Atopic Dermatitis - What Causes It And How Can It Be Prevented?

Atopic dermatitis is an inflammatory skin disease that most commonly affects children during their first year of life. This is also commonly noted in older children and occasionally in adults. This disease causes severe itching that many a times leads to skin infection. These rashes are commonly seen over the scalp, neck, cheeks and face. As the child grows older and also in adults, the rashes mainly appear in body creases and folds.

Aetiology-

The exact cause of atopic dermatitis is still under research but it is believed that people with the following are at risk of getting atopic dermatitis-

  1. People with asthma or hay feve r
  2. Living in cities and in coldclimate
  3. Family history of dermatitis

Dermatitis is not caused by food allergies but people with dermatitis are allergic to certain type of food like milk and other dairy products, shellfish and nuts.

Symptoms-

  1. Dry itchy and scaly skin
  2. Redness and rashes
  3. Scratching and itching
  4. Appears bumpy and develop knots in thickened areas

Complications

If left untreated, atopic dermatitis may results in:

  1. Asthma and hay fever: More than half of the children with atopic dermatitis develop asthma and hay fever by age 13.
  2. Chronic itchy, scaly skin: Atopic dermatitis can cause the affected skin to become discolored, thick and leathery.
  3. Skin infections: Repeated scratching that breaks the skin can cause cracks and open sores. These increase the risk of infection.
  4. Irritant hand dermatitis: This especially affects people whose core work requires their hands to be wet and exposed to detergents, harsh soaps, and disinfectants.
  5. Sleep problems. The itch-scratch cycle can result in poor sleep quality.

Treatment-

It’s important to visit the dermatologist for treatment to control the disease and prevent it from worsening. Treatment includes medicines to control itching, reduce skin inflammation (redness and swelling) and clear infection.

Prevent Relapse-

The dermatologist will also suggest lifestyle modifications which are very important to manage the disease.

Tips for people with dermatitis-

Ways to ease discomfort :

  • Make sure the humidity and temperature levels are comfortable.
  • Apply moisturizer twice daily to make sure the skin does not get dry and cracks.
  • Enquire your doctor about how to use wet wrap therapy and its effects.
  • Make sure your child’s finger nails are cut short and smooth.

Bathing care -

  • Bath your child with warm water not with hot water and make sure you complete the bath within 5-10 mins.
  • Just pat the skin after bath with soft cotton towels and don’t make it dry completely.
  • Apply the medicine if advised after bath. Make sure you apply the medicine before the skin gets completely dry.
  • Apply moisturizer over the medicated skin.
  • Avoid bubble bath and prefer bleach bath every two days if the skin gets infected.

Choosing right clothes-

  • Uses limited amount of detergents to wash the clothes.
  • Rinse the clothes with adequate amount of water and dry it completely under direct sunlight.
  • Wash new clothes with plain water before wearing to your child to remove excess dyes and fragrances.

. Watering since birth may be due to mild infection or congenital non-canalization of nasolacrimal duct (COND). Watering due to mild infection will disappear with antibiotic eye drops within a week but if there is COND,it is not going to disappear with antibiotic eye drops, for this you should consult experienced ophthmologist as earliest possible to learn how to do lacrimal sac massage by his or her mother while lactating,it gives the hundred percent result if done before 6 months old ,as the age increases , effects of lacrimal lacrimal sac massage decreases & one has to go probing under G.A.&if not successful after repeated probing ,one has to go for DCR in later life.

My baby is 1.5 month old, he is passing green stool, he is on formula and breast milk both.
You have to stop the formula and if it is given in bottle you must stop bottle feeding and give it with spoon or a sipper . Give only breastmilk for a healthy baby

Hi doctor ,I am a mother of 1 month old baby boy .early days after my delivery I didn't get milk .after a week I get milk .but my boy after sucking 1 hour he still get hungry so I include scaling formula milk. Is it safe n I give formula milk in bottle bt after having some milk he throws out some milk .why.

MBBS, DGO - Preventive & Social Medicine
Gynaecologist, Sri Ganganagar
A mother usually produces enough milk that can satisfy hunger of their babies. Its all that faulty feeding technique and duration and timing of feed. Look into it and avoid formula milk.

Preserving stem cell (cord blood) is a good thing. But the so much hyped benefits are not absolutely true. Research works are going on, so if affordability is not a barrier, you should go for it.

Two Main Causes Of Erythroblastosis Fetalis!

MBBS, MS - Obstetrics & Gynaecology

Two Main Causes Of Erythroblastosis Fetalis!

What is erythroblastosis fetalis?
Erythroblastosis fetalis is also known as haemolytic anaemia in the newborn. This occurs due to blood incompatibility in the mother and foetus. Due to this incompatibility, the antibodies present in the mother’s blood, will pass through the placental barrier and attack the blood cells of the foetus. This will lead to the destruction of the red blood cells of the foetus and it is likely to cause anaemia in the foetus. This condition varies from mild to very serious. In its moderate or severe stage, the erythroblasts or immature red blood cells are formed in the blood of the foetus and this disease is called erythroblastosis fetalis.

Why does it happen?
The two main causes of erythroblastosis fetalis are Rh incompatibility and ABO incompatibility.

  1. Rh Incompatibility: When the mother is Rh –ve and the father is Rh +ve, there is a good chance for the baby to be Rh +ve. The antigens present in the blood of the Rh +ve baby will behave like a foreign agent and the mother will produce antibodies against it. If it is the first pregnancy, then the child may not be at risk, however, if the second child ends up being of +ve blood group again, then the antibodies present in the mother’s blood will attack the baby and may result in a spontaneous abortion.
  2. ABO Incompatibility: This usually occurs when the mother’s blood group A, B, O does not match the baby’s. This causes fewer complications in comparison to Rh incompatibility, but it may be severe if the child has a very rare blood group.

How to avoid it?
It is a highly preventive condition. Firstly, you have not tested your blood group; it is advised to get it tested along with the blood group of the father. If you already know your blood group, then you must mention it to your doctor. If the father has negative blood group, then there will be no problem. However if the father is Rh positive, then it is advised to get routine tests done by the doctor.
The other preventive measure to take is a treatment called RhoGAM or the Rh immunoglobulin. It reduces the reaction of the mother to the baby’s blood cells. This shot is administered around the 28th week of the pregnancy. It is also administered 72 hours after the birth of the baby with the positive blood group.

Please check if he is having intestinal worms. Also note if this is genetic or not. Also in growing age most of the children do look sick and bony but as age advances everything will be fine. Give him a glass of milk daily - do note give any food-supplement - horlicks/bournvita/complain etc.

22 days male baby passing loose motions. After taking medication also. give some suggestions.
