Breast Cancer - The Truth !

Breast Cancer is the most common cancer of females in urban cities. It is a curable disease if detected early. The usual age of presentation is usually after 40 to 50 years of age, though it is becoming common in earlier age as well. This could be due to many factors like late marriages, older age of pregnancy, diet and lifestyle, etc. The usual presentation is a swelling or lump in the breast, not associated with pain. That is the reason why females are advised regular self-breast examination, clinical breast examination and annual mammograms for screening purpose in order to detect early breast cancers. Many times, the breast lump is associated with blood stained discharge from the nipple. There can be skin changes on the breast like thickening, crusting or sometimes itching. If the skin lymphatics get blocked, there may be pitting and or dimpling of the skin too. There may be lumps felt in the armpit felt too. Many times the lump is not noticeable or felt and felt by chance during a bath. These are some of the commonest presentations. The lady should immediately consult a doctor and get evaluated in such scenario. Many women are at increased risk of developing breast cancer like those who have a positive family history especially first degree relatives like mother& sister. Breast cancer is usually a disease of women but 1% of cases are also seen in men.

If a lady feels any such lump she should immediately bring it to the attention of a doctor or oncologist who can do the necessary evaluation and investigations. Breast cancer is curable if detected early, hence the importance!

The primary modality of treatment is surgery followed by chemotherapy and or radiotherapy and or hormonal treatment depending on the report of the specimen after the surgery. Many times if the lump cannot be operated upfront, initially chemotherapy and or targeted therapy can be given to shrink the tumor for better resectability.

People should be aware that having cancer is not the end of life. It can be treated like many other diseases and the most important thing is awareness and early detection for a durable cure.

Brain injury can happen as a result of trauma, infection or degeneration with old age. Whatever be the reason, injury on the head can result into brain dysfunction and should be given required medical diagnosis. In these cases, occupational therapy can be a major help.

It helps in improving health, providing rehabilitation and educational service to enable people to participate in things they are interested in and manage their daily activities. Occupational therapy can be used to manage issues relating to memory, organization and attention span, behavior and emotion control, safety issues and issues which contribute to society. Depending on expectations, different therapies can be sought. Read on to know a little more about how occupational therapy can help adults with brain injury.

  1. The person's skills and what they want to do are to be assessed and accordingly a daily plan that can be easily followed is made. Where possible, use technology like smart phone or voice recorder to manage this plan. The therapist can train the person to use these tools, review progress and make changes as necessary.
  2. The therapist will also teach ways to do things on their own like cooking and small shopping, so they become independent and are therefore less frustrated.
  3. Patients in need of anger management, the therapist will help them to identify what induces anger or frustration and support in managing those before it turns into actual anger. This will help the person relax and be more positive.
  4. This could also be done using small roleplays, wherein they are taught ways to respond and be more relaxed and calm.
  5. If you are keen on participating in social events or volunteering opportunities, then the therapist can recommend options for you based on your areas of interest. The therapist can also work with the identified organization (school, NGO, or workplace) to inform them of the kind of support required by you. Work behavior and social behavior can be taught to help the affected person succeed.
  6. If the affected person happens to be alone, then they need to be assessed for ability to judge (e.g., self-awareness, impulsivity, and reliability), even while doing daily activities like bathing and dressing. Driving is permissible only if cleared for it by the therapist.
  7. The home needs to be assessed for safety. You may have to make some safety modifications, for instance replace power tools with hand tools. Else, shared housing options, where independence and having someone close by is possible, should be evaluated.

In Spinal cord Injury, patients generally do not go to Physiotherapist for a longer duration or they just go for few days in the starting injury phase. At that time they are not able to walk or depending upon their level of injury and everything. They might not able to take care of themselves also. But here we try to make them independent as much as they can to help them move their body accordingly. We try to make them stand. We try to focus on their sitting and their walking also. So, it will be more of the confidence increasing for those kinds of patients.

