Take plenty of salads, eat light today like khichdi or dalia. Take lemon water with sugar and salt. Also you can take tab rabonik plus 20mg (rabeprazole), it will help.

Pay close attention to your food choice. ·        Eat less carbohydrate food sources like chapatti, rice, noodles, pasta, bread etc as they help in gaining weight and storing fat. ·        While having lunch or dinner eat vegetables and dal to fill your stomach rather eating lots of chapattis or rice. ·        Have a healthy breakfast like poha, idli, upma, plain dosa, oats, dalia or khichdi with vegetables. ·        Eat a balance diet including good quality proteins like: fish, lean chicken and eggs. If you are a vegetarian then include skimmed milk, low fat paneer, butter milk, soy bean or soy milk in your diet. ·        Avoid ghee, butter or vanaspati. Prepare your diet with vegetable oil like sunflower, safflower soybean or olive oil. ·        Eat plenty of salads. ·        Eat fresh fruits including apple, orange, pineapple, papaya, sweet lime, berries, guava, watermelon, mask melon etc. ·        Avoid eating outside junk and fried foods like pizza, pasta, roll, burgers, cakes, chocolates and cookies. ·        Have an early dinner. She may be having fissure or pile so she needs to be examined by a general surgeon and proctoscopy to be done to find out what exactly it is, keep her stool soft by taking plenty of fluids orally and high fibre diet even she can take syp cremaffin 3 to 6 tsf hs daily, use ointment lox anally before defecation, proper treatment follows according to examination.

Gallstone removal surgery is mainly of two types i.e. keyhole surgery and open surgery. In the case of the former, the patient is not required to stay in the hospital after the surgery, but in the case of the latter, the patient cannot be immediately discharged. Even though after the surgery the amount of bile that is necessary is made by the liver, still patients sometimes feel difficulty in digesting foods with fatty acids. But there are some steps which you can undertake that will give you relief from the complications that arise after the surgery.

  1. A proper dietA proper diet is very necessary for the fast recovery of your gall bladder. So, introduce different food groups gradually. To start with you can have liquid foods like broth or gelatin and then progress to solid foods. You should avoid fatty foods and fried foods as much as you can. Maintain a low-fat diet. Hence, the food items which you must discard from your list includes pizzas, creamy soups, burgers, chocolate, spicy foods, meat gravies and oily foods. And probably after a month, you can start eating the high fiber foods like nuts, legumes, whole-grain bread, broccoli, cabbage and others which would help prevent constipation.
  2. Do some mild exercise: Exercise is essential in improving body condition in any circumstance, and gallstone removal surgery is no exception. Heavy exercise that will place stress on your wounds is not recommended. You can go for a walk or indulge in such mild workouts. Any physical activity that causes discomfort should be stopped immediately.
  3. Increase your fluid intake: Don’t let your body be dehydrated. Drink water from time to time. Dehydration will slow down the recovery process and also result in fatigue. Drink fruit juices too as they will not help in restoring the nutrients in your body but also prevent dehydration.
  4. Take care of your incision area: Taking care of the cuts is very important otherwise they might get infected. So take care of your incisions and change the bandage regularly. Wash the area carefully, and you can apply any antiseptic too but only with the advice of the doctor. You must avoid swimming pool or warm water or bathtub till the incision is completely healed.
  5. Take pain medications if it becomes unbearable: If you experience too much after the surgery, then you can ask your doctor to prescribe some pain medications that will help you with the pain. However, don’t panic as the pain is completely normal in the initial days and you may even feel pain in other portions of the body like your shoulders as well.

Hello doctor! My friend is experiencing an acute pain inside his anus opening. He is unable to even sit properly and it is very difficult to pass motions. There seem to be some boil inside the opening which is causing this. In fact it is little swollen also if you probe inside. From the outside nothing is visible. A few days back also he had experienced the same thing but that was only for a day or two. However then the boil was relatively outside and was reddish. He had applied betnovate c cream then and it was fine. But this does not seem to be working this time and he is in a very bad condition. It is paining a lot. Request your help.

Hello lybrate-user. It seems that your friend is developing a perianal abscess some times its keep on appearing if there is internal opening in the anorectum. Sometimes it leads to fistula in ano. You need to take antibiotics and painkillers along with laxatives. Consult a surgeon who will after confirming the diagnosis, drain the pus and advise you accordingly.

A Guide to Living Healthy: Preventing Cirrhosis of the Liver!

Cirrhosis of liver is slow and gradual replacement of normal healthy liver tissue with scar tissue which results in poor liver function and blockage of flow of blood through liver which comes from intestines. As more scar tissue replaces normal healthy liver, liver begins to fail.

What causes cirrhosis?

