I am 47 years age male, I am travelling by everyday from 150 kmts by train from past 5 months. I have been suffering from uti every month at least once, and when it causes I am very tired for at least 3 to 4 days. I have consulted a Doctor, he has given some syrup called citralka and he asked me drink lot of water if I do so, it will be there for 4 days, why it cause repeatedly, and sometimes I feel very tired. Please give some suggestion.

Respected Sir UTI's are less common in males than females. Any UTI in men is a complicated UTI and the cause has to be investigated. Is the UTI documented with a Urine Culture and Sensitivity report? Kindly get a Uroflowmetry and Ultrasound KUB done for proper diagnosis and further management. Please drink plenty of oral fluids ~ 2.5 litres/ day. Hope this helps Thanks.

Respected Sir As per the details provided your estimated GFR is 22 ml/min/1.73 sq. M, comes under CKD Stage IV. 1.Kidney function is severely decreased in stage 4 CKD. Protein waste, toxins and minerals build up in the body and lead to uremia with symptoms such as nausea, vomiting, loss of appetite, abnormal taste, bad breath, nerve and sleep problems, difficulty concentrating and fatigue. Fluid retention due to a decrease in urine output may also occur. 2.Protein Restriction: A protein intake of 0.6g per kilogram of body weight is beneficial for glomerular filtration. Ensure you do not take more protein than recommended. 3.Fluid Restriction: Restrict the amount of fluids you consume. The total volume should be your daily urine output+ 500 ml. Suppose your daily urine output is around 500 ml, you should not take more than 1 litre of fluids (water and other fluids combined) per day. 4.Potassium Restriction: Avoid High potassium foods like bananas, prune juice oranges, orange juice, milk, tomato juice, prunes, nuts, chocolate, dried peas and beans. 5.Phosphorous Restriction: Foods containing high phosphorus are milk, beans, black eye peas, nuts, chocolate, yogurt, cheese of any kind, liver, desserts prepared milk. 6.Sodium Restriction: High sodium foods include salt, all types of frozen meats, corned beef, sausage, pizza, Chinese food, fast foods, cheese, pickles, soy sauce, potato chips and all types of canned soups. The above diet cannot obviously prove to be cure but it can increase your life quality and feeling of your well-being. Kidney Transplantation provides an improved quality of Life. OVERVIEW

Urinary Tract Infections are common infections, affect men and women of all ages, and vary dramatically in their presentation and sequelae.

Urinary Tract Infections are a common cause of morbidity and can lead to significant mortality.

WHAT IS AN UTI?

A UTI is an infection involving any part of the Urinary Tract such as the Urethra, Urinary Bladder and the Kidneys.

UTI is an inflammatory response of the urothelium (lining of the urinary tract) to bacterial invasion.

INCIDENCE AND EPIDEMIOLOGY

Women are most at risk for a UTI. Nearly half of all women (50%) will experience a UTI during their lifetime.

WHY?

Women have a short urethra which makes it easier for the bacteria to enter the bladder and cause infections. Also the urethra is in close proximity to the anal canal, which is the source of these organisms.

What causes the UTI`s?

The most common causative organisms of UTI`s are bacteria from the bowel such as E.coli, proteus, pseudomonas etc.

Once the bacteria enter the urethra, they travel upwards causing infection in the bladder and sometimes other parts of the urinary tract.

Sexual intercourse is commonly associated with UTI`s in women. During intercourse, the bacteria in the vaginal area are sometimes transferred into the urethra by the motion of the penis.

Using spermicides or a diaphragm also can cause more frequent UTI`S , due to increased binding of the organisms to the lining of the urethra.

Menopause also increases the risk of getting a UTI. During menopause, the level of estrogen decreases which causes changes in the periurethral tissues that can lead to a UTI.

In men, urethritis is usually the result of bacteria acquired during sexual contact.

SYMPTOMS AND SIGNS OF A UTI

  1. A strong persistent urge to urinate.
  2. A burning sensation during or after urination.
  3. Passing frequent, small amounts of urine.
  4. Blood in the urine (Hematuria) / Cloudy Urine / Foul smelling urine.
  5. Pain in the lower abdomen and the pelvis.
  6. Penile discharge in men.
  7. Pain in the flank region/ back, fever , nausea and vomiting, fatigue  (Infection of the kidneys)

HOW ARE UTI`S DIAGNOSED?

The Urologist will take a proper history and examine the patient and order some basic investigations like Urine Analysis and Urine Culture and sensitivity. UTI`S in males require more detailed investigations like the Ultrasound of the Abdomen to elucidate the cause of UTI.

TREATMENT

Based on the severity of the symptoms, the urologist may prescribe antibiotics empirically based on the common sensitivity patterns. The antibiotic may be changed based on the Urine C/S report , which usually takes around 48 hours time. You may also be prescribed a medicine that will relax the bladder while the antibiotic starts to work.

