Pain in my leg ,some one says uric acid problem what can i do?

MD - Internal Medicine, MBBS
Endocrinologist,
Pain in my leg ,some one says uric acid problem what can i do?
A high uric acid level can be caused when your body either produces too much uric acid or your kidneys don't eliminate uric acid rapidly enough. A high uric acid level may cause increasingly frequent attacks of gout, or it may never cause problems. A high uric acid level may also cause some people to develop kidney stones or kidney failure. And some people with a high uric acid level also develop high blood pressure, heart disease or chronic kidney disease, but it's often unclear whether this is a direct cause or merely an early warning sign of these conditions. Factors that may cause a high uric acid level in your blood include: Diuretic medications (water pills),Drinking too much alcohol,Genetics (inherited tendencies),hypothyroidism (underactive thyroid),Immune-suppressing drugs,Niacin, or vitamin B-3, obesity, psoriasis, Purine-rich diet — organ meat, game meat, anchovies, herring, gravy, dried beans, driedpeas, mushrooms and other foods Renal insufficiency — inability of the kidneys to filter waste Tumor lysis syndrome — a rapid release of cells into the blood caused by certain cancers or by chemotherapy for those cancers,Also, you may be monitored for high uric acid levels when undergoing chemotherapy or radiation treatment for cancer. Avoid protein diets, avoid more milk products. bad habits , fast food/fat/oily,salty foods. Coffee, smoking, alcohol intake. Non-veg., take whole grain, oats, green veg.,plenty of water in day time, 1 glass of water at night before bed.Take tab. Zyloric 100 mg once a day.Take tab. Bencid 500 mg once a day. Tab. I am diabetic & fat also which green tea I should take because there are so many in market. Please advise.

MBBS, MD - Consultant Physician
General Physician, Ghaziabad
I hope you are not thinking that just by drinking green tea you will loose weight. If yes then trust me you won't ever. Take whichever green tea you want. It will just help a bit that too only if you control your diabetes and do exercises. First of all tell me: 1. Your weight and height. 2. Age. 3. Your blood sugar levels. 4. Medicines you are taking. Revert with the asked details for a better help.

Hello lybrate-user, there is no magic that can help to reduce the belly fat. Eating right and exercise is the only thing that helps. Being a diabetic, its necessary to manage your sugars & body weight as a priority. Increase your protein and calcium intake by including nuts, pulses, dals, dairy products, lean meats - fish and chicken, eggs (if non-vegeterian). These will help you in managing the sugars as well as solve the deficiencies.

Mr. Lybrate-user, please clarify who is having diabetes? age of the patient and type of diabetes? these details are not clear. Anyway, if some one has diabetes then treatment has to be started with appropriate drugs. Plus diet control and regular exercise also are required. To suggest any treatment there is a need to know fasting, pp sugar level and hba1c%, dietary pattern and extent of exercise being done daily. It will be better if these details are given. Thanks.

Are Dark Cracks Behind Neck A Sign Of Diabetes?

MBBS, Diploma in Diabetology
Endocrinologist, Mumbai
Are Dark Cracks Behind Neck A Sign Of Diabetes?

Many overweight and obese people have darkened and a rather flaky texture behind neck, darkening in armpits and other parts of body with creases and folds like elbows, knees or knuckles. This is commonly mistaken as smudge of dirt, uncleanliness or lack of hygiene. Some people think that it is due to repeated friction caused by necklace or chains etc. In fact it is a sign of'diabetes' which is already developed or might develop in future.

This darkening is due to excessive cell growth and differentiation of fibroblasts caused by excess of insulin in the body especially in obese individuals and type 2 diabetes patients.

In obese individuals and type 2 diabetes patients excess amount of fat especially abdominal fat (tummy fat) causes resistance to action of insulin secreted by pancreas and tends to hamper its action. In return, body produces more and more insulin from pancreas to keep blood sugar under control. This excess amount of insulin secreted by body promotes this skin cell growth leading to the visible condition called'acanthosis nigricans' in medical terms.

After producing excess amount of insulin pancreas start exhausting, this converts into diabetes. At this stage diabetes can be prevented by simple lifestyle modifications & focussed weight loss.

