Dr, My wife survived a stroke back in Sep 15. Her speech is returning slowly but her motor movements are very slow, can you suggest any exercises to help her recover fast and get rid of raised hands all the time. Any help on her will be appreciated.

PDDM, MHA, MBBS
General Physician, Nashik
The general recommendation is that survivors exercise at least three days a week for 20 to 60 minutes, but that depends on their individual functional capacity. For many stroke survivors, multiple 10- to 15- minute bouts of moderate-intensity exercise may be better tolerated. Simple activities that slowly build endurance and strength, such as walking around the neighborhood or engaging in household chores, add up and make a difference.

Hye. Thankyou for the query. You must understand that keeping long gaps between meals and eating consequently heavy meals is one of the common reasons for acidity. Eat small frequent meals. Reduce the main meal by a small amount. Avoid oily or fried foods. Avoid taking glassfuls of water while eating foods. Avoid lying down as soon as soon you have eaten. Stress management is crucial too. These tips will help manage sugar levels as well. Avoid tea, coffee, cigarette and alcohol. Take jeera water after meals.

You firstly stop causative factors from your life style. Like drinking too much cold water, eat too much sweets, day sleep, water after meal, lack of exercise, curd, menda etc. I will need your detail history. Till you can follow this * start drinking hot water * stop day sleep * start any exercise in morning, like suryanamaskar, walking, joging, sports * drink juice of turmeric n ginger with honey * take jau barley, instead of ghenhu wheat. * use cow ghee n milk only * boild rise in open pot, don't use cocker. Withdraw water if rice.

Your Triglycerides are still in normal range but on borderline--------For this you should control your weight to maximum 70 Kg and try to focus on following factors--------- • Try to keep your consumption of saturated fats to a minimum. • Aim to replace saturated fats, with moderate amounts of ‘good’ fats also known as monounsaturated and polyunsaturated fats. • Grill, steam, boil and bake foods instead of frying and roasting. • Eat plenty of fibre-rich foods. • Enjoy healthy snacks such as fruit, nuts and seeds, olives etc. • Eat at least two portions of fish per week, one of which should be oily • Stay physically active. at least 30 minutes of moderate intensity exercise on five or more days of the week. • Avoid smoking. • Try and manage your stress levels.

Hi dear, if still you have any queries consult me. Good luck read below. It is very common to sense your heart beat while lying in bed - rarely is it anything serious, but certainly this sensation will be aggravated if your blood pressure is high. If your blood pressure is normal, it most likely is not a cause of concern. It may also be intensified by any heightened" catecholamine" release (for example, adrenaline) state, such as caffeine consumption, anxiety, fear, etc.

Please get your liver functions checked through blood tests. Preferable to get an ultrasound of the abdomen. Both of these are screening tests to rule out alcohol induced liver injury. Based on the test results further advance testing may be required.

Dear Doctor, I had written to you earlier regarding me taking Gluconorm SR for my high Blood/Sugar level. My last two recent readings are 123/177 and 128/203.My doctor had earlier recommended me taking Glucorm after breakfast. Do you think that I should increase the frequency of taking this medicine. I also want to know, when is the best time to Take Glucorm. Should it be after or before meals.

MD - Consultant Physician
General Physician, Mumbai
An increase in the dose of Gluconorm is probably required provided you have been taking care of your diet and exercise. A good way to see if your DM is under control is to do the HBA1C level. Gluconorm can be taken after food.

Yes, your bp is quite high. Which can result in these symptoms. Try to first control your blood pressure by taking a salt restricted diet and eating healthy and doing exercise daily. Also check your cholesterol levels. If it doesn't improve then consider medical management.

Always experiencing palpation on my heart during the mid of the night. Waking up from nightmare, throat feeling sore, feet feels on fire, stomach rumbling, and neck feels tight! Especially during a cold weather day, or after I am extremely tired during the day time. To me it's more like a heart attack, but I've even appeared in the emergency room, they told me my heart looked OK. This pattern has been repetitively occurred like a myth for four years without an accurate diagnose. All I can say is that I self- diagnose myself as Chronic fatigue syndrome for 4 years too. Could you tell me what is wrong with me (chances of anxiety is very low), I'm waiting for a more reasonable answer!

Always experiencing palpation on my heart during the mid of the night. Waking up from nightmare, throat feeling sore,...
To rule whether it is from heart or not, you will need a Stress Echo and for the palpitations a Electrophysiology Study, in this we study your heart rhythm to see if you are having any abnormally fast or slow heart beat and if we find any we can treat it. A mere ECg will not be able to diagnose all this. So my advise is that instead of self diagnosing yourself get your self properly evaluated. Get your TSH, lipid profile done as well.

As per your description of your situation, it shows that you have anxiety from traveling in train in night. This situation of yours may be because of that you associated some fear with traveling in night and it has become anxiety of traveling in night. Before planning for next journey, start one exercise at your home daily: when you go on bed for sleeping, before sleeping after closing eyes visualize whole situation in which you have fear, while you are lying on bed. In this process, you may have fear again but immediately stop your visualization, start deep breathing for approx five minutes and after deep breathing, start again visualization that situation, if you face fear again, go for deep breathing. Practice this whole process every day for at least ten minutes. I hope, you will get relief.
