Hello lybrate user as you asked about your problems related to diabetes and bp as well as gastric discomfort, I would like to advise you that your metabolism isn't pretty and toxins are accumulated in the form of acid and gaseous flatulence with upcoming acidic belching and cause of gaseous pressure to upward on your chest and heart. It gives you heart pain and sometimes located at your under chest. You have low hb about 7% so it is very low. You had not mentioned about your sugar levels as fasting and pp. You should have consult with ayurveda doctor who will help you for your problems.

Hello, I appreciate your concern. There are many factors which play role. Few of them are lifestyle, family histroy, smoking, stress, type a peronality. Other secondary causes like kidney diseases, cushing's disease, drugs (medicines). If any of these are present, then yes you could be one of them. So, do check bp regularly. To consult a physician for further management if found to have. Even if you are not one of them, should start working on maintaining a healthy and balanced lifestyle. People used to get hypertension after 60's because they worked hard and ate a healthy diet.

Palpitations - Everything You Need to Know

MBBS, MD - Internal Medicine, DM - Cardiology, Cardiac Device Specialist (CCDS - Physician )
Cardiologist, Delhi
Palpitations - Everything You Need to Know

What is Palpitation?
Palpitation is a feeling of awareness of your own heart beat. It is usually described as heart rate being either too fast (racing), too slow or a sensation of missing a beat.

Types of palpitations?

Normal (Benign)

  1. These palpitations occur as a response to physical or mental stress like exercise, fever, pain, fear, anxiety etc.
  2. They are harmless and settle on their own once the precipitating factors disappear. They do not require any medical treatment.

Abnormal

  1. These palpitations occur due to some underlying abnormality in either the structure of heart or the beating of heart.
  2. These can be dangerous and at times life threatening. They may or may not settle on their own and require some medical treatment.

Warning signs that suggest palpitations are abnormal?
Palpitations are abnormal, if they are associated with chest heaviness, chest pain, uneasiness, sweating, weakness, giddiness, feeling of black out, fainting, nausea, vomiting, seizures. Palpitations are more likely to be abnormal in people who have some existing heart disease and these patients should report to their doctor at the earliest.

What to do during palpitations?

  1. Stay calm.
  2. Call for help. If u are alone avoid driving, use a taxi or auto, go to your nearest hospital/doctor and try to get an ECG during the palpitations. If you are helping someone having palpitations, if they faint, call for help, start CPR if they remain unresponsive and take them to the nearest hospital.

Which doctor can treat palpitations?
Doctors who specialise in the treatment of palpitations are called Electrophysiologists. They specialise in heart rhythm and are capable of performing a variety of complex tests to identify and treat different types of palpitations.

Which investigations are used to diagnose palpitations?

  • ECG: Taken during palpitations and when the patient is normal is a very helpful tool.
  • Holter: Externally applied recorder, which continuously records heart rhythm for 24 hrs.
  • ELR: Extended loop recorder, is like holter, but it records rhythm for longer durations.
  • ILR:  Internal loop recorder is attached within the body for recording rhythm for long duration.
  • EPS: Electrophysiology study, is the most sure shot test to diagnose, identify and treat Palpitations.

What is EPS?
EPS stands for Electrophysiology study. By this test a trained Electrophysiologist studies the conduction and formation system of heart beat, to understand the source, cause and type of palpitation. It is a simple and safe procedure of 2 to 3 hours and can be done as a day care procedure (by admitting the patient for a few hours in hospital, with discharge on same day).

It requires fasting for 4 hrs, some standard blood investigations and is done with local anaesthesia and if required it can be combined with treatment like ablation in the same sitting. In the procedure Electrophysiologist take catheters into your heart to study and stimulate the palpitations and understand them. Once found they can use various techniques to stop the palpitations, the techniques are called Ablation.

What are the treatment options available for palpitations?
There are many options depending upon the nature of palpitations and condition of the patient:

  1. Drug therapy
  2. Cardioversion where either a drug or electric shock is given to stop palpitations immediately.

Ablations using many sophisticated computer softwares
Device Therapy like ICD (Internal cardiac defibrillator)
Combination of all the above therapies.