Patients must make sure they take the complete course of treatment as directed and to return back if the symptoms are not relieved.

Patients are also advised to drink plenty of oral fluids, which help in flushing out the bacteria from the urinary tract. Patients are also asked to avoid coffee, alcohol, and soft drinks containing caffeine until the infection is cleared because these can irritate the bladder and cause worsening of symptoms.

Kidney Infections (Pyelonephritis) usually require hospitalization and more intensive management.

PREVENTION OF UTI`S

  1. Drink plenty of oral fluids to flush out the bacteria
  2. Do NOT hold the urine for long periods of time. Urinate when you feel
  3. Wipe from front to back after a bowel movement.
  4. Urinate before and after sexual contact emptying the bladder completely each time.
  5. Avoid using spermicides/ diaphragms , in case of recurrent UTI`s
  6. Avoid use of feminine “hygeiene” products like douches and sprays, which could upset the delicate bacterial balance of the genital region.

Respected Sir 1.You will pass out this 5 mm stone spontaneously without any medicine. Just make sure you drink plenty of oral fluids daily, atleast 2.5 to 3 litres per day 2.Limit the amount of salt intake in your diet. Do not add extra salt. 3.Limit the intake of animal proteins like red meat and fish 4.Include seasonal fruits and vegetables in the diet on a daily basis. 5.Avoid soft drinks and energy drinks like Red Bull. 6.Limit high oxalate containing foods like chocolates, nuts,spinach etc. It is very difficult to predict when the stone will come out .Get a repeat ultrasound done after 3 months. Respected Sir 1.Peyronie`s disease leda to pain on erection and erectile dysfunction. 2.No definitive medical management is available at present. 3.Vitamin E might help 4.Colchicine and L-Carnitine also might have some role. 5.Pentoxyfilline might relieve the pain associated with erection. 6.Now a days, Intralesional injections are available which might to some extent alleviate the severity of the disease. 7.Your Urologist might be able to prescribe you the required management. Respected Sir 1.Based on your symptoms, you might be having phimosis (tight foreskin). 2.Are you able to retract it completely? Try to slowly retract the foreskin back daily and clean the undersurface with soap and water and keep the area dry. 3.A tight foreskin presents with difficulty in the intercourse, like you may develop cracks and tears on the foreskin. 4.Please consult a Urologist who would assess the condition and provide you appropriate treatment. 5.You might require surgical removal of the foreskin (Circumcision) if the problem persists. Respected Sir 1.From the symptoms you presented, you might be having a tight penile foreskin (phimosis) with difficulty in retracting the foreskin. 2. I am 23 years old virgin male. For the past 1 year I am getting mild white painless discharge from my penis every single day (4 to 5 times a day) .Im getting this discharge not while urinating but it comes at frequent irregular times simply without any feeling. I am frequently getting urinals two or three times in the night time there by getting disturbed sleep. Occasionally wetting my trousers and bed because of the urge to urinate. Respected sir You have not mentioned your age. I assume you to be over 50 years of age. Your symptoms suggest you might be having a benign enlargement of the prostate gland. Its a physiological condition in males over 40 years of age. Kindly get yourself evaluated by a Urologist, who will examine you and advise you investigations to corroborate the diagnosis and prescribe medication. I am 24 years old. My penis when flaccid has excessive skin on penis head. Though when erections. This excessive skin gets removed. But then while erections I can't the skin over the penis. Respected Sir You might be having a tight foreskin (Skin covering the tip of the penis. Thats the precise reason why you are not able to retract the foreskin freely. Respected Sir 1.Any stone removal surgery carries the risk of infection spreading into the main blood stream, leading to sepsis, which is the precise cause of tachycardia (increase in the pulse rate) and low blood pressure (hypotension). It is due to the release of endotoxins and bacteria from the stone into the blood stream. 2.Blood transfusion is not required and the sepsis can be tackled by ensuring sterile Urine culture prior to the stone surgery and the use of broad spectrum antibitotics in the perioperative and post operative period. I have gone through ultrasonography. Doctor told us that you have 7 mm stone and advice me to go for an IVP (intravenous pyelogram) test .In that test report of everything is normal. There is clearly written that no any calculated calculus is found and al the functions are normal. I would like to mention that there is 5 days gap between sonography test and IVP test. Doctors who has done IVP test told me that kidney stone may come out from the body from urine. Please suggest what I have to do?

Respected Sir 1.Have gone through the details 2.Hopefully you might have passed out the calculus. 3.However some stones are radio lucent, means they are not visible on x ray. 4.Please drink plenty of oral fluids ,atleast 2.5 to 3 litres / day. 5.Get an Ultrasound repeated after a month to confirm the same. Respected Sir Pus cells indicate inflammatory response of your body to an infection. Mere presence of pus cells does not indicate infertility. Please try taking plenty of oral fluids and some antioxidant foods like green leafy vegetables and fruits. I would suggest you to get the semen analysis repeated after a month and reassess.
