Hello doctor, Am 8 months pregnant. Its my 2nd marriage. I don?t have family support. Am consider as 2nd wife. My husband was take care of well. Suddenly he got mind diversion with his old college girl friend. I had advised him very calmly. But he didn?t listen. Now he is taking advantage and saying its my life, I can live as my wish. I felt too much of stress. After few months, his girl friend had fight with him. She left him permanent. They just spoke and chat over mobile. Because she is living in abroad with her family. She realized situation and she left him suddenly. But my husband felt that, he will not get anything until he stay with me. He feel negativity towards me. He become too depressed person. Oneday he told, his mother ask him to go Sabari malai. So he told me, ? I want to go sabari malai. Also I need to be separate for sometime?. He took work from home for 3 months. Before leaving from here he told, ? I need some peace. I need change. I want to go to my friend home and I ll get back here after 2 days?. I said ok to him. Because I can sense his depressive mind state. So I agreed. 2 days left I called him regard his return date. He didn?t respond proper. After 5 days again I called he scolded me. He ask me to focus my career and pregnancy. When he was good with me he didn?t even allow me to walk little distance. He used to feed me food daily. But when this girl enter his full mind get diverted and changed. Now he could?t recover from her memories. Because of that, he is not realizing my position. Am now alone. He is saying he is having some other family problem. He can not b with me. Go to years parents place. Am trying to make him no family ll entertain all these. But he is not listening anything. Doctor am so scared of night. I could?t sleep proper. Am missing him. Now he is in native. I seriously don?t know he ll come or not. doctor can you please clarify regard his mind state? Will he come back to me for my delivery? Once he see his baby means he will not go away from me. Please help me to handle him. I need your good suggestion to make him understand my position.

Its is important for you to understand that maintaining good health is very vital not only for you but also for the baby. So you need to focus on your health first. You can take help from your parents and relatives and friends who have goodwill for you to sort out the things in a respectful manner . Getting stressed out and worrying may not really help. Do your best and leave the rest. Being optimistic and keeping the faith will help you overcome all your problems. This is a temporary phase in your life which will move away with time. Hi, start folic acid 5mg tablet once daily for your wife in the preconception period. If your wife is having prod, get a pelvic ultra sound and follicular study from 9th day of cycle to see if she is ovulating. Get her blood sugar and thyroid test done. Just a balanced nutritious diet is enough.

Breast Cancer - Know More to Say 'No' More!

Cancer is the abnormal, uncontrolled growth of cells in a particular body part. With continued growth, pieces of this tissue travel through the blood to different body parts and continue to grow in the new area. This is known as metastases. Breast cancer is one of the most common forms of cancer and affects about 1 in 8 women in the USA. Read on to know more details of breast cancer – breast anatomy, causes, symptoms, risk factors, detection, prevention, and of course treatment.

Anatomy: The main function of the breast is lactation through its milk-producing tissue that are connected to the nipple by narrow ducts. In addition, there is surrounding connective tissue, fibrous material, fat, nerves, blood vessels and lymphatic channels which complete the structure. This is essential to know as most breast cancers develop as small calcifications (hardened particles) in the ducts or as small lumps in the breast tissue which then continues to grow into cancer. The spread can happen through lymphatic or blood flow to other organs.

Warning signs/symptoms: The following are some symptoms that need to be watched out for if you have a predisposition to breast cancer.

  1. A lump in either of the breasts or armpits
  2. Change in size, shape, or contour of either breast
  3. Redness of your breast or nipple
  4. Discharge of clear or bloody fluid
  5. Thickening of breast tissue or skin that lasts through a period
  6. Altered look or feel of the skin on the breast or the nipple (dimpled, inflamed, scaly, or puckered)
  7. One area on the breast that looks very different from the other areas
  8. Hardened area under the breast skin

Either one or a combination of these should be an indication to get a detailed checkup done. Early diagnosis results in controlling the disease with minimal treatment and reduced complications.

Causes and risk factors: The exact cause for breast cancer is yet to be pinned down. However, risk factors are clearly identified, and women with risk factors need to watch out for warning signs.

  1. Family history: Of all the risk factors, the family history is the most important. Breast cancer runs in families, and if there is a first-degree relative with the breast cancer, the chances of developing it are almost double. Two genes BRCA1 and BRCA2 are the carriers of the disease, and this testing can be done in women to identify if they are at risk.
  2. Family history of other cancers: Even if there is no breast cancer, if there are other cancers that run in the family, watch out.
  3. Age: Women over 50 are at higher risk of developing breast cancer.
  4. Race: Caucasian and Jewish women are at higher risk of breast cancer than African-American women.
  5. Hormones: Greater exposure to the female hormone estrogen increases the chances of developing breast cancer. Women who use birth control pills for contraception and hormone replacement after menopause are at a higher risk of developing breast cancer.
  6. Gynecologic milestones: Women who have abnormal menstrual milestones need to watch out. These include those who attain menarche before 12 years of age, get pregnant after 30, attain menopause after 55, and have menstrual cycles shorter than 26 days or longer than 29 days.
  7. Obesity and alcohol abuse are also likely to increase a woman’s chances of developing breast cancer.

