I am diabetic. I use to take 10iu daily before break fast. My fasting sugar level is some what normal. But pp level is beyond my control even after taking mixtard insulin. How can I control that.

Hello, taking right dose of insulin is the best way to control fasting and pp sugar. Even if after taking insulin, pp sugar goes high, you can try tab. Vobit 0.2mg one after food twice daily to control pp sugar. Check fasting sugar after seven days to find control.

Hello, diabetes mellitus when present for long time and if not well controlled causes a number of chronic complications including peripheral neuropathy. This condition produces pricking and burning sensation in feet. One of the prime requirement is strict blood sugar control like fasting around 100 mg, pp 150 to 160 mg and hba1c%<6.8. Consistently well controlled diabetes may not produce serious consequences. Plus to reduce the symptoms there are some medications available like methylcobalamine & combinations with other vitamins, plus other drugs. The very concept of heat in the body causing various problems it self is incorrect. Our body is warm, if any extra heat is produced that comes as fever. Thanks for the question.

Increase her zinc content and iron content of the diet. Non veg are excellent sources of both these nutrients. This will reduce her fatigue

My sugar level is at 200 before breakfast inspite of having regular medicines, Sulicent and thumb and 10 units of huminsulin in the morning as advised by a diabetologist one year ago. Of course I am having normal food and morning and evening tea without sugar, Can you please suggest any modifications.

Mr. lybrate-user, Thanks for the query. Ideally when one is taking treatment for diabetes then blood glucose should be controlled as follows: Fasting < 100 mg, PP 160 to 180 mg & HbA1c% <7. This will help the person to prevent or postpone development of severe complications of diabetes. Fasting 200 mg is too bad. What is the usual PP glucose level and HbA1c%? It appears that current treatment is inadequate, plus your dietary intake is more and there appears to be no physical activity. To burn extra glucose daily exercise for over 1 hour is needed. At your age at least 5 days a week exercise is needed. Having excess weight is another reason for high glucose levels. So you will have to look at all those aspects. Thanks.

U need complete evaluation with epilepsy protocol mri. Why do you want operation if it can be treated with medicines. Brain operation is not without risks. If its medically refractory and surgically curable I may be of help. Whethet it can be cured by surgery we will only know after thorough evaluation.

Dietary management for diabetics:

CHEW MORE, EAT LESS : Studies show that the longer time you spend chewing, the lesser calories you consume. Chewing for longer prevents over-eating, by giving the brain more time to receive signals from the stomach that it is full. It is not only great for digesting, but it also helps in limiting your portion size. Try chewing your food 35 to 50 times per mouthful.

DON'T SKIP BREAKFAST : No matter how late you are running to work, avoid missing breakfast. There is a reason why it is called the most important meal of the day. Your body (rather your brain) expects to be refueled a few times each day, so when you skip breakfast, you feel so hungry that when lunch time comes, you overeat or maybe choose foods items that are not the healthiest choices. Missing breakfast slows down your metabolism. More importantly, always eat your breakfast, within an hour of waking up, as your body has gone without food the entire night.

CONCENTRATE ON YOUR FOOD : How many times has your mother reprimanded you to not talk, read or watch TV while eating food? Not without a reason. When you are multitasking during meal times, your brain isn't focussing on the amount of food that you are consuming and you are more prone to piling on pounds. This doesn't mean that you cannot enjoy a meal with your friends. You can do so, but just pay attention to what you are putting in your mouth!

DON'T OVERCOOK YOUR MEALS : Overcooking food kills or reduces the number of nutrients your meal contains, and when you don't get enough nutrients, you don't feel satisfied and soon start to get junk food cravings. To combat this, try eating more raw foods like sushi and salads. Steam, bake, blanch or grill vegetables and grill or bake meat and fish. Avoid microwaving.

FRUITS BEFORE MEALS : It is advisable to eat fruits at least 30 minutes before any heavy meal. This way, the fruits will digest quickly. Eating fruits on an empty stomach detoxifies your system and supplies you with a great deal of energy for losing weight.

HAVE SMALLER MEALS, MORE OFTEN : Ideally, you should eat five to six times per day, with a two to three hour break between each meal or snack. Eating more frequently gets your metabolism going and keeps it working at an elevated level continuously throughout the day. But don't use this rule as an excuse to overeat.

AVOID EATING AFTER 8 PM : It's good to eat your last meal before eight in the evening. This way you don't land up binging on a snack before dinner time. If you find it hard to resist, have some herbal tea or brush your teeth after dinner immediately to switch your mind off from the idea of eating. THINK AND DRINK : While everyone knows it's necessary to drink at least eight to 10 glasses of water, very few know when one should consume it. When you drink is as important as what you drink. If you drink water in-between meals, the liquid slows down digestion, diluting the acid in your stomach and so the nutrients from the food that you eat are not well absorbed. To avoid this try to drink water 15 minutes before or after a meal, and if you really need to drink while you eat, limit yourself to small sips only.

Mr. lybrate-user, Thanks for your query. Diabetes affects small and large blood vessels, particularly when the disease is present over a long period (over few years). Therefore, erectile dysfunction (due to affection of blood supply to penile muscles), is a common complication. This is seen oarticularly rapidly when disease is uncontrolled with high blood glucose levels and even in well controlled disease after a few years. There is really no permanent cure. Most important is strict blood glucose control with FBG <100 mg, PP 150 to 160 mg & HbA1c%<6.8. There are a class of drugs called as PDE-V inhibitors which have been found quite useful in overcoming the problem. Thanks.

Mr. lybrate-user, Long standing diabetes tends to cause a number of complications including erectile dysfunction and loss of libido. This can happen even if there is fairy good control, but if there is persistent hyperglycemia, then it develops more rapidly. Ideally blood glucose should be FBS closer to 100 mg, PP between 150 to 180 mg & HbA1c% <6.8. Even in spite of maintaining such tight control there can be ED. So primary need is to achieve strict blood glucose control and if needed you may have to take PDE - V inhibitors, as and when needed.

Hi I am39 years old diabetic since 8 years on regular treatment on janumet50/500 at morning and janumet50/1000 at night and I am hypertensive also on regular treatment on cardace 2.5but still my sugar levels are high my fasting sugar is 172 mg/dl and post prandial sugar is 245 mg/dl and my HBA1C LEVEL IS 8.2 AND I HAVE MICROALBUMINURIA ALSO PLEASE HELP. It seems your Diabetes is not under control and you are having complications of Diabetes like micro albuminuria which can lead to Diabetic nephropathy. You have to take additional Insulin . Consult Diabetologist for advice and treatment

lybrate-user, TSH value of 10.55 shows you have overt hypothyroidism and it needs to be treated right away with appropriate dose of thyroxine. There is no other option. Plus you wil have to exercise regularly so that normal weight is maintained. Thanks.
