There is no cure for rheumatoid arthritis, But treatment can help reduce inflammation in the joints, relieve pain, prevent or slow joint damage, reduce disability and enable you to live as active a life as possible. Early treatment and support – including lifestyle changes, medication, supportive treatments and surgery – can reduce the risk of joint damage and limit the impact of the condition.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are multiple treatment measures for crps but what was the initiating factor in your case? also since crps is a complicated entity it is best managed by a physician in person as it requires physical therapy to prevent joint stiffness and contractures, medications to deal with osteoporosis, sympathetic blockers or sometimes surgical sympathectomy, and other medications for pain relief as well. Do not hesitate to contact me if you need any further assistance. Thanks & regards Dr. Akshay kumar saxena consultant orthopaedics fortis hospital, new delhi.

Hi thanks for your query and welcome to lybrate. If you have pain which is localized to your back and neck region without any radiation or any associated neurological symptoms, then do intensive physical therapy, take a short course of anti-inflammatory medications, take care of posture both neck and lower back region, u can also use analgesic spray for local application etc, if it does not improve then please contact me as we might have to get some investigations done for you. Do not hesitate to contact me if you need any further assistance. Thanks & regards Dr Akshay Kumar Saxena

She says if she stands she feels fine but when she sits on sofa or on flat ground she feels pain. She says that pain is more prominent in the morning and evening. Could you suggest what could be concern.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Your wife is very young to actually have persistent back pains. If she has localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

I'm 50 years old. And i'm having a severe pain in my shoulders. The pain starts from the neck, and comes to the shoulders. Whenever the pain comes, the area gets swelled up. What should I do? Is it related to physiotherapy or should be consulted to neurologist? what should be done to this pain?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You will have to tell me since when have you been experiencing neck pain, the radiation of pain is it to shoulders or goes in your hands also? is there any weakness, numbness, paresthesias etc also associated with it? swelling can be due to multiple reasons, but if your swelling is in both shoulders then it can be related to your physiotherapy, my advice will be to stop your physical therapy till swelling subsides, take anti inflammatory medications initially, do ice packs at home, take adequate calcium and vitamin d as per your requirement and let me know in 2-3 weeks time how you are feeling? do not hesitate to contact me if you need any further assistance.

Physiotherapy And Soft Tissue Injuries

Physiotherapy And Soft Tissue Injuries

A soft tissue injury occurs due to overuse of ligaments, muscles or tendons. Most cases of soft tissue injuries occur due to uncontrolled or unexpected movement. A Certain example of the same includes awkward stepping of foot and getting the ankles rolled. Soft tissue injury can also happen from overuse of muscles, tendons, and ligaments. The example of the same includes using the same hand for heavy lifting day after day. Certain soft structures of the body that are prone to injuries are Achilles tendons and calf muscle.

Difference Between a Sprain and a Strain:

A strain is defined as a trauma that occurs to the tendons or muscles because of overstretching. Tendons are soft in nature and are responsible tying the muscle and the bone together. Sprain, on the other hand, are trauma caused due to overstretching of the ligaments. There exist 3 severity levels: grade 1 is mild tenderness and minimal stretching of the soft tissue, grade 2 is a tear of fiber resulting in swelling and moderate pain, grade 3 is a rupture of the soft tissue resulting in sharp pain, deep swelling and immobility of the affected area.

Pre-medical Care:

Immediately after the injury, certain things such as total rest and immobility should be ensured, an ice pack should be applied to the structure, compression, and elevation should be ensured in order to avoid any major damage to the soft tissue. Certain things that hamper the healing process immediately after the injury include applying heat to the injured area, consumption of liquor, exercising or running with an injured condition and massaging the injured area.

Physiotherapy For Soft Tissue Injury:

  1. Pain Relief: The first task for a physician is to ensure that the level of pain comes down. Although this is the primary task of a medical practitioner, physiotherapists ensure that the pain does not aggravate to an unbearable level. They recommend certain movements and advice against some movements which could do harm to the injured structure.

  2. Full Structure Movement: The immediate goal of a physiotherapist is to ensure that the full movement of the injured structure is regained at the earliest. This includes identifying abnormalities, verifying the range of motion and suggests corrective exercises ensure that the full movement of the soft tissue can be regained without having to go through the hassle of a surgery.

  3. Muscle Strengthening: While the injured structure is in the process of healing, physiotherapists ensure that the surrounding muscles support the tendon and the ligament by binding well. This is done by suggesting very individualistic exercise for the muscle to hold up. It also negates the possibility of a recurring injury at the same place.
