Fistula-in-Ano or Anal Fistula - How It Can Be Treated?

An anal fistula, is also called as fistula- in -ano, it is a small channel that develops between the end of the large intestine called the anal canal and the skin near the anus. This is a painful condition, especially when the patient is passing stools. It can also cause bleeding and discharge during defecation.

Genesis of fistula-in-ano
Almost all anal fistulae occur due to an anorectal abscess that begins as an infection in one of the anal glands. This infection spreads down to the skin around the anus causing fistula-in -ano. The anorectal abscess usually leads to pain and swelling around the anus, along with fever. Treatment for anorectal abscess involves incising the skin over the abscess to drain the pus. This is done usually under local anesthesia. A fistula-in-ano happens when there is failure of the anorectal abscess wound to heal completely. Almost 50% of patients with an abscess go on to develop a chronic fistula-in-ano.

Symptoms

  1. Pain- Constant pain which gets worse when sitting down
  2. Irritation around the anus, like swelling, redness and tenderness
  3. Discharge of blood or pus
  4. Constipation or pain while evacuation
  5. Fever

Diagnosis
A clinical evaluation, including a digital rectal examination under anesthesia, is carried out to diagnose anal fistula. However, few patients may be advised screening for rectal cancer, sexually transmitted diseases and diverticular disease.

Treatment
The only cure for an anal fistula is surgery. The type of surgery will depend on the position of the anal fistula. Most patients are treated by simply laying open the fistula tract to flush out pus, called Fistulotomy. This type is used in 85-95% of cases and the fistula tract heals after one to two months.

  1. Seton techniques: A seton is a piece of thread (silk, plastic) which is left in the fistula tract to treat anal fissures. This is used if a patient is at high risk of developing incontinence after fistulotomy.
  2. Advancement flap procedures: When the fistula is considered complex, carrying a high risk of incontinence, then this advanced technique is used.

Other techniques like Fibrin glue and Bioprosthetic plug are also used to surgically treat anal fistulas. In the Fibrin glue technique, glue is injected into the fistula to seal the tract, after which the opening is stitch closed. Bioprosthetic plug is a cone shaped plug made from human tissue, which is used to block the internal opening of the fistula. After this stitches are used to keep the plug in place. 

Whatever the surgical technique, one can experience minor changes in continence. Patients usually don’t require antibiotics after surgery but have to take pain medication. They may also have to use gauze to soak up drainage from anus. After surgery, patients should seek help if they have increased pain or swelling, heavy bleeding, difficulty in urination, high temperature, nausea or constipation. If you wish to discuss about any specific problem, you can consult a General surgeon.

lybrate-user, Thanks for the query. As you are aware hypothyroidism being a hormone deficiency state, treatment with thyroxine has to go on for lifetime. The condition is very easy to treat if one takes correct dose of levothyroxine regularly. No other supportive treatment is required. Homeopathy to my knowledge, has absolutely no role to play. But if you want to take it then choice is yours. As regards being under weight, you may have to increase the dietary intake of all the three types of foods -carbs, fat and proteins. For hypothyroidism you have to avoid soy beans and soy containing products. All other foods are fine. You consider taking whey protein supplements in addition to your regular diet. Plus when on treatment with thyroxine TSH should be maintained < 3 mU/L to be free from all the symptoms of that condition. So you need to increase the dose by 12.5 mcg per day. Then check TSH again after 6 weeks. Thanks.

In am into gyming. I have started taking dainabol (methandrostenolone. 20 mg a day. 10 mg in before gym. And 10 mg before my dinner. From last 5 days. Yesterday I found little clot of blood in my stool. So what would be the reason. Should I stop talking it. Or is it normal?

I am 33 years old married women. I did some test for post pregnancy.in blood test thyroid problem deducted. My THS is 6.54.Doctor give me thyronorm 100 mcg. In ultrasound test of whole abdomen. All part is normal. They give opinion in report Fatty liver grade 3. I am very tense because of I am not very fatty. But my abdomen side some fat shows. I don't have diabetes.

Fatty liver is the deposition of excessive fat in the liver and it may be asymptomatic mostly. usually there is no treatment for the same except making some dietary modifications and excercise which may relieve it. Homoeopathy offers good solution for the same . Its done after identifying the cause of this condition by studying the body and the mind of the patient which is genetically determined. if you are interested opt for a homoeopathic consultation. usually fatty livers wont cause a problem but some cases may go for liver damage and cirrhosis

Use of stimulant decreases the symptoms to a much good effect and if we don't give stimulant medication there will be more of conduct disorder and progress in disease and for side effects we have to monitor the person and advice them accordingly and spine block injection is effective for six months.

Take high fiber low fat diet. Drink lot of water everyday. Take small and frequent meals at regular intervals of 2-3 hrs. Avoid outside food completely. Ask me privately for customized diet plan for you.

Dear Lybrate user, You may take only one dose I'd Sulf. 1 M three drops in water empty stomach. Avoid coffee, onion, spice totally. Apply coconut oil in the annus. Avoid constipation. Also.Take fibre food daily. Call back after three days.

Yes, thyroid disease is associated with hair loss. Your tsh value is normal but ideally it should be done fasting.

Dear lybrate-user. What you describe look like urinary tract infection. It could also be other more serous types of infections. Best idea is to show to a doctor in person for clinical examination and at least get a urine test done to see if there is infection. Your doctor might advise a ultra sonography of the abdomen also. Your problems are multiple a if taken together it is a part of syndrome x. Sleep apnea and breathing are significant problem and can even be life threatening. It is easy to assess and control the disease just get evaluated by a pulmonologist.

So if you do not want add extra weight, then there is no option, but to find time for exercise. You should avoid soy beans, soy containing products, cabbage, cauliflower, broccoli, broccoli, radish, peanuts, mustard and coffee. Also consume more of green leafy vegetables salads and fruits. Should use whole grain flour for chapati and roti. These precautions in addition to thyroxine in adequate dose will help in controlling tsh level in turn hypothyroidism">dipaliji, by mentioning" thyroid level" are you talking about high tsh levels? because there is nothing like thyroid levels.
