Intracytoplasmic Sperm Injection (ICSI)

Intracytoplasmic Sperm Injection i.e. ICSI is different from conventional IVF i.e. In Vitro Fertilization because a single sperm is selected by the embryologist to be injected by a needle into the egg rather than a dish with many sperms placed near the egg so that the fastest swimming sperm enters it. Once the sperm is injected directly into the egg, it fertilizes, after which the embryo is transferred into the womb or uterus. 

Your doctor might recommend ICSI treatment for you if you have an extremely low sperm count resulting in infertility or other medical sperm conditions such as poor motility or poor morphology. It may also be suggested if prior attempts at IVF procedures have failed in fertilizing the egg or the fertilization rate is extremely low. You must also consider ICSI treatment if you resort to embryo testing or if the sperm needs to be collected from the epididymis or testicles surgically. 

However, you must keep in mind that if your partner has a genetic problem resulting in low sperm count, it could be passed on to your child, if you decide to use your husband's sperm for this process.

Success Rates of ICSI:
The success rate of ICSI treatment depends on your age and your fertility problem. Your eggs are healthier when you are young and as you age, chances are that they will become less healthy.

  1. 35 per cent if you are under 35 years of age
  2. 29 per cent if you are between 35 to 37 years of age
  3. 21 per cent if you are between 38 to 39 years of age
  4. 14 per cent if you are between 40 to 42 years of age
  5. 6 per cent if you are between 43 to 44 years of age
  6. 5 per cent if you are above 44 years of age

Advantages of ICSI Treatment:
Intracytoplasmic Sperm Injection gives you a chance to conceive a child genetically when most other options are exhausted. Males may be unable to ejaculate on the day the eggs are collected due to anxiety. ICSI proves to be advantageous because sperm may also be surgically extracted for this process. It is extremely helpful for those couples who suffer from unexplained infertility. However, one of the most important factors is that ICSI treatment does not affect your child's physical or mental health.

Causes and Symptoms of Hypopituitarism

The pituitary gland is a small, pea-sized attached to the base of the brain. It controls growth, functioning and development of other hormonal glands. Deficit of one or more pituitary hormones is a state of Hypopituitarism. This clinical term is used by endocrinologists to mean that one or more pituitary glands are deficient. In hypopituitarism, the pituitary gland does not produce the normal amounts of hormones.

Causes-

Hypopituitarism has the following causes—

  1. Brain tumours

  2. Sarcoidosis (A lung disorder)

  3. Tumours in the pituitary gland

  4. Hemochromatosis (Too much iron in the body)

  5. Tumours in hypothalamus (Controls the production of hormones)

  6. Histiocytosis X (A rare autoimmune disease where the immune cells damage the organs)

  7. Tuberculosis

  8. Stroke

  9. Sheehan syndrome (Excessive blood loss during childbirth)

  10. Lymphocytic hypophysitis (An inflammation in the pituitary gland)

  11. Radiation treatments like chemotherapy

Symptoms-

Symptoms depend on which hormones is not being produced.

  • Adrenocorticotropic Hormone (ACH): Lack of ACH affects the adrenal glands (endocrine glands producing hormones) and cortisol (steroid hormone). Symptoms include—

  • Thyroid-stimulating Hormone (TSH): Deficient TSH mostly affects the thyroid (produces growth-regulating hormones) gland. The symptoms are—

    • Fatigue

    • Weight gain

    • Skin dryness

    • Sensitivity to cold

    • Constipation

  • Luteinizing hormone (LH): Low luteinizing hormone in women affects their ovaries and ovulation. Symptoms include-

    • irregular periods

    • loss of libido

    • infertility

In men, lack of LH affects their testes and sperm production. Symptoms include-

  • Follicle-stimulating hormone (FSH): This hormone affects men and women the same way the luteinizing hormone does. Thus the symptoms are the same.

  • Growth Hormone: In children, the growth hormone affects their bone, fat and muscle. They have the following symptoms—

    • Lack of height

    • Increase in body fat

    • Abnormal peak bone mass (bone tissue present after the skeleton matures)

In adults, the whole body is affected. Body fat increases while muscle and bone mass decreases.

  • Prolactin: Prolactin deficiency only affects women. The prolactin hormone gets the production of milk started after childbirth. Lack of prolactin thus affects breastfeeding.

  • Antidiuretic Hormone (ADH): The kidneys are affected by lack of ADH. Symptoms include—

    • Diluted urine

    • Frequent urination

    • Feeling excess thirst

  • Oxytocin: Lactation and childbirth are affected by lack of oxytocin. If you wish to discuss about any specific problem, you can consult an endocrinologist.

As you r perceiving pain like menses then you must be getting menses. Hold on for 4 more days. If menses r missed by 7 days then only you can do urine test to find out pregnancy.

You should go to a doctor so that you can be examined. Pain in breasts just before menstruation is common and is usually harmless. However its best to be examined before any medication is started.

Had not intercourse but his penis was inside vagina for not more than a minute, and when he removed he saw a drop of precum over his penis, I washed my vagina and in 20 hours I took ipill too. I also experienced radish pale urine 2-3 times, and a little blood piece once in the urine. My periods were about to come on 27 july but have not come yet. I am worried if I am pregnant.

Best way is to do a urine pregnacy test urself right away. If you r pregnant it will show positive (two lines. Buy any of the kits available in market preganews or velocit etc if its positive and you do not want pregnancy. Visit a gynaec. Abortion is possible with pills only if its negative. Use condom/ oral pills.

Hi lybrate-user. No treatment for vaginismus is possible without an examination and therapy. However persistent pressure for treatment at times psychological trauma which deters the patient from visiting the doctor even more. I suggest first you relieve of her anxiety. Take her to a psychologist who can prescribe her medicines for anxiety. Sometimes in some cases anti anxiety medicines in itself treat the vaginismus. You never know, it might actually do the trick.

My uterus and ovary all are normal and periods correct. When I went to hospital my doctor advice me premence capsule for 15 days. I have pms problem every month. Actually I got my last periods on 5th february, 2016 and then no periods in 5th march, I had sex on 13th march using condom but to be sure took I pill on 14th march within 22 hrs, got bleeding on 24th march but to a minimum till 26th march. As you already had menses after having sex on 13 th march, you can not be pregnant. Menses comes like this after taking i-pill for a month or so, but it had prevented pregnancy.

Hello, Its not a good idea to use unwanted 72 for contraception after marriage as it is very hard and fast acting abortive pill which harm your uterus in a very bad way. You can use I pill daily, instead of unwanted 72. Use I pill daily for 21 days after the end of your period. You will have a hundred percent contraception the whole month.

if there is dryness may be she is having some fungal infection. use vaginal lubricants just before the act as this will reduce the pain. consult a doctor to rule out infection.