  1. Alcohol: Amount of alcohol which can cause liver damage varies from person to person, however those who consume alcohol for long in significant amount are more prone to develop liver damage.
  2. Hepatitis B or hepatitis C: Caused by hepatitis B and C viruses, respectively, which are acquired by contact with contaminated blood (like needlestick injury, blood transfusion, injection drug abuser), sexual contact with infected person and from mother to child during childbirth.
  3. Non-alcoholic fatty liver disease (NAFLD): Caused by presence of extra fat in liver (after excluding significant alcohol intake) which causes inflammation and scarring leading to cirrhosis. Usually seen in patients who are overweight or obese, having diabetes, high fat in blood, high blood pressure or metabolic syndrome.
  4. Drugs causing liver injury.
  5. Others: Autoimmune hepatitis (body`s own immunity acting against liver), Wilson disease (build up of copper in body), hemochromatosis (excess of iron in body), certain bile duct disorders, etc.

What are the symptoms of cirrhosis?

  1. Early disease can present with nonspecific symptoms like weakness, fatigue (loss of energy), decrease wish to take food, vomiting or nausea, weight loss, etc.
  2. Advanced disease can present with fluid accumulation in legs (edema) or abdomen (ascites), facial puffiness, yellowish discoloration of eyes or urine (jaundice), redness of palm (palmer erythema), decrease urine output, small red lesions on skin, easy bleeding following trauma, blood coming out of mouth/anus/other body site, black colored feaces, mental confusion, flapping tremors of hand, etc. In men it can cause impotence, breast enlargement and shrinking of testis.

What are the complication of cirrhosis?

Portal hypertension: It is a common complication of cirrhosis which is due to increased pressure in portal vein. Portal vein is main blood vessel which carries blood to liver from stomach, intestines, spleen, gallbladder and pancreas. Because of scar formation in liver the normal flow of blood from these organs to liver is hampered. As a result of blockage of blood flow to liver there are few complications which can arise like accumulation of fluid in abdomen (ascites) and legs (edema), formation of enlarged veins (varices) in food pipe (esophagus), stomach, etc., enlargement of spleen (splenomegaly), mental confusion/altered behavior/altered sensorium (hepatic encephalopathy), respiratory discomfort (hepatic hydrothorax or hepato-pulmonary syndrome) or decrease urine output/rise in creatinine (hepato-renal syndrome).

  1. Increase risk of infection because of immune system dysfunction.
  2. Development of liver cancer (Hepatocellular carcinoma)
  3. Easy brusibility or bleeding following light trauma.
  4. Gall bladder stone formation
  5. Metabolic bone disease

What are the stages of cirrhosis?

  1. Compensated cirrhosis: Liver damage is damaged but no abdominal swelling (ascites), blood in vomiting or black/red color feaces (variceal bleed), altered behavior/sensorium (hepatic encephalopathy), respiratory discomfort (hepatic hydrothorax or hepato-pulmonary syndrome) or decrease urine output/rise in creatinine (hepato-renal syndrome).
  2. Decompensated cirrhosis: Presence of abdominal swelling (ascites), blood in vomiting or black/red color feaces (variceal bleed), altered behavior/sensorium (hepatic encephalopathy), respiratory discomfort (hepatic hydrothorax or hepato-pulmonary syndrome) or decrease urine output/rise in creatinine (hepato-renal syndrome).

How cirrhosis is diagnosed?

  1. Your doctor will take good history and do proper physical examination. If there is suspicion of cirrhosis then he will subject you to some blood test, ultrasound abdomen, fibroscan/elastography, upper GI endoscopy and if needed CT scan or MRI of abdomen or liver biopsy.

  2. Ultrasound of abdomen in cirrhosis may shows coarse liver echotexture, nodular liver surface, dilated portal vein or collateral, enlarged spleen or abdominal fluid (ascites).

How to prevent cirrhosis?

Best way to avoid development of cirrhosis from predisposing stage of liver illness is to recognize and treat early. Few advices to keep liver healthy are:

  1. Dietary modification: Eat healthy balanced diet. Avoid high calorie food or drinks, saturated fat, sugar and refined carbohydrates. Keep yourself hydrated.
  2. Lifestyle modification: Maintain healthy body weight. Avoid being overweight or obese. Aerobic exercise (like brisk walk 30-45min/day atleast 5days/week) regularly helps to lower liver fat.
  3. Avoid: Avoid use of contaminated needles, sharing of items of personal hygiene (like shaving razors, toothbrush, nail clippers, etc), use of illicit drugs, self medication with over the counter drugs or using drugs beyond doctor`s advice.
    1. Practice safe protected sex
    2. Alcohol
  4. Motivate others: to stop drinking and follow hand hygiene and doctor`s advice.
  5. Hand hygiene: Wash hand with soap and water regularly before eating, after going to toilet and after touching dirty objects.
  6. Vaccination: For hepatitis A or hepatitis B, if you are not vaccinated or not already infected or unsafe antibody titre. Transmission rate of hepatitis B from mother to child at birth can be reduced with vaccination and immunization of newborn starting within 12 hours of birth as well as by using antiviral drugs (if indicated).

Prevent others from getting infected from you if you harbor virus causing liver damage.

How to treat cirrhosis?