Stages: Starting from stage 0, higher stages indicate advanced disease.

  1. Stage 0: The growth which has begun in the milk-producing tissue or the ducts has remained there (in situ) and not spread to any other area, including the rest of the breast.
  2. Stage I: The tissue slowly becomes invasive and has begun to affect the surrounding healthy tissue. It could have spread to the fatty breast tissue and some breast tissue may be found in the nearby lymph nodes.
  3. Stage II: The cancer at this stage grows considerably or spreads to other parts. There are chances that cancer may grow and also spread.
  4. Stage III: It may have spread to the bones or other organs but small amounts are present in up to 9 to 10 of the lymph nodes in the armpits and collar bones which makes it is difficult to fight.
  5. Stage IV: The cancer is widespread to far-flung areas like the liver, lungs, bones, and even the brain.

Screening: This is one of the most effective ways to identify the disease in its early stages. This will help in controlling cancer from spreading with minimal treatment.

  1. Self-examination: A thorough self-examination to look for changes in terms of shape, size, colour, contour, and firmness should be learned by all women. Watch for any discharge, sores, rashes, or swelling in the breasts, surrounding skin, and nipple. Examine them while standing and when lying down.
  2. In most women, annual screening mammograms are advised after the age of 40. However, in women who have a strong family history or genetic makeup, it is advisable to have screening mammograms starting at age 20 every 3 years and then annually from the age of 40.
  3. Women in high-risk categories should have screening mammograms every year and typically start at an earlier age.
  4. Ultrasound screening can also be given in addition to mammograms.
  5. Breast MRI is another way to screen for breast cancer if the risk is greater.

Breast Cancer Prevention: Now that there is so much awareness about causes and risk factors, there are definitely ways to prevent or delay the onset of the disease.

  1. Exercise and a healthy diet with reduced amount of alcohol are definitely effective in minimising the chances of developing cancer.
  2. Tamoxifen is used in women who are at high risk for breast cancer.
  3. Evista (raloxifene) which is used to treat osteoporosis after menopause. It is also widely used in preventing breast cancer.
  4. In high-risk women, breasts are surgically removed to prevent the development of cancer (preventive mastectomy).

Treatment: As with all cancers, treatment would depend on the stage at which it is identified and include a combination of chemotherapy, radiation, and surgery. As noted earlier, if you are at risk, look out for warning signs as early diagnosis is the key to maximum recovery.

No drug should be taken without obstetrician' advice in pregnancy. Cetrzine is not a "sleeping" pill. So break your habit! through it is category B drug it is best avoided on 1 st trim.

If your weight is normal, you just need to maintain same weight by jogging or brisk walk daily for 4-5km. If you have excessive hair and irregular menses, you should start taking krimson-35 tablets for 3-6 monts. New hair growth can be stopped with krimson-35 tablets and it will also regularize your menses. Irregular menses may reappear after the course of krimson-35, but you have to learn to live with it. If your menses does not come in 3 months, you can take tablet deviry 2 times a day for 5 days to get your menses.

You have periods problem. I advise you to do meditation for mental strength by doing slow and deep breathing relaxation exercises in the morning. Take proper balanced vegetarian diet. Take 8 to 10 glasses of water daily. Avoid non veg and eggs. Keep your weight normal as compared to your height. This will help you in your periods.

Age of pregnancy is usually calculated in terms of months or weeks. It should be calculated from first day of last menses. With 8 th January as first day of last menses, today u r almost 2 months or 8 weeks. Yes sex can be practice during pregnancy. After 5 months of pregnancy one needs to change the position of male partner. Intercourse can be practiced in male standing position or sideways position. Male partner should not lie on the tummy or growing fetus. Pain in breast is a sign of pregnancy

If she is having normal 28 days cycle, the ovulation occurs in the middle of the menstrual cycle. 0-10, 20-28 days are safer as compared to other days. Pregnancy is occurs mostly in ovulation period (day 11- 20).

Information from internet says that the STDs like Chlamydia, Gonorrhea mostly will not show symptoms. Is that true? I received unprotected oral sex 4 months back and suffering from urinary problems now. It is prolonging and the pain or burning sensation is there in my genitals. U must consult your obstetrician as soon as possible as you might need antiboliotic therapy. Vaginal discharge and pain in abdomen are to be taken seriously during pregnancy as it puts your kid at risk. In ayurveda, there are various treatments available depending upon your condition and severity as well as type of infection. Some amount of vaginal discharge is common in pregnancy. But if you have pain and excessive discharge, you need few tests to diagnose the severity and type of infection. You should have to care for perineal hygeine. Use some intimate wash for perineal hygeine, avoid using synthetic garments and take plenty of water orally. In case it still persists please feel free to have a gynaecoligist visit.

There is no need and one should not try to delay periods. You may use hot fomentation, like hot water bottle or you may dip your leg in hot water tub with added salt and see if it helps you.