We generally prescribe patients to do movements and increase their strength and motivations. In this condition, if they do not move, there might be different kinds of serious problems like bed su, the stiffness of the joints which will again lead to major issues which will hinder their improvement and progress. So, we prescribe our patients to move as much as they can to help them and gain their mobility soon.

No issues. Low cholesterol has no problems. But yes you can decrease the dose of the statins by half and recheck the lipid profile after 3 months. Long term statins treatment also has side effects. Also you need to understand about cholesterol Cholesterol is a waxy, fat-like substance that occurs naturally in all parts of the body. Your body needs some cholesterol to work properly. But if you have too much in your blood, it can combine with other substances in the blood and stick to the walls of your arteries. This is called plaque. Plaque can narrow your arteries or even block them. High levels of cholesterol in the blood can increase your risk of heart disease. Your cholesterol levels tend to rise as you get older. There are usually no signs or symptoms that you have high blood cholesterol, but it can be detected with a blood test. You are likely to have high cholesterol if members of your family have it, if you are overweight or if you eat a lot of fatty foods. The very term 'breast cancer' can send shivers down a woman's spine. The associated pain, agony, sufferings are enough to deter a person. The effectiveness of the treatment depends on the stage in which the condition is detected. Unfortunately, many women neglect and overlook the early signs, thereby delaying the treatment.

Self-awareness is vital for prevention, early detection and treatment of breast cancer. Treatment and prevention can only work when a person is alert and agile. If you notice any unusual symptoms, waste no time and consult an experienced physician at the earliest. However, prevention is always better than a cure. A few healthy habits, if practiced diligently, can go a long way to prevent and reduce the incidences of breast cancer.

  1. A healthy body weight: Overweight and obese women are at a greater risk of suffering from breast cancer. The key to this problem is as simple as shedding off the extra kilos. Research suggests that incidences of breast cancer are slightly lower in women who maintain body weight within healthy limits. Enrich your diet with fruits and vegetables. Drink water and other healthy fluids (coconut water, fresh fruit juice,) at regular intervals.
  2. Breastfeed for a healthy breast: Breastfeeding is important for the health of both the baby and the mother. While babies get the necessary nourishment, women who are into breastfeeding are less likely to suffer from breast cancer. Thus, all healthy lactating mothers should breastfeed their babies.
  3. Keep the vices at bay: Severe smoking and alcohol consumption can harm your body beyond imagination, with breast cancer being one the many harmful side effects. Drinking within limits (1 drink or even less per day) is still acceptable, but anything in excess is sure to spell doom.
  4. Some of the imaging and diagnostic tests can be harmful to your body: Repeated exposure to the radiations from CT-scan can put a woman at an increased risk of developing breast cancer. Limiting the exposure to such radiations may help to check the incidences of breast cancer.
  5. Physically active and agile: Lack of physical exercise and an unhealthy lifestyle can wreak havoc with health complications as serious as breast cancer. It is important to keep your body physically active. Light to moderate exercise (for 30 minutes), jogging, cycling, swimming and other physical activities can work wonders for your body. Avoid stress and anxiety. Proper rest and a sound sleep are equally important.

Various research and studies suggest that women who are into hormone therapy are more susceptible to breast cancer. Such patients are often advised to avoid and replace hormone therapy with alternative treatments. Breast cancer is a type of a cancerous infection that develops in the skin cells of the breast. This condition is more commonly diagnosed after you have skin cancer and is more common in women than men. There are certain signs to identify this disease, which are as follows.

1. Breast lumps - Lumps are patches of skin that occur due to the thickening and swelling of the skin. Breast lumps is a very common phenomenon for women but that does not always lead to a breast cancer; however, frequent and reoccurring instances of the same increases the risk of breast cancer. A breast lump is easily identifiable through the naked eye as it looks different from the surrounding tissues.

