All About Diabetic Nephropathy

MBBS, MD - General Medicine, DM - Nephrology, DNB (Nephrology)
Nephrologist, Delhi
All About Diabetic Nephropathy

Nephropathy is also known as renal disease. It is any type of damage or disease relating to the kidneys. Though not everyone with diabetes has nephropathy, however, diabetic nephropathy can cause kidney failure.

How does diabetes affect the kidney?
Kidneys are responsible for taking waste out from your blood. They have a lot of tiny blood vessels to do this. High blood sugar may kill such blood vessels. Once these blood vessels are destroyed, the kidneys may not function as well or it may even lead to kidney failure.

What increases your risk of getting diabetic nephropathy?
There are several factors which increase your risk of getting diabetic nephropathy including: \

  1. High blood pressure
  2. High cholesterol
  3. Smoking
  4. Being Native American, African American or of Hispanic origin, for whom risk factors have proved to be magnified
  5. If you have family history of diabetic nephropathy

Symptoms and diagnosis:
There are very few noticeable symptoms, which appear when you have diabetic nephropathy, except swelling in your arms and legs. The diagnosis is done by checking for a type of protein in your urine known as albumin, which is not supposed to be there. Getting the diagnosis done early is crucial.

Preventive measures:
You can prevent kidney damage by doing the following:

  1. Keeping your blood sugar levels under control - keep HbA1C < 6-7%
  2. Keeping your blood pressure under control in the range of 130/80
  3. Eating healthy food
  4. Exercising regularly
  5. Not eating too much protein
  6. Not eating too much salt
  7. Reducing smoke or excessive tobacco usage

Medicines for treatment:
If you do get diabetic nephropathy, here are some medicines, which can help cure it

  1. ACE inhibitors which are also called angiotensin-converting enzyme inhibitors
  2. Angiotensin II Receptor Blockers (ARBs)

Possible complications:

  1. Blood pressure may rise
  2. Triglyceride levels and cholesterol levels may rise.

Here are the top 6 kidney stone prevention tips: Hydrate with H2O. One of the best measures you can take to avoid kidney stones is to drink plenty of water, leading you to urinate a lot. So, be sure to keep well hydrated, especially when engaging in exercise or activities that cause a lot of sweating. Eat and drink calcium and oxalate-rich foods together during a meal. In doing so, oxalate and calcium are more likely to bind to one another in the stomach and intestines before the kidneys begin processing, making it less likely that kidney stones will form. Don't reduce the calcium. Work to cut back on the sodium in your diet and to pair calcium-rich foods with oxalate-rich foods. Take action! Without the right medications and diet adjustments, stones can come back, and recurring kidney stones also could be an indicator of other problems, including kidney disease. Next time you drive past a lemonade (or limeade) stand, consider your kidneys. Chronic kidney stones are often treated with potassium citrate, but studies have shown that limeade, lemonade and other fruits and juices high in natural citrate offers the same stone-preventing benefits. Beware of the sugar, though, because it can increase kidney stone risk. Instead, buy sugar-free lemonade, or make your own by mixing lime or lemon juice with water and using a sugar substitute if needed. To prevent uric acid stones, cut down on high-purine foods such as red meat, organ meats, and shellfish, and follow a healthy diet that contains mostly vegetables and fruits, whole grains, and low fat dairy products. Limit sugar-sweetened foods and drinks, especially those that contain high fructose corn syrup. Limit alcohol because it can increase uric acid levels in the blood and avoid crash diets for the same reason. Eating less animal-based protein and eating more fruits and vegetables will help decrease urine acidity and this will help reduce the chance for stone formation.

What is thw correct procedure of urinating for girls someone said that urinating in a flow is injurious and we should pause for a moment while urinating and should urinate in pauses this is correct form of urinating and it is an exercise for uterus. Is that so?

MD, MBBS

What is thw correct procedure of urinating for girls someone said that urinating in a flow is injurious and we should...
There are no medical tips for correct procedure for urination by girls. The only thing to be observed or be careful about is to prevent contamination and infection of urethra by pot or by anal bacteria while washing. Female urethra is small and more vulnerable to UTIs as compared to males. It is always advisable to pause for a moment while commencing to urinate but doing it in pauses is not advisable. Do it slowly with control as fast urination may cause injury to urethra. Never hold urine for very long times. Do regular pelvic exercises to prevent incontinence in old age.

Sometimes a white colour urine comes. Its not semen but something else gives burning sensation. I feel weak after it ...
Even if the colour of urine is normal, a burning sensation needs you to do a urine examination to rule out crystals, red cells and urinary tract infection. If there is suggestion of infection do a culture and sensitivity test to get proper choice of antibiotics. If there is no infection, an ultrasound examination of Kidney Ureter Bladder may help with some finding. Depending upon the reports, further needful may be advised.
