Extremely Low Thyroxine And Its Complications!

What is Thyroxine?
Thyroxine, also known as T4, is the main hormone which is secreted by the thyroid gland into the bloodstream. Although it is an inactive form, most of it is converted to an active form called triiodothyronine by organs such as the liver and kidneys. The hypothalamus in the brain along with the pituitary and thyroid glands modulate the production and release of thyroid hormones.

What Happens If You Have Too Little Thyroxine?
If the level of thyroxine in the blood is low, then the pituitary gland releases more thyroid-stimulating hormone (TSH) to try and stimulate the thyroid gland to make more thyroxine. Therefore, a raised level of TSH means the thyroid gland is underactive and is not making enough thyroxine. When the thyroid gland produces very little quantity of thyroxine, then the condition is known as hypothyroidism.

As thyroid hormones are essential for physical and mental development, so hypothyroidism during development or before birth and during childhood causes mental impairment and reduced physical growth.

Symptoms usually show a slow and gradual progression before they become worse over months or years as the level of thyroxine in the body gradually falls.

In adults it causes a decrease in metabolic rate resulting in symptoms that include:

  • Fatigue
  • Intolerance of cold temperature
  • Low heart rate
  • Weight gain
  • Reduced appetite
  • Poor memory
  • Depression
  • Stiffness of the muscles
  • Constipation
  • Infertility
  • Slow movements and thoughts
  • Muscle aches, cramps, and weakness
  • Dry skin

Less Common Symptoms include:

Etiology

  1. Autoimmune diseases: The immune system makes antibodies against your body cells
  2. Poor iodine intake 
  3. Use of certain drugs like amiodarone and lithium
  4. Surgical removal or injury to thyroid gland
  5. Underactive thyroid gland (congenital hypothyroidism)

Complications

  1. Low thyroxine level causes the blood fats (lipids) - cholesterol etc., to rise
  2. Pregnant females have an increased risk of developing some pregnancy complications like:
  3. Pre-eclampsia (high blood pressure and protein in urine)
  4. Anemia
  5. Premature labor
  6. Low birth weight
  7. Stillbirth
  8. Serious bleeding after the birth
  9. Hypothyroid coma (myxedema coma) is a very rare complication
  10. Myxedema coma is a rare but life-threatening state of extreme hypothyroidism

Diagnosis and Investigations:

It involves a blood test which screens the levels of TSH and free T4 and T3. Usually, the reports show high TSH and T3 and T4 levels.

Treatment:

Treatment usually involves taking a daily tablet of thyroid hormone levothyroxine to replace the missing thyroxine. Treatment works very well for most people with hypothyroidism but is required for life.

Hello, Please mention as to how many days prior does the menses arrive? A regular menses between 23-35 days is considered normal.

I've used abortion pills at 3 months of pregnancy. In the first week too much bleeding and pain but after 3 weeks light bleeding which occurs specially in the evening for one an hour bt too much pain. Too much pain is normal?

Self medication of abortion pills can be risky ,abortion pills are used upto 7 weeks of pregnancy. If even after 3 weeks you're having pain and bleeding you should immediately consult a doctor in person.

Increased prolactin is a cause of infertility ,take your medicines regularly, once your prolactin levels become normal you will be able to conceive cabergoline is given to reduce prolactin.

Diabetes affects all organs in the body – Eyes, kidneys, nerves, reproductive system all are affected, In fact, from toe to hair. For diabetic control, patient and doctor should be in constant touch for regular follow up and advice. The common symptoms are :-Both types of diabetes have some of the same tell-tale warning signs. Hunger and fatigue. Your body converts the food you eat into glucose that your cells use for energy. But your cells need insulin to bring the glucose in. If your body doesn't make enough or any insulin, or if your cells resist the insulin your body makes, the glucose can't get into them and you have no energy. This can make you more hungry and tired than usual. Urinating more often and being thirstier. The average person usually has to pee between four and seven times in 24 hours, but people with diabetes may go a lot more. Dry mouth and itchy skin. Because your body is using fluids to make pee, there's less moisture for other things. You could get dehydrated, and your mouth may feel dry. Dry skin can make you itchy. Blurred vision. Changing fluid levels in your body could make the lenses in your eyes swell up. They change shape and lose their ability to focus. Diabetes, often referred to by doctors as diabetes mellitus, describes a group of metabolic diseases in which the person has high blood glucose (blood sugar), either because insulin production is inadequate, or because the body's cells do not respond properly to insulin, or both. It is a difficult task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose – SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For diabetes, hypertension (high BP) and many long-term illness control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me with all details like your daily eating pattern and medicines you take. I assure you a proper guidance. If you keep in touch personally. If your pregnancy test is positive and you do not want a pregnancy you will have to undergo a abortion. If it is negative you or your partner will have to use some contraceptive. Your partner can use a condom.
