymphomania or hypersexuality, sexual addiction is a state where one is obsessed with sexual thoughts, feelings and behaviours to the extent that it interferes with one’s work, health, relationships and daily activities. The patient follows a routine of acting out sexual desires, followed by denial, shame, despair and confusion. Sometimes but not always, sex fantasies extend to abnormal sexual behaviors that are considered immoral or illegal.

This cycle has to go on for at least six months before sex addiction is confirmed. Other signs of sexual addiction
1. Loss of control and addiction
2. Withdrawal symptoms
3. Lack of emotional attachment

Fortunately, just like other addictions, sex addiction can also be managed.

Diagnosis
Only a trained therapist can diagnose sex addiction. There are a few conditions that can produce the same symptoms as hypersexuality like drug abuse and mental health conditions like obsessive- compulsive disorder and attention deficit hyperactivity disorder. Some sex addicts can be suffering from these mental health conditions concurrently with their nymphomania. That’s why a trained psychologist has to consider the case in its entirety before making a diagnosis.

Treatment
The first step towards treatment of hypersexuality is the person recognizing that they are out of control sexually. Sex addicts can remain in denial until a significant life event like a divorce or a health crisis prompts them to seek treatment.

Many treatment options are available as cures for sex addiction which include outpatient programmes including counseling and residential sex de-addiction programmes. Residential programmes offer patients a lot of motivation and support and as the patient is removed from his environment, triggers and routines, he finds it easier to overcome sex addiction. Goals of a sex addiction treatment center programme typically include:

  1. Commitment to abstinence
  2. Developing healthy sexuality
  3. Rebuilding relationships
  4. Stress management

Treatment for sex addiction usually follows the 12-step model initially developed for substance abuse disorders. Cognitive behavioral techniques help patients to learn thoughts and behaviors that will result in more effective management of their desires and ways to achieve and maintain a healthy, balanced lifestyle. In some cases, medicines used to treat obsessive-compulsive disorders or impulse control disorders can also be used to curb the compulsive nature of the sex addiction. Sleep is known to most as a period of rest and relaxation, but there are multiple disorders which are associated with sleep too. Hypersomnolence or hypersomnia is one of the most common sleep disordersThis is characterised by excessive daytime sleepiness and prolonged periods of sleep at night. They wake up fresh in the morning, but can easily go back to sleep any time of the day without feeling strained or tired. There is again no freshness associated upon waking up. Funny as it may sound, they can doze off even during a meal or in the middle of a conversation.

Causes: This condition is most common in men and in the early adolescent days.

  1. There is no underlying cause that has been identified, though there is a genetic predisposition.
  2. People with history of head trauma are more affected
  3. People with anxiety, depression, bipolar disorders, epilepsy, and other mental disorders are also more likely to be affected
  4. People with sleep apnea often are hypersomnolence
  5. Restless legs syndrome, narcolepsy
  6. Neurodegenerative disorders like Parkinson’s and Alzheimer’s tend to sleep more.

Symptoms:

  1. Sleeping for longer hours at night time
  2. Excessively sleepy during daytime
  3. Able to doze off anytime during the day
  4. Inappropriate and involuntary sleeping pattern
  5. Having difficulty getting out of bed after waking up, even after prolonged sleep
  6. Feeling of disorientation
  7. Feeling agitated, anxious, lethargic, and irritated
  8. Impaired cognition and memory
  9. All these lead to extreme state of drowsiness
  10. Increased tendency for hallucinations
  11. Reduced appetite and even weight loss in some people
  12. There is restlessness, reduced pace of thinking and speaking
  13. Overall reduced energy levels
  14. Social interactions are affected including work life, social functions, etc.

Diagnostic criteria:

  1. If a person happens to sleep more at night or falls asleep during the day easily for more than 3 times a week, it is usually concerning.
  2. The added feature of no tiring physical activity can also lead to suspicion
  3. In acute cases, these symptoms are present for at least a month and in chronic cases, it could be seen for up to 3 months
  4. It cannot be explained by lack of sleep on a regular basis
  5. One or more of the above conditions could be present, which can help in making the diagnosis

Treatment:
Most cases of hypersomnia would need treatment only to improve their energy levels and improve the quality of life. Very often, treating the underlying condition can lead to improvement of hypersomnia. The tired, lethargic, restless feeling affects the overall performance of a person, leaving them with a lowered self-esteem. Mood disorders are common psychological problems that affect a vast portion of the human populace all over the world every year. These occur when there is a shift or imbalance in a person's psyche which affects his or her daily function and interpersonal relations in varying degrees. Each individual is affected by these conditions in a different capacity and responds to them in a distinct manner.

Mood disorders generally manifest themselves as fluctuations in a person's temperament. It is triggered by various factors. These mental illnesses range from mild to severe, the latter requiring urgent psychological counseling and treatment. A mild condition may even develop into a painful and chronic one without any form of remedial intervention.

What are the most common types of mood disorders?

There are many distinctive categories of mood disorders that can affect a person. The most commonly occurring ones are as follows:

  1. Major depressive disorder
  2. Bipolar disorder
  3. Dysthymia
  4. Cyclothymia
  5. Seasonal affective disorder (sad)

Major depression is the most frequently occurring type of mood disorder. It causes mental and emotional turmoil, as well as physical anguish. Those affected by it may experience a single episode in isolation or multiple episodes over a period of time. The milder version of this condition is known as dysthymia.

Bipolar disorder is a comparatively rare condition that causes alternating states of extreme mania and depression. It is also known as manic-depressive illness. The mild version of this is called cyclothymia.

Seasonal affective disorder or sad is a depressive disorder that is known to affect those living in colder climes. It is caused generally by the lack of warmth and light, the onset of which begins in late autumn and lasts till the end of winter.

