Hello, Takayasu's Arteritis is a vascular disorder involving the Aorta (and its branches) leeaving the heart. It is common in women ageing 20-40. The first & foremost step is to clear the blood vascular system of any cloggings. This disease will cause sudden increase in fever or high ESR levels initially. In Ayurveda it could be termed as Raktavah Srotas Avarodh. It requires cleaning of the circulatory system through various forms of Ayurvedic treatments including panchkarma.

What is the severity of pain? is the pain continous or intermittent? is it pulsating? since pain is a complex phenomenon, there are many treatment options medications, therapies, and mind-body techniques. Apart from pain killers, there is evidence that certain dietary supplements and vitamins can help with certain types of pain. Fish oil is often used to reduce pain associated with swelling. Do have hot and cold water fomentation thrice a day for 3days of 15 minutes duration and still pain persists do consult neuro physiotherapist or gynecological physiotherapist have ift & laser physiotherapy treatment for 10 days 15 minutes duration each followed by spinal extension exercise once pain subsides or consult me privately via lybrate for further information best wishes

Waht do we do for the conservative management of hillsach or benkart lesion? Other than surgery would I go physiotherapy?

If you have bankart and hillsachs, best treatment is surgery. Other treatments are not useful. You have to avoid position which cause pain or dislocation. You must visit shoulder Arthroscopy surgeon.

Hi = relax and stay positive. = increase fluid intake. = eat seasonal fruits for natural vitamins = do streching of hamstring, quadriceps, and calf muscles. = if you are overweight, reduce weight by cycling or swimming as it gives low impact on joints. = do check your calcium and vit. D = apply moist heat pad to affected joint and muscles. If still you dont get relief consult physiotherapist as there are various causes for leg/ joint pain and some test may be required for further investigation. Best wishes.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Backache of 1 day duration can be due to just muscle strain. If you have localized back pain with - no neurological symptoms like numbness, paresthesia etc - no associated symptoms like fever, swelling etc, my advice is: - take rest for 3-5 days - avoid forward bending, lifting heavy weights etc - take an analgesic like diclofenac or something which suits your body for 5-7 days - take muscle relaxant for same time as analgesic to relieve muscle spasm - ice packs at the site of pain, muscle spasm - local analgesic spray can be used but no massage is required let me know in 2 weeks time how you are feeling and then we can suggest you some exercises for lower back muscle strengthening. Bisphosphonates - How Long Should Osteoporosis Patients Take Them?

Bisphosphonates are the most commonly prescribed medicine for osteoporosis patients. This medicine is taken for no more than three to five years in a stretch. A doctor typically takes a call after this time period to find out whether it is suitable for the patient to continue the medicine for a longer time based on the individual risk factors.

Bisphosphonates contain four main ingredients namely Relcast, Fosamax, Boniva and Atelvia. This medicine slows the rate of bone loss and reduces the risk of fracture. This being said, there are some potential side effects of Bisphosphonates as well:

  1. Problems in the jawbone: Consumption of Bisphosphonates can lead to a condition known as the osteonecrosis. This is a disease of the jawbone swelling, pain, and infection of the jawbone. The risk increases if a patient goes through an invasive surgery such as tooth extraction.
  2. Thigh bone fracture: Prolonged intake of Bisphosphonates is believed to be a rare kind of thigh fracture known as the atypical femoral fracture. This is very similar to a stress fracture that causes subtle pain and worsens with time.
  3. Stomach upset: Bisphosphonates intake can lead to oesophageal ulcers and sharp pain in the abdomen. It can also lead to mild fever lasting for a time period of 2-3 days along with upset stomach and body ache. This symptom mostly occurs when there is an irregularity in taking the medicine.

Choosing the right Bisphosphonates:
Drugs of the Bisphosphonates class are similar in nature. While some vary in terms of maintaining potency and bone density, the overall effectiveness of this drug is on the higher side. The common aim of this drug is to reduce the rate of fracture. The decision of choosing a drug over another depends on factors such as cost, preference, convenience and tenacity to stick to the exact dosage. Doctors often prescribe the monthly dosage at one go. The dosage can also be prescribed on a weekly basis depending on the patient’s convenience.

The effect of Bisphosphonates:
Consumption of intravenous Bisphosphonates is considered to be the best medicine for fracture prevention. It is, therefore, very unlikely that a patient with osteoporosis will see additional improvement once he has taken Bisphosphonates for more than three years. This is perhaps the reason why most doctors suggest stopping this medicine after a patient consumes this for more than 3 years. Some other factors that a doctor considers while making this decision include past history of fractures, the age of the patient and the present bone density. If however, the bone density gets worse, a doctor can always restart Bisphosphonates until further improvement is witnessed. 1.Let him do warm saline gurgling 2-3 times daily 2.Take Benadryl CR syp 2 tsf 2-3 times daily 3.Take crocin 500mg 2-3 times daily after food. 4.Drink plenty of water 5.Take rest 6.If no relief he has to take antibiotic after consulting doctor

Do physiotherapy. Also take rhus tox 200 - twice daily for 2-3 weeks. As this is a progressing disease and will take time to heal - you need to keep patience.
