Normally 1 year every time I do test . Please tell what medicine to take for vitamin - D . It came less in the test.

PGDD, RD, Bachelor of Home Science
Dietitian/Nutritionist, Mumbai
Thanks for the query. Vitamin D is essential for bone health. If you don't spend enough time in the sun or if your body has trouble absorbing the vitamin, you may not get enough. Firstly it is important to understand that you need to have a balanced diet to help absorption of all vitamins. But there are some things you can do to increase the Vitamin D levels. Firstly get sunlight. Sunlight spurs the body to make vitamin D. A small amount of sun exposure without sunscreen can do the trick. If you're going to get it from the sun, about 20 to 25 minutes of exposure is helpful. Include in your diet foods like fatty fish like salmon and mackerel, an egg yolk, dairy, fish oil, clarified butter and nuts. Also you may have to take a supplement as per the physician's advice.

Mr. lybrate-user, Diabetes affects blood supply to penile muscles causing erectile dysfunction. Hence, strict blood glucose control is very essential. Whether the remedy mentioned is useful or no, I have no idea. But strict glucose control is a must. Thanks.

How to Diagnose and Manage Acute Coronary Syndrome

DM Cardiology
Cardiologist, Delhi
How to Diagnose and Manage Acute Coronary Syndrome

The word acute coronary syndrome refers to a group of symptoms that are caused by blockage of the blood flow to the heart muscles.  The most common result of this is myocardial infarction or heart attack as it is popularly called. Reduced blood flow leads to death of some portion of the heart muscle wall. While the word heart attack sounds almost fatal, it need not be the case. Knowing how to identify an attack and being aware of some simple measures can help save lives.

The tell tale signs of a heart attack are as follows:

  • Chest pain and discomfort usually described as a tightness or burning in the chest region
  • Pain along the left side of the shoulder and neck, going up into the jaw, down to the arm
  • Nausea and vomiting
  • Profuse sweating
  • Difficulty breathing
  • Dizzy or fuzzy feeling
  • Tired, extreme fatigue
  • Anxious, apprehensive feeling

However, be also aware that there are a lot of people who experience a silent heart attack. Women, obese, elderly, and diabetic patients can have silent attacks and depending on severity, either they go on with life as usual or can have a fatal attack.

Once you are doubtful of a heart attack, the next step is to reach the closest medical facility for a diagnosis.  In addition to a detailed examination and history, the following two tests will be performed.

  1. Electrocardiogram (ECG): A 12-lead ECG will measure electrical activity of the heart and identify irregular electrical activity, which is indicative of a myocardial infarction.
  2. Blood tests:  Presence of certain enzymes in the blood, CK-MB and troponin are indicative of a heart attack.  A complete electrolyte profile also will be done, and increase or decrease of some electrolytes is helpful in diagnosing a heart attack.
  3. In addition to these two, chest radiography, cardiac angiography, echocardiogram, stress test, and computed coronary tomography may also be required to confirm the diagnosis.

Once diagnosed, the first step would be to relieve the symptoms, negate the effects of reduced blood flow, and restore cardiac function.

  1. Dissolve the clot: Using thrombolytics like clopidogrel
  2. Nitroglycerin: To dilate the blood vessels and improve blood flow, especially to the heart muscles
  3. Anticoagulant therapy: Blood thinners are usually used to avoid blood clot formation; aspirin and heparin are the most commonly used agents.
  4. Drug therapy: Blood pressure maintaining drugs like beta blockers and/or angiotensin-converting enzyme (ACE) inhibitors are also used
  5. Use of Statins: Statins are used to reduce the amount of cholesterol in the blood and stabilize plaque deposits.

In very severe cases, angioplasty and stenting or coronary bypass surgery may be required. Educating people on how to identify a heart attack and manage it is very useful and can help save lives.

FLU (also known as the INFLUENZA) is a contagious respiratory illness caused by flu viruses. It can cause mild to severe illness, and at times can lead to death. The flu is different from a cold. These droplets can land in the mouths or noses of people who are nearby or possibly be inhaled into the lungs-------------------------Method of prevention after a Flu infection---- 1. People should stay away from sick people and stay home if sick. 2. Wash hands often with soap and water. 3. Linens, eating utensils, and dishes belonging to those who are sick should not be shared without washing thoroughly first. 4. Eating utensils can be washed either in a dishwasher or by hand with water and soap and do not need to be cleaned separately. 5. It's been two years since I am married. Now we are planning for a baby. In this last two years we never had any intercourse. We used to do orgasm. Recently we had a intercourse which lasted for two mins or so. I want to know whether sperm comes out while doing intercourse or not. And also does this regular orgasm affects sex life.

MBBS, MD - Obstetrtics & Gynaecology
Gynaecologist, Gurgaon
yes sexual intercourse even for two mins does allow sperm to enter the female genital tract. Suggest you should have a regular sexual life in order to conceive, rather than masturbation.

He has been smoking from an year and smokes around 3-5 cigarette's a day Sometimes his right side of the chest starts pumping for some reason Is it because of smoking or some other reason? Sometimes he even has a very very mild pain in his heart.

Diploma in Obstetrics & Gynaecology, MBBS
General Physician, Delhi
At 17, it is good to know that you need counselling for others to stop smoking, he should be a responsible citizen as his smoke is affecting his near and dear ones through inhaling secondary smoke. Since you know that his problem is due to cigarette smoke, do help him give up through his strong will power minus any substitutes like ecigarettes etc.