Treatment of cirrhosis is based on cause of cirrhosis and complication of cirrhosis. Main aim of treatment in early stage of cirrhosis is to slow the progression of cirrhosis and prevent complications development

  1. Stop alcohol, antiviral drugs for hepatitis b or hepatitis C, proper sugar level if diabetes, steroid or other medications for autoimmune hepatitis, medications to reduce copper from body in patients with Wilson`s disease, etc.
  2. Drugs to lower portal pressure (beta-blocker or nitrates), drugs to remove fluid from body (diuretics), drugs to lower ammonia level for encephalopathy (lactulose and others), drugs to improve kidney function (albumin, terlipressin and others).
  3. Opt for liver ransplant, If cirrhosis is life threatening or treatment of cirrhosis complication is ineffective.
  4. Low salt high protein diet
  5. Stop alcohol intake even if you have other cause of cirrhosis.
  6. Stop smoking
  7. Avoid over the counter drugs (especially pain killers)
  8. Vaccinate if not done already or infected

Hi I want to know that I am having a problem that suddenly at any time I feel like sudden urge to pass stool and it happens about 3 to 4 times a day when I go to washroom little stool comes out with constipation I have to push very hard to pass stool and then after little stool has passed the stool stops coming how hard I try it feels like that my stomach has not fully emptied but stool stops coming. You have to improve your food habits do 1. Take 2/ 3 glass of warm water in the morning before brush 2. Take more water in day 3. Take meals at fixed hrs, chew food properly/ completely, no eating quickly 4. Take small amount of food at a time, take more frequent meals - may be five times a day. 5. No spicy/ fried/ fast/ junk/processed food. Caffeine, mints or mint-flavoured foods, pungent, sour things, tomato-based foods. 6. No smoking, chewing gum, and carbonated beverages. 6. No milk for few days, can take curd, no uncooked salad 7. Avoid constipation 8. Use nibu pani (lemon water) 2/ 3 times a day 9. Do not drink/ store water in plastic bottles. 10.relax and walk for 30 mts daily 11. Take enough rest- do not lie down immediately after eating. Take dinner 3 hrs before sleeping. No late nights 13. Hello, you can take homoeopathic medicine robinia q (5 drops in little water) thrice a day for a week and update. *drink lot of water. *include fruits, raw vegetables and fresh vegetable juices in your diet. *avoid fried and spicy food. *tea/coffee should not be taken in excess. *meals should be taken regularly and on time. *if overweight, loose weight. *avoid stress and it should be avoided by doing yoga and exercise.

Heart burnis due to acidity. To treat acidity eat small, frequent meals(4-5 times a day), avoid spicy, oily / fatty food, maida containing food items, fruit juices/ sugary drinks/tea/coffee. Increase intake high fibre diet. Take light dinner before 8 PM. Don't sleep or bend forward for atleast 2 hrs. after meals in day & night. You can take rest by using 2 pillows below head in recling position .Drink plenty of water, Go for walk/do light exercise . For symptomatic relief you can take Tab. Nexpro RD once daily empty stomach for 10 days then update or You can also consult gestroenterologist .

it may gas distention trouble due to indigestion. have simple bland food (Khichadi) for next few days avoiding junk, oily, spicy fried and deep-fried food items. drink lot of water. Reduce intake of tea – coffee. Instead u can have green tea. U may drink milk (room temperature). dont lye-down or sit immediately after having meals. have a walk instead. maintain good bowel habits. for gas some herbal medicines might also help you but after proper history taking. Eat when you feel hungry and stop when you are satiated. •Leave one-fourth of your stomach empty. •Sit quietly for a few minutes after finishing your meals. •Stay upright after eating. •Avoid smoking. •Avoid gas producing foods – heavy fried foods, excessive spices and chillies, papads/ legumes, milk products (if lactose intolerant). •Chew well. It is important to chew every bite of food 20 times. •Switch to de caf or restrict the intake of caffeine, by limiting tea/ coffee consumption to 2 cups a day. •Cut back on red meat consumption helps if you suffer from acidity. Lean meat such as chicken breast or fish, can take the place of acid-forming red meat. •Consult physician for medication (antacid) if problem persists. HOME REMEDIES Drink some water – As soon as you start feeling discomfort drink a glass of warm water. Ginger to relieve acidity – Make yourself a cup of ginger tea by mixing a teaspoon of freshly grated ginger root in a cup of boiling water. Let stand for 5 minutes and then drink. Chew a small cinnamon stick or chew 2 to 3 cardamoms help to relieve acidity . Avoid lying down immediately after meals – Eat at least two hours before bedtime. Mint infusion – Add fresh chopped mint leaves to a cup of boiling water. Let stand for a few minutes and sip slowly after meals. Cloves - Suck on a clove for natural acidity relief. Fennel (saunf) -Fennel is a digestive herb with many medicinal and culinary uses. It acts as a popular “after mint”.

Hi Mixing two teaspoonfuls of ghee in a glassful of milk and taking this once a day acts as a liner inside the gut and will heal all the destruction caused by the high flow of Pitta. Take pranacharya aceno syrup 2-2 tsf twice a day. Take pranacharya aceno capsule 1-1 twice a day. And avipattikar churna 1-1 gm with warm water. Take pranacharya udar bhaskar kshar churna twice a day with warm water. Take fresh butter milk empty stomach. Avoid tea. Coffee.spicy food. Junk food.