What causes a mood disorder?

Mood disorders are very difficult to diagnose as they can be caused by an assortment of triggers and the symptoms are not always apparent. This is particularly true in the case of dysthymia and cyclothymia.

Some of the usual factors that lead to mood disorders are:

  1. Chronic stress and anxiety
  2. Anomalies in the functioning of the nervous system
  3. Prolonged periods of isolation
  4. Genetics

Recently most of us were shaken by the death of the well-known television actress, Pratyusha Banerjee. Her sudden death has brought to light the very common yet under-addressed problem - Depression -  that a number of youths are living with.

Most mental and emotional health issues are inconspicuous in nature and, therefore, cause more harm than they otherwise should, most people still live with a number of symptoms of depression without even realizing it or do little to address it. When the depressive symptoms last for more than 2 weeks and affect the individual's personal, social or occupational life, it is known as clinical depression or major depression. There is often a delay in opting for the treatment because of the social taboo associated with depression.

Although cases of depression can vary from an individual to the other, there are signs that one can look out for, to identify a depressed person:

  1. Low mood most of the time of day along with frequent crying spells
  2. Feeling of unworthiness and low self-respect are very common in a depressed person. Due to these feelings, the affected person may become very critical and disapproving about himself or herself.
  3. Lack of enjoyment and loss of interest in a depressed person makes them no longer derive pleasure from things that once gave him or her happiness.
  4. Exhaustion and insomnia are signs that one should look out for in a depressed individual. These are usually due to changes in sleep patterns, from sleeping a lot because of feelings of constant fatigue or due to instances of wakefulness.
  5. From indulging in comfort eating to losing the will to eat, predominantly loss of appetite characterizes a depressed person.

  7. Unexplained bodily pains and aches are another characteristics of the condition.

The risk of a mental illness increases, if one or more of your family members have a mental disorder, but this increased risk doesn't guarantee that you will develop a mental illness. Not only have some mental illnesses been found to be hereditary in nature, but certain studies have found that some major mental illnesses can be traced to the same genetic variations.

Some hereditary psychiatric disorders are:

  1. Obsessive compulsive disorder: Obsessive compulsive disorder is a disorder of the brain and behavior. A person suffering from OCD has uncontrollable, recurring thoughts and behaviors they cannot stop repeating. Ocd also causes severe anxiety in such people. In 2000, a study conducted by researchers in Washington d. C. And Baltimore concluded that having one or more ocd family member (s) may increase the chances of you developing it too. In 2010, a study tracked down possible chromosomes that may be responsible for OCD.
  2. SchizophreniaSchizophrenia is a chronic mental illness that affects and dictates how a person feels, thinks and behaves. Those suffering from schizophrenia may lose touch with reality and experience delusions, hallucinations, thought disorders and movement disorders. It has been seen that people who have an identical twin with schizophrenia are 50% more likely to develop the disorder and those who have one parent suffering from schizophrenia are 18% more likely to inherit the illness.
  3. DepressionMajor depression or clinical depression is a common though severe mood disorder. It is characterized by an episode of sadness or apathy along with other symptoms that last at least for two consecutive weeks. Those suffering from depression may feel helpless and worthless, lose all interest in daily activities or activities they used to enjoy and feel unable to take part in normal day-to-day activities. Other symptoms of depression also include sleep pattern and appetite changes, chronic fatigue, concentration or focus problems and physical discomfort. Some patients with depression may become suicidal.

In 2011, a study pinned down a specific chromosome that may trigger depression development. Research on the hereditary properties of depression within families shows that some people are more prone to develop the disorder than others. If you have a parent or sibling that suffers from depression, you might be 1.5 to 3 times more likely to develop depression than those who do not have a close family member suffering from this condition. Sleep is one such time when people usually are expected to lie down still and get rest. However, we have all heard of sleepwalking, a condition where a person walks during the sleep. Though it may sound strange, there is a deeper explanation for it both from a causative point of view and from managing it.

Things you should know?

  1. Sleepwalking happens when a person moves back from a deep sleep to a light sleep or awakening state.
  2. The person who is sleepwalking is usually not aware of it.
  3. Activities may range from simply getting up and sitting up in bed to walking around the room. They could also open the door and walk out to the neighbourhood. Moving furniture, changing dresses or driving a car, may also be some of the actions Most of these activities happens completely without their knowledge.
  4. Mostly happens in children up to the age of 12, but can be seen in adults also, where it assumes a more severe form.
  5. The person who is sleepwalking has a fixed stare with glassy eyes. They may appear dazed and lost when they are awakened.
  6. They may not respond when they are actually sleepwalking, or respond very slowly
  7. They can be brought back to bed and put back to sleep without being disturbed. Most children would go back to sleeping when this is done
  8. Though the parents can be very worried when they see children sleepwalking, reassurance is required, as it usually disappears as they cross teenage.
  9. There could be chances of small injury like tripping or fall during the sleepwalking episode
  10. Sleepwalkers may be more restless compared to other children during their waking hours
  11. Bedwetting is also quite common in children who sleepwalk

Causes

  1. Inappropriate sleep pattern with lack of sleep for prolonged periods is one of the main reasons for sleepwalking
  2. Stress
  3. Excessive drinking
  4. Medical conditions like heart rhythm problems, acidity, gastric reflux, and seizures
  5. Psychiatric conditions like panic attacks and post-traumatic stress disorder can also lead to sleepwalking

Management

  1. For a parent to see their child walking around in their sleep can be quite disturbing.
  2. Reassurance is required stating that it is just a temporary phase and will not stay beyond the teenage years
  3. Most people do not require any intervention unless accompanied by severe symptoms like going out of the house or driving
