Mr Parv, Exercise for more than an hour a day, like brisk walking, jogging or running or a gym workout is good enough. Plus diet control if you have diabetes, then avoid sweets, sugar, jaggery, junk foods and pastries. Consume more of green leafy vegetables, salads and permitted fruits. Take prescribed medications regularly without fail.

Hi lybrate-user delayed period could be because of hormonal imbalance that is due to deranged thyroid levels. Please get a urine pregnancy test to rule out pregnancy. Mostly it would be negative only as you used double protection.

I have a question about my diabetes. I am suffering from hypertension since 6 years & Diabetes from 2.5 years and take tablets for both diseases in morning. For diabetes I take glimepiride 2 mg + Metformin 500 mg prescribed by doctor. My query is dat when I test my pp sugar after 2 hours of meals it comes around 240 but when I again test after 3 hours of meals (1 hour after pp sugar test), it comes around 150. Means the value of pp sugar test which should come after 2 hours of meals generally comes after 3 hours of meals. Is this ok or should I need further tests or medicine changes.

Dear Ashish, I acknowledge and appreciate this keep observation by you. This is what commonly occurs in Type-2 diabetes. This is reflective of the body's inability to handle the high glucose load of the meals taken due to insulin resistance which leads to higher than desired post meal blood glucose levels. Yes we need to adjust your medicines for this as this is not desirable and can lead to end organ damage in the body. As per lybrate policy, I cannot post medicine prescription in public forum, hence pls get back to me privately

What causes Hashimoto's thyroiditis, and what is the best method of treatment? Can iodine help this condition?

CCEBDM, Diploma in Diabetology, MBBS

Mr. Lybrate-user, hashimoto's disease (ht), is an autoimmune disease where there are antibodies prepared against thyroxine secreting cells, this leads to deficiency of thyroxine hormone and hypothyroidism. There is lymphocytic infiltration of thyroid gland, it becomes firm and lobulated. There are anti bodies against thyroid peroxidase enzyme (anti tpo ab). One of the causes for ht is said to be excess iodine intake through iodinated salt. Most of the patients with ht are likely to have other autoimmune diseases like t1 dm, rheumatoid arthritis etc. Treatment is with thyroxine tablets, iodine has no role in this disease.

Ayurvedic Approach Towards Hypothyroidism

Hypothyroidism is a hormonal disorder caused by reduced amounts of thyroid hormone. It is more common than hyperthyroidism and affects women more than men. Iodine is a main ingredient of the thyroid stimulating hormone (tsh) and iodine deficiency leads to hypothyroidism.

Tsh is required for effective metabolism and reduced amount leads to affecting overall metabolism of the body and causing the following symptoms.

Slow pulse
Weight gain
Reduced attention/concentration span
Intolerance to cold
Hoarseness of voice
Irritability
Inability to concentrate
Depression/low mood
Fatigue
Swollen face and droopy eyes
Dry skin
Mental
Premenstrual tension
Muscular pain
Ayurveda believes that this is due to an imbalance between the 3 body systems - vata, pita, and kapha. In hypothyroid people, the pita is supposed to be overpowered by kapha which reduces metabolism.

Treatment is aimed at bringing out balance between the two.

The various ayurvedic components include:
Panchakarma and external therapies like vamana, shirodhara, virechana, basti, udwarthana, kashaya seka are shown to play a significant role in managing symptoms of hypothyroidism.

Activities:

Specific asanas and pranayamas that are directed at the thyroid have been proven to be beneficial.

The most effective asana is sarvangasana or the shoulder stand. It improves blood flow to the thyroid and thereby improves its function. Matsyasana, surya namaskara, pavan muktasana, and halasana are also beneficial for hypothyroid patients.
Ujjayi is the most common pranayama which acts on the throat area and stimulates reflexes in this area.   nadi shodhana pranayama also is proven to have beneficial effects on the metabolism.
Internal remedies:

Appetizers and digestive agents are given to improve metabolism. These include herbs and whole grains and specific food products including dairy products.

Food changes:

Food has a major role to play, and some of the do's and don't's are as follows.

Increase water consumption, preferably warm water, to improve digestion
Include more iodine-containing foods like white onions, fish, shellfish
Include more whole fibers like brown rice, barley, green gram, bengal gram, cucumber, and sugarcane juice
Increase consumption of milk and milk products
Avoid sour food items
Herbal products recommended for hypothyroidism include kanchanara, (purple mountain ebony), jatamansi, shilajita, gokshura, brahmi, and punarnava
Apply a paste of jalakumbhi which helps by supplying iodine to the goiter directly
Jalakumbhi juice can also be consumed daily (11 to 22 g)
Coconut oil, being rich in medium chain fatty acids, increases the body's metabolism and helps in managing symptoms of hypothyroidism
Lifestyle changes: in addition to the above, meditation and regular exercises to maintain a healthy lifestyle, reduce stress, and manage weight have shown to help control symptoms to a large extent.

Hypothyroidism is a chronic condition but can be managed with a combination of food, exercise, and lifestyle changes.
