I have lower back pain. I can't sit. I went to doctor he asked me to do x ray. He said its a spasm as your back looks stiff he told me to rest for a week and have multivitamin and calcium + a pain killer after week I was better now its hurting again.

The usual cause of low back pain is the sedentary lifestyle that we lead these days. There is hardly any movement. We remain seated most of the time and stand for very short periods of time. You should do regular exercises Morning & evening walk Take care of your posture Take rest breaks while doing work Avoid being in one position for long duration (eg standing for long hours) Do back & knee isometrics Active movements of all joints Apply hot fermentation on painful areas/joints of body For feet do contrast bath therapy Take hot water in 1 bucket & cold water in another then immerse your feet in hot water for 3min then in cold water for 1min, keep repeating for 20min, twice daily.

There is a fun saying that anybody who has a back is going to have back pain. Jokes apart, its a stark reality that we are witnessing and will continue to witness in the coming decades due to the over dependence on machines for day to day activities and a higher prevalence of sedentary life style, especially among the young generation whose work depends on computers and staying indoors mostly.

90% people suffer from back pain in their lifetime, among them a fair number need medical help for the same.

Symptoms may include:

  • Difficulty in sitting/walking/standing due to back pain
  • Pain radiating to buttock, thigh, and or leg
  • Muscle spasm and stiffness, sometimes in the morning or at the end of the day
  • Soreness / tenderness in spine

Possible causes may be:

Strain of muscle or ligament - most common
Lumbar disc herniation (disc prolapsed/SlipDisc) - Pain mainly in buttock, leg and foot (sciatica)
Facet/spinal joint arthritis - usually causes slow back pain and leads to referred pain in thigh as the disease advances
Spinal canal stenosis and spondylolisthesis-narrowing of space available for spinal cord/nerves in the spinal tunnel -leading to sciatica and back pain that typically increases with standing /walking.
Spinal compression fracture - sudden onset of pain especially in postmenopausal women, osteoporotic individuals and long term steroid use, especially in asthmatics.
Less common causes can be:

  • Infection
  • Ankylosing Spondylitis
  • Coccydynia
  • Piriformis syndrome
  • Spinal tumours

Treatment

Treatment depends upon primary cause of pain. A large number of patients improve by conservative treatment or natural healing but short period of rest, reduced activity, analgesics/painkiller medication and physical therapy etc. help in recovery.

In some acute and non-responding cases, patients need Injection therapy where medicine is directly injected in and around the painful nerve/joint that provides short to long term relief.

In patients not responding to conservative/injection therapy, surgical treatment is offered. Depending upon the cause it may vary from keyhole surgery, microscopic surgery, minimally invasive spine surgery or open surgery. Contrary to public perception, spinal surgeries achieve the desired goals in 80-90% of the cases when correctly indicated and there is hardly any risk of paralysis nowadays.

Hello lybrate user, back pain that is associated with day time sedentary jobs like computer professional s are associated with paraspinal muscle spasms. The best way to tackle this sort of pain is to do well spaced stretching exercises of the back, back extension exercises, using hot water bags to provide relief and avoiding stooping posture while sitting. Posture also plays a very important part in back pain, always maintain a straight posture while sitting. While getting up from the bed try to first go on your left or right side of the body and get up side ways. Avoid cruntches or sit-ups: they may increase the pressure on your back and may predispose to slip disc. Always maintain a healthy lifestyle in terms of regular brisk walking, meditation, eating healthy. P. S: try to avoid pain killers (analgesics) as much as possible as many are associated with side effects that you may not be aware of. Stay healthy. Stay happy god bless you.

several lifestyle changes may be required. Most back pain gets better with a few weeks of home treatment and careful attention. Over-the-counter pain relievers as prescribed by me may be all that you need to improve your pain. A short period of bed rest is ok, but more than a couple of days actually does more harm than good. This is ankylosing spondylosis. Ayurvedic treatment has best result in this. We have specialised therapies for ankylosing spondylosis. Exercise is very important is this. . Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Hi thanks for your query and welcome to lybrate. If you have have localized neck pain with no other symptoms like radiating upper limb pain, neurological symptoms like numbness, weakness, paresthesias etc and with no other associated constitutional symptoms like evening rise in temperature, loss of weight, excessive sweating at night etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your neck while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. We will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of neck and few blood tests for evaluation. Hi dear lybrate user if you have back pain since long time you must always consult a doctor in physical who can examine and tell you why this is happening. But if it is a mild and pain of recent origin, modern medicine would suggest you some painkillers and muscle relaxants, but these have temporary results only and long term usage of these medicine have side effects. So the best treament would be homeopathic medicine (like bryonia and rhustox are common homeopathic medicine) for internal medication and along with this take phyisiotherapy session for 3-4 days only and you will be completely fine.

Dear, Osteoarthritis is age-related wear and tear of your joints and is expected with aging in almost everyone. Most of older gentlemen have weakness of the muscles around the knee -the quadriceps, hamstrings and calf muscles. That adds up to the strain on the knees and increases the pain. The treatment is multidirectional. Avoid climbing up and downstairs, low dose pain killers (as advised by your family practitioner) will reduce your acute pain. Do not squat for your toilet. Use the higher (western) commode. Start on a few sessions of physiotherapy to strengthen your muscles. Make a practice of regular walking. If pain is excessive make use of a walking stick support.
