Hi. I am 31 years old guy. I have got two questions to ask. First one is that I have a doubt that I am having piles as after passing stools when I try to wash my anus with water I can feel a small swelling comes out and few minutes after washing my anus it disappears. I don't have any bleeding though. I have a difficulty while stooling as my initial stools are really hard and I have to give a lot of stress during stooling initially. I want to know whether I am having piles or fistula and how to reduce swelling? My second question is that my stools have never been this hard like what I am experiencing these days. What medicine should I take to make my initial stools soft as I have to give a lot of stress during stooling initially. Please help me.

Dear, 1you are suffering from first degree piles-haemorrhoids. It is not fistula. 2. Hard toilet means constipation which is a lifestyle problem and not a disease. To treat, 1. Increase fluid intake, 2. Takr emolient laxative like duphlec sy or cremaffin plus 1 tsf at night, 3, take naturrolax grannules or isabagol powder 3-4 tsf in at least 300 ml. Of water, 4. Apply zincal ointment twice a day inside anus, 5. Take high fiberand high redue diet, 6. Avoid taking pain killers so far as possible, 7. Increase physical activity like4 walking, playing physical games or exercises regularly, 8. Keep regular hours of sleep and food.

Mr. lybrate-user, Thanks for the query. Now a days testing urine for diabetes is hardly recommended. It is always necessary to test blood glucose. Glucose in urine appears only when blood glucose goes above 180 mg, as above that kidneys are not able to completely absorb the glucose. Hence some of it is lost in urine. So to know whether one has diabetes and whether it is in control or not, it is necessary to check blood glucose. Thanks.

Recent onset yellow eyes are suggestive of jaundice. You need to visit your physician to check that who might advise blood tests for liver function.

I am suffering from recurrent urinary tract infection. I was admitted in the hospital 3 months back as the e. Coli infection reached my kidneys. I was given ertapenem for 1 week and after that no growth came in reports. Again after 2 weeks infection was above 1 lakh. Started to take nitrofurantoin tablets (martifur Mr. 100) and urimax 0.4 capsules. After one months of taking the medicine the infection was less than 1 thousand. But after 15 days nitrofurantion became resistant and growth became 50 thousand. Please note that I have done kubp sonography test and it is very normal. But I am type 2 diabetic. My diabetes is under control from past three months. Please suggest what to do to cure it completely?

Dear , I must know your age, urine report sonography with full bladder volume, bladder-wall thickness and post void residual urine. As you have recurrent/persistent infection one has to find the cause. It may be that the dose was not right or duration was inadequate or drug was not taken as it should be taken or usg was wrongly interpreted or was not done in much detail as shown earlier. So please post the reports with films so that I can guide you properly.

Hello, Thanks for your query on Lybrate "As" per your clinical history is concerned you can try [NITFUR tab-100mg] three times a day for 7 days and follow lifestyle modifications like this - 1)Drink plenty of liquids, especially water. Drinking water helps dilute your urine and ensures that you'll urinate more frequently allowing bacteria to be flushed from your urinary tract before an infection can begin. 2)Wipe from front to back. Doing so after urinating and after a bowel movement helps prevent bacteria in the anal region from spreading to the vagina and urethra. 3)Try to empty your bladder soon after intercourse. Also, drink a full glass of water to help flush bacteria. 4)Try to avoid potentially irritating feminine products. Using deodorant sprays or other feminine products, such as douches and powders, in the genital area can irritate the urethra. 5)Baking soda will raise the acid-base balance of acidic urine and give you relief from the pain. Neutralizing the acidity in the urine also will help speed up recovery. Simply add one teaspoon of baking soda to a glass of water and drink it twice a day. Hope that helps