Tobacco is an unhealthy habit & addiction. It causes the blood vessels to constrict. Also in diabetics, the blood has an increased tendency to clot as the blood is slightly thicker due to increase in the concentration of clotting factors. So a diabetic person with a tobacco habit is at an increased risk to develop clots in heart vessels or brain vessels causing either a heart attack or a stroke or gangrene if the peripheral vessels develop a clot & are blocked. hence quit your tobacco as early as possible.

Firstly your hormonal treatment should be perfect and for that you keep on doing regular consultations and check-ups with your doctor, better, an endocrinologist or a physician with good knowledge of endocrinology. Secondly, you have to perform exercises regularly and may join a gym or better you start playing badminton or table tennis or lawn tennis, which involves exercise, recreation and sport’s talent and creates interest in you while pure exercises are boring. You may also do dancing and swimming. These are also good exercises as well as recreation. Thirdly, diet control or taking balanced diet. Reduce carbohydrates. So, avoid rice, potato and ghuiyan (colocasia). Also reduce wheat. In place take besan’s (gram flour) chapaati. Increase pulses containing proteins. Soyabeen has good amount of proteins. Use soya badi or soya paneer. Use skimmed milk or skimmed milk products. Also avoid meat or fish containing fat and high calories. Prawn are low fat high protein diet. Also decrease overall intake of food. Take lot of salad like cucumber, tomato etc to quench your hunger. Take plenty of low fat green vegetable dishes like loki, tori, shaljam, paalak, mooli, shimla mirch. Also use green tea. Take plenty of water frequently. Avoid sugar or sugared tea or coffee, chocolates, sweets and very sweet fruits like grapes. You may take seasonal fruits like amrood, ev (apple), banana in limited quantities.

I am a hypothyroid person. Please suggest me some exercises and how to meditate? As I have never done these before.

I am a hypothyroid person. Please suggest me some exercises and how to meditate? As I have never done these before.
Lybrate-user, meditation and exercise both are really good to maintain a good health. But it is equally important to control hypothyroidism effectively. For that you should maintain tsh levels between 3 to 4 mili units/l. Otherwise all the symptoms will not be in check. Exercising for 1 hour every day will help in maintaining a near normal weight, otherwise hypothyroidism will lead to increased weight. The best exercise to begin is brisk walking for 1 hour or more every day. A very simple and easy to do. Otherwise, if you are willing to do a bit more then better to join a gym and do various aerobic activities and strength building exercises under the guidance of a qualified professional instructor. This will help you get a real hang exercise and you will enjoy. As regards meditation, there are many methods which are advocated. One way is to sit in a comfortable chair taking full support to your thighs, using the full width and depth of the chair, then concentrating your gaze on an object at 45 degrees in front of you, taking 6 deep breaths and then closing your eyes. Then instructing your self that your whole body beginning with scalp is relaxing completely, reaching slowly down to the toes. Once you do this say to your self that" I have relaxed completely, every single cell, every muscle, bone and joint everything has relaxed. I am feeling wonderful. I am enjoying this lovely feeling of relaxation. You may imagine or visualise about a beautiful place that you have visited, bring it right in front of your eyes, feel you are there. Stay in that mood for over 10 to 15 minutes or longer. While being in this situation give some positive instructions/ suggestions to yourself about what you want to do, or objectives, goal, immediate and future ones. Say to your self, that your going to succeed. Just enjoy that lovely feeling of relaxation. Say you are healthy, you will over come the problems that you face including hypothyroidism. Will have great success in life. Then after a stipulated time count 1 to 6 and open your eyes. This is one way of relaxing or meditating. There other ways like spiritual meditation. Try any thing that suits you. All the best. Thanks.

You have not felt any pain due to nerve weakness as a result of neuropathy. Infection sets in if there is a wound as you have no wound, chances of infection is minimal. Can you send me the list of medicines you take and reports of your fbs, 2hr ppbs and hba1c for better control. Are you taking vitamin b neuro supplements? tell me details privately.

MBBS, CCEBDM, Diploma in Diabetology

Mr Tasveer, Is he on treatment for hypothyroidism? If yes what is the dose being taken? Secondly the level mentioned by you of 179 is of what, plus there is no mention of any unit. So difficult to comment. If you have any doubt please get the test repeated, that will clarify the situation. Are you referring to TSH (thyroid stimulating hormone)?
