Identification and Management of Perinatal HIV Treatment

FHM FELLOWSHIP IN HIV MEDICINE, MD, MBBS
General Physician, Hyderabad
Identification and Management of Perinatal HIV Treatment

HIV testing in pregnancy should be done early so as standard care can be taken for the pregnant women. The HIV testing must be repeated in the third trimester, usually before 36 weeks of gestation. This should be done in women with HIV seronegative and also in pregnant women who are at a high risk of developing HIV infection.

Expedited HIV tests should be performed at the time of delivery and labor and this is a must in women who have not been documented for HIV. The results of the test should be available within an hour of the test and the testing should be available 24 hours. In case the results are positive then, infant postnatal antiretroviral and intrapartum drug prophylaxis should be started immediately.

Women who have not been able to get tested for HIV at the time of labor and delivery are suggested expedited screening for HIV. Their screening can be done immediately postpartum or their babies should undergo screening. In case the infant and mother, both are positive, then infant antiretroviral drug prophylaxis should be started immediately. These mothers should avoid breastfeeding their babies, until the supplemental HIV tests are negative. In infants with positive HIV, prophylaxis should be discontinued and antiretroviral drug therapy should be started.

In case of acute HIV infection during pregnancy, that is in the intrapartum period, or during breastfeeding, initial testing can be performed with an antigen/antibody combination immunoassay. If the supplemental test is negative, then an additional test which is the virologic test (DNA, RNA) are necessary for the diagnosis of the HIV infection. If the mother is HIV positive, then this information must be documented in the infant's medical record and also communicated to the infant's care provider.

The knowledge of an antenatal maternal HIV infection allows the:

  1. Women with HIV infection to get the correct antiretroviral therapy along with prophylaxis for the infections, which might occur due to the immunocompromised state of the body. This also prevents and decreases the risk of transmission to their partners.
  2. When there is provision for antiretroviral therapy for the mother during pregnancy and labor along with antiretroviral drug prophylaxis for the baby there is less risk of perinatal transmission of the HIV.
  3. The HIV-infected women should be counseled for a cesarean delivery. The option of an elective cesarean reduces the perinatal transmission of the HIV.
  4. The HIV women should be counseled about the risk of breastfeeding. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Unknown Pleasure Points In A Woman's Body

Unknown Pleasure Points In A Woman's Body

Pressing the right buttons in a female body can heighten the pleasure markedly. The human body is a very fascinating entity that people know very less about. These hidden spots in a woman’s body will definitely turn her on and ensure an intense orgasm and in some cases, multiple orgasms too.

Here are a few clandestine pleasure points you need to keep in mind for your next sexual encounter:

  1. Ears: The ears have 120 acupressure points which is why you should be paying attention to caressing the ears. Biting, caressing, licking and blowing into the ears can arouse a woman tremendously. If you are a great talker, whisper dirty, sexy things into her ear to increase the spread of sexual energy in her body.
  2. Wrists: The wrists are an erogenous zone which not much people pay attention to. However, the wrist is covered with a thin layer of skin which classifies it as a pleasure point because when the wrists are kissed and nibbled, a woman gets turned on immensely.
  3. Nape of the Neck: This spot is packed with nerve endings that make it a spot to be paid attention to for maximum arousal. Kissing and licking the nape of a woman’s neck is tingling and inflames the sexual arousal in her. Hence, this is a part you should be paying extra attention to during foreplay.
  4. Lower Back: Pay attention to this sensitive spot as a part of your foreplay repertoire as massaging the lower back is very stimulating to a woman. Other than an elevated arousal, massaging the lower back soothes the pelvic muscles and makes the intercourse smoother.
  5. Inner Thighs: Perilously close to the genitals, the inner thighs are an underrated pleasure point which most guys don’t focus on. Kissing and licking the inner thighs will turn a woman on in no time. During foreplay, you can work your way up from the inner thighs to the clitoris to arouse her further.
  6. Back of Neck: The reason your neck is such a hot spot: it’s extremely sensitive, full of blood vessels, and is associated with vulnerability. Soft, lingering caresses are clutch.
  7. Pelvis: Concentrating some affection on her pelvis is a great idea; however, you’ll have to resist the urge to slip down to her vagina while you’re so close. Like teasing her inner thighs, kissing and licking around her pelvis will excite her until she’s begging for more. Prolong the sensation by leaving the region to focus on another body part for a while.
  8. Behind Her Knees: This area is one that most men don’t think of when trying to stimulate a woman, but it is, in fact, quite sensitive. Gently caressing the back of the knee under her skirt while the two of you are in a public place will make her ready to get busy once you get home. Don’t forget to pay some more attention to this special spot once you’re alone too.
  9. The Arm: The area of her arm below the shoulder and between her elbow on the exterior part of her arm is another clitoris.