Your Electrophysiologist and you as a team can make a choice about the options that will be best for you.

Thank you for the query. As per the history you have provided, you are hypertensive (BP 130/90 mm of Hg, on Tenormin 25 and Stamlo 5 mg), have hyperlipidemia (raised cholesterol) and non-diabetic. It would be helpful, if you would have posted the reprots of the lipid profile. Itis advisable to maintain your Total cholesterol <200 mg%, LDL <120 mg% and HDL >45 mg%. The Triglyceride level should be <150 mg%. Since you have limited walking capacity, you need to modify on the diet. Reduce the intake of saturated fat (ghee, butter etc) and refined carbohydrate. The Statin (Atorvastatin, Rosuvastatin) group of medications is effective in reducing cholesterol, but should be taken only after consulting your physician. Regards.

I am suffering from chronic pain and chest pain I didn't use any tablets for this tell any other process to cure.
Certainly chest pain is not something to ignore. But you should know that it has many possible causes. In fact, as much as a quarter of the U.S. Population experiences chest pain that is not related to the heart. Chest pain may also be caused by problems in your lungs, esophagus, muscles, ribs, or nerves, for example. Some of these conditions are serious and life threatening. Others are not. If you have unexplained chest pain, the only way to confirm its cause is to have a doctor evaluate you. You may feel chest pain anywhere from your neck to your upper abdomen. Depending on its cause, chest pain may be: Sharp Dull Burning Aching Stabbing A tight, squeezing, or crushing sensation Visit your doctor.

Your tests came back negative and therefore Your increased Blood pressure, surely could be due to your anxiety. That is also natural. Anxiety, in any kind is supposed to hike your BP by around 30 mmgg. You need to answer your anxiety problem first. For that you should diagnose your type of anxiety with a psychologist, get psychotherapy treatment.

I am suffering from dry cough last 2 weeks and then my heart pain so you suggest me.
I advise you the followings for your condition: Drink plenty of warm fluids. Drinking warm fluids helps to loosen the secretions in your sinuses so that your mucus flows more freely, making you feel better faster. Studies show that drinking warm fluids relieves cold symptoms such as runny nose, cough, sneezing, sore throat, and fatigue.

You must go and meet with a counselor immediately and if that person advises that you meet with a doctor you must do so and cooperate to your utmost. Please visit these professionals along with your parents. In the meantime please do the following sincerely because you could resolve the problem better with good cooperation: Have a good night’s sleep, have a good breakfast of more proteins, meditate often, remain free of stress, eat a lot of fiber, nuts, avocado, exercise regularly, eat dark chocolate, do Yoga meditation exercises, etc. I suggest you do the opposite of what this depression makes you feel like doing (actually, not doing): you will need to fight this condition. You must become active; stay upright during the daylight time; meet people; never sleep during the day, wake up by 6 am every day, play some active games, especially contact games, do physical exercises, talk to people and join some social clubs, attend Yoga classes etc. Watch sitcoms on TV or comedies and cheer yourself up. Go for excursions in groups, for outings, camps, conferences, and religious conventions. Get a pet dog and spend time training it, exercising it and relating to it. Expose yourself to some sunlight every day, at least, 30 minutes but not in the scorching heat. Whatever happens, please incorporate these three important adaptations in your life: always be responsible, be respectful, and be functional. If you did these three, lots of things will go well in life. Please pray and have faith in God to alleviate your sufferings. Don’t wait for others to help. Use your own motivation, which might be at its lowest, but persevere and win this battle. Above all to be really happy, you need to live in love and for love. Learn all about emotions and how to handle them and that will get you out of the depression rather easily and quickly. A counselor is there only to facilitate you, all the hard word must come from you, and your cooperation with that person is very critical for your success. Be positive every day and learn to be contented with what you have. Do some left brain exercises: it is the happy brain. Here are a few suggestions: shut your left nostril and breathe, move your eyes from right to left and vice versa for at least half a minute at a time, and do callisthenic exercises with some form of counting, regularly. Whatever happens please cooperate with the therapy and do not discontinue until the condition is completely resolved. Please do whatever is permissible for your age.