2. Blood discharge - The nipple is one of the most sensitive areas in the human body. In case of women, it is meant to emit milk during pregnancy, in order to feed the infant externally. A probable symptom of breast cancer can also be a frequent discharge of blood from the nipple without any visible damage or injury.

3. Breast changes - If you are experiencing any visible changes in the size, appearance or shape of the breast, then it is an indicative sign that you may have breast cancer. Thus, it is of paramount importance that you observe, touch and physically examine your breast regularly for changes. If there is any major change detected then it is advisable to consult a doctor.

4. Inverted nipple - The tip of the nipple is generally bulging and pointing outwards both in cases of men and women. But sometimes the tip of the nipple is pushed inside due to excessive suction caused by the skin tissues present inside the breast around the nipple area. This suction forces the nipple to grow inwards which is abnormal and might harm the breast and skin in various ways.

However, apart from these main symptoms, some other signs are also there that increase your chances of being diagnosed with breast cancer. I am 63 year old. I am having sciatica, back pain and numbness in both legs. I do not have BP and my sugar level is with in limit. As per CT scan report the impression are as follows: Lumbar Spondylitis with Disc herniation at L4-5, L3-4 levels and disc bulge at L5-S1 level. Canal stenosis at L4-5 level. Cervical spondylosis - Decreased height of C5 and C6. Vertebral bodies with end plate irregularities at C4-C7 levels. Disc bulge at C3-4 level. Disc herniation with canal stenosis at C4-5, C5-6 and C6-7 levels. Can it be cured with Homeopathy.

When Is Surgery Recommended For Herniated Disc?

The symptoms caused due to herniated disc can be very severe and can also cause a bit of disability. The disc of the spine is like a cushion and separates the set of bones on the backside. The discs are shock absorbers of the spine and are mainly composed of 2 parts, a soft jelly-like centre called the nucleus and a tough outer covering called the annulus.

Effects of Herniated Disk

A herniated or cracked disc is a severe condition and it seems to happen most commonly in the lower back or neck.  It happens when a fraction of the soft centre gets pushed through the destabilized area due to degeneration, trauma or by putting pressure on the spinal column. 

Nerves located at the back of every disc are responsible for transmitting pain, motor impulse, bladder control etc. in our body. While a disc gets herniated, the external covering of the disc tears and creates a bulge. The soft jelly gets shifted from the centre of the disk to the region where the damage has occurred on the disc. Most commonly, the bulge occurs in areas where the nerve is located and it causes strain and irritation of the affected nerve. It has been observed that individuals may or may not feel any painful sensations even if their disc gets damaged. Other symptoms may be weakness of muscle groups or difficulty in controlling the bladder.

When is surgery recommended for herniated disc?

Surgery for herniated disc is recommended only after options like rest and pain relievers do not work. If the pain persists even after these options, then it becomes important to go for surgery. Surgery is also considered early if there is weakness of muscle groups or acute problem in bladder control. At times, emergency surgery is also required to avoid paralysis in a patient.

However, there are certain risks involved in this surgery like infection, bleeding or nerve damage.  There are chances that the leftover disc may bulge out again. If you are a patient suffering from degenerative disc disease, then there are chances that problem occurs in other discs. It is very important that a patient maintains healthy weight to prevent any further complications.

Abnormal Uterine Bleeding - How Best to Diagnose It?

It is perfectly normal for women to experience the periodic monthly bleeding cycle. However, if a woman experiences uterine bleeding which is abnormal and dysfunctional, it could be a symptom of infection. Other causes of abnormal uterine bleeding include hormonal imbalance, infection in cervix and cancer of the uterus. Many women can also experience abnormal uterine bleeding during first trimester of pregnancy.

The following are considered to be abnormal or dysfunctional uterine bleeding:

  1. A menstrual cycles occurs between 21 to 35 days, anything shorter or longer than this is abnormal
  2. No period for 3–6 months (amenorrhea) is abnormal
  3. Spotting or bleeding between periods
  4. Spotting or bleeding after intercourse
  5. Bleeding that is heavier or lasts longer than usual
  6. Spotting or bleeding after menopause

What can cause such a situation?