When you find the point that is that one special pressure point and when pressed on directly and held for 10 seconds or more, she has the capability of moving into orgasmic breath and wave of pleasure fills through her body.

The Takeaway

While sexual arousal seems like a highly individual sort of thing, people generally tend to agree on which spots get their motors running. These so-called "erogenous zones" are linked to feelings of sexual arousal —stimulating these spots can ramp up feelings of sexual pleasure and make sex more satisfying. The best way to find out what you and your partner like is to communicate, experiment, communicate some more, and repeat.

5 Simple Ways to Relax and Recharge

5 Simple Ways to Relax and Recharge

The world is getting busier with every passing day. All are running constantly from playing one role to the other, switching from one task to another. It seems like the messages or the phone calls cannot wait even for like 5 minutes. Do we feel a sense of done at the end of the day? Nah!! We always go to sleep with what to prioritize for next day and the routine continues. Holidays or vacations are no exception either, with our mobiles and devices constantly reminding us of what we should do.

However, of late, there has been a lot of emphasis on the need to relax and recharge the mind and the body. While the body is easier to manage, the mind is a more challenging issue as it is constantly ticking.
Here are some ways to help you relax, suggest you go completely blank in your mind and not do anything during this period.

  1. Exercise: Whether it is a vigorous workout at the gym, aerobic dancing, or lying in the pool, take a pick. While it may sound like a tiresome errand, most of these heavy workouts relax and recharge very effectively. The energy level post a good workout is to be experienced to be believed.
  2. Simply sit and meditate: May sound silly, but it is equally difficult to do too. Try to sit quiet with closed eyes without any movement, physical or mental, for about 5 minutes and experience the peace for yourself. When you open your eyes, you will feel rejuvenated.
  3. Square breathing: Like a square with 4 equal sides, this has 4 equal steps. Inhaling for x seconds, holding it for x seconds, exhaling for x seconds, and waiting out for the next x seconds. 4 or 5 is a good count to start with, but if there are health concerns, then even 2 or 3 should help relax and recharge.
  4. Music: Music is said to be one of the best soothing agents, create a playlist of your favorite genres and list of music. Be it instrumental or vocal, rap or reggae, classical or pop, take your pick and get lost in it completely. If you are game, even learning music can be very relaxing.
  5. Read a book: Another very popular technique is to take a book and get lost completely in the story and its characters. Again, pick something that you would enjoy and see you relaxed and recharged.

Each of these requires some time, the most common question is when do I do this? Tell yourself, you don't get time, you need to make time. Once you see the benefits of recharging your brain, you will make time for it. If you wish to discuss about any specific problem, you can consult a psychologist.

Vaginal Atrophy: What the CLOSER Survey Reveals

Vaginal atrophy (VA) has a great effect on women and their male partners, according to survey results recently published in the Journal of Sexual Medicine.

It’s possible that these results may prompt more communication about VA between couples and their healthcare providers.

Vaginal atrophy refers to changes in the vagina caused by menopause. When estrogen levels drop, the vagina may lose some elasticity, leading to dryness, discomfort, and pain. Some studies estimate that over half of women experience some degree of vaginal atrophy. However, VA is not something many women freely discuss.

To learn more about how VA affects women and their male partners, a research team from Italy and the United States conducted a survey, which was administered online to 4,100 women and 4,100 men from nine countries: the United Kingdom, Finland, Norway, Sweden, Denmark, Italy, France, Canada, and the U.S.

The assessment was called the CLOSER (CLarifying Vaginal Atrophy’s Impact On SEx and Relationships) Survey.

The women were between 55 and 65 years old and had stopped menstruating at least 12 months prior to the survey. They also had symptoms of VA.
The men were in relationships with postmenopausal women aged 55 to 65 who had vaginal discomfort. They were not partnered with the female participants.