Some of the common causes leading to abnormal bleeding are as follows:

  1. Miscarriage
  2. Ectopic pregnancy
  3. Pregnancy
  4. Cervical or uterine infections
  5. Fibroids
  6. Hormonal imbalances
  7. Problems with blood clotting
  8. Polyps
  9. Polycystic ovarian
  10. Endometrial hyperplasia
  11. Cancer of the reproductive tract

How to Diagnose it?

Most women tend to ignore abnormal bleeding, taking it as something to do with age or hormones. A detailed physical examination and history is done to understand menstrual cycle patterns and family history. In addition, the following would be used.

  1. Ultrasound: The pelvic organs are examined through sound waves to locate the problem area
  2. Hysteroscopy: Through a thin device that is inserted into the vagina, the doctor takes a look at the inside of the organs and identify the cause for the bleeding
  3. Endometrial biopsy: The uterine lining tissue is removed and examined under microscope to look for tissue changes that could be causing the bleeding

How Best to Treat It?

This would depend on the reason for the abnormal bleeding. However, in most cases, combinations of the following are useful in treatment.

  1. Hormone replacement can be done depending on the age and gynecological history, the type and the dose of the hormone would be decided upon. These could be in the form of tablets, vaginal creams, injections, or through an intrauterine device
  2. Non-steroidal anti-inflammatory drugs (NSAIDs) like ibuprofen are useful in reducing bleeding and controlling cramps during excessive menstrual bleeding.
  3. Antibiotics may be useful if there is infection of the pelvic organs.
  4. Polyps, cysts, cancers, and other growths can be removed via hysteroscopy and sent for biopsy to confirm they are not cancerous.
  5. Endometrial ablation where the endometrial tissue is treated with heat can be used to control bleeding permanently.
  6. Hysterectomy would be done if other forms of treatment have failed. This could depend on their gynecologic history and other considerations as she cannot get pregnant after this.

MRI IMPRESSION 29 August 2016 -Posterior diffuse disc herniation at L4 -5 level with bilateral ligamentum flavum hypertrophy causing narrowing of bilateral neural recesses with compression of bilateral traversing nerve roots at same level. -Posterior diffuse disc bulge at L5-S1 level indenting ventral thecal space at same level -Changes of lumbar spondylosis I was having pain at my right hip joint on 15/8/16. It went unbearable extending to RT leg. On 3rd September night as usual on bed all the 24 hours trying to sleep on 4th at 5 am all my pain went. Till then no pain but having burning sensation on right foot, tingling, falling rt foot asleep when I sit on chair. Please advise:- 1. Surgical intervention required? Or 2.Pregabalin,Tolperisone, methylcobalamin,Calcium and D3 with rest will cure me fully? Or 3. Somekind of spinal exercises also required? ERODHA.

Regular exercises and maintaining good posture whie sitting is the key to avoid most of the back pain problems. Please do take frequent breaks every hr and strech your muscles and do not sit for long.

Sleep on a hard bed with soft bedding on it. SPRING BEDS, FOLDING BEDS OR THICK MATRESS ARE HARMFUL Use no pillow under the head. DO HOT FOMANTATION. Paracetamol 250mg OD & SOS x 5days. Caldikind plus 1tab OD x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. The womb or the uterus is a muscular structure that is fixed in its place by the ligaments and the pelvic muscles. If these ligaments or muscles become weak or stretch, they no longer will be capable of holding the uterus, resulting in a prolapse. Uterine prolapse happens when the uterine slips or sags from its usual position into the birth canal or vagina. The main indications of uterine prolapse are recurrent bladder infections, constipation, a tug at the pelvic area, the cervix or uterus protruding out of the vagina, problems during sexual intercourse, increased discharge and vaginal bleeding.

What causes it?