For both men and women, VA/vaginal discomfort were described as, “dryness, itching, burning, or soreness in the vagina, bleeding during intercourse, pain during urination, or pain in the vagina in connection with touching and/or intercourse.”

The survey included questions about vaginal discomfort, its symptoms, and its effects on intimacy, relationships, and self-esteem. Participants were also asked about erectile dysfunction (ED) and VA treatments, such as local estrogen therapy (LET).

Most of the women (72%) said that they told their partners when they first felt vaginal discomfort. The other 28% did not do so, explaining that they were embarrassed or thought their symptoms were a normal part of aging. Thirty-six percent said they would try self-treatment before telling their partner about their symptoms.

The authors noted that the men were more likely to discuss VA than the women were, although they were not always aware that sex was uncomfortable for their partner.

Half the women said they were “upset their body does not work as it used to.” Others reported that they felt old and sexually unattractive. Some lost confidence as a sexual partner and others felt that the symptoms would never go away.

Many sexual behaviors changed as a result of VA. Over half the participants said they had less sex and almost a quarter stopped having sex altogether. Sixty-two percent of the women and 76% of the men said they avoided intimacy.

Fifty-eight percent of the women used vaginal moisturizers or lubricants to alleviate their symptoms. Only 15% of the men knew that their partner was using this type of treatment.

Local estrogen therapy appeared to improve sex lives, relationships, and quality of life. Over half the men and women said that sex was less painful and almost half the women said sex was more satisfying.

The researchers stressed the importance of communication about VA between couples themselves and with their healthcare providers.

They also explained the role of healthcare providers in that discussion. “Because women may not be willing to openly discuss the vaginal discomfort they are experiencing, it is crucial for [healthcare providers] to be proactive in helping women address the issue of VA,” they wrote.

Cumin (Jeera) - Can It Have Side Effects?

BAMS, MD - Ayurveda
Ayurveda, Navi Mumbai
Cumin (Jeera) - Can It Have Side Effects?

Cumin seed is a culinary herb that is local to Asia and is particularly used in India on everyday basis. Cumin seeds are otherwise called jeera, caraway and kummel. It is a product of the cumin herb, however, it transforms into a seed only once it has dried. Cumin is an esteemed spice as it has numerous health advantages, usually for digestive conditions like colic, dyspepsia and flatulence. Nevertheless, consuming cumin seeds can have numerous side effects as well; so consult your specialist before you use it for medicinal purposes, as it can cause certain side effects as well, such:

  1. Acidity or heartburn: Cumin seeds are known for their gas easing properties, yet it can bring about one of the most widely recognized digestive issue that is heartburn. It tends to facilitate the escape of more gas into your gastrointestinal tract.
  2. Liver damage: Oil present in the cumin seeds is very unstable and can bring about liver and kidney damage, in case one consumes cumin seed in huge quantities for a long stretch of time. Cumin oil is used for animals to prevent or reduce muscle fits and spasms.
  3. Burping: The carminative impact of cumin seed may bring about excessive burping. Also, burping is sometimes referred as a ructus or belching, which includes too much bloating. Gas from the intestinal tract and stomach is released through the mouth. At times, burping has an awful smell and a trademark sound.
  4. Abortifacient effects: Cumin seeds may also affect pregnant ladies. Consuming a lot of cumin seeds can prompt an unnatural birth cycle or instigate untimely or premature labor.
  5. Low blood sugar level: Consuming cumin seeds in large quantities can bring down the glucose level in the body. One must remember this in case you are setting out toward a surgery in the near future. During surgery, it is crucial to keep up the glucose level.
  6. Narcotic effects: Cumin seed has opiate properties. They need to be consumed with precaution, as they can be addictive. Opposite reactions of cumin seed are mental blurring, sleepiness and nausea.
  7. Heavy menstrual cycle: Cumin seeds may cause heavy bleeding during the menstrual cycle. In the event of cumin seeds being consumed in large amounts, you may experience heavier periods.
  8. Causes allergy: Utilization of cumin seeds can bring about skin rashes and sensitivity. So individuals with skin sensitivity need to consume less cumin seeds to improve the condition.
  9. Not good for diabetics: Diabetic patients need to control their glucose level. They must maintain regular glucose levels to stay sound. Fluctuating glucose levels are a strict no for most diabetics. As mentioned recently, cumin seeds may diminish your glucose level rapidly. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.
