Purpose and Types of Brain Tumor Surgery

Although surgery is considered to be the first step of treatment in any form of tumor but its feasibility depends upon the type, size and location of the tumor. It is not necessary that every kind of brain tumor might require surgery.

Indications of Brain Tumor Surgery

  • Diagnosis of the type of tumor by taking a sample of the tumor for laboratory examination
  • Complete resection of tumor
  • Removal of the tumor as much as possible to slow down its growth and improve the symptoms.
  • Remove the tumor in order to help other treatment
  • Provide direct access for other forms of treatment like chemotherapy, radiotherapy etc.
  • To relieve pressure caused by tumor on surrounding structures

Types of Brain Tumor Surgery

The different types of surgery that are performed in cases of brain tumor include the following:

Craniotomy: Craniotomy is the most commonly performed procedure to remove brain tumor along with a piece of bone. The removed bone is replaced and the tumor is sent for histopathology (biopsy). The surgery is performed using a high end microscope by any of the following techniques:

  • Extended bifrontal craniotomy
  • "Eyebrow" craniotomy (supra-orbital craniotomy)
  • "Keyhole" craniotomy (retro-sigmoid craniotomy)
  • Orbitozygomatic craniotomy
  • Translabyrinthine craniotomy

Shunt: A narrow piece of flexible tube with a pressure regulated valve in between is called a shunt. This is used to relieve the intracranial pressure caused by obstruction of the natural brain fluid (CSF) pathway by tumor mass. The procedure involves insertion of a drainage system into the brain to remove or drain excess of blocked fluid into the peritoneal cavity.

Stereotactic surgery: This surgery is done by creating a three-dimensional image called stereotaxy using computers. It aims to find out the location and position of the tumor. In fact it can also be done to aid tumor removal, implant radiation pellets or for other treatments.

Embolization: It is a procedure used to reduce the amount of blood supply to a tumor by blocking the blood flow in the selected arteries. It is generally performed before the main surgery.

Endoscopy and endoscope assisted surgery: Endoscope is the tool which helps to closely visualize the tissue through small openings in difficult to reach areas. This tool can be used in various brain surgeries to precisely reach the target location without damaging the adjacent structures.

Adjuvant modalities to improve outcome: In addition to above mentioned surgeries, following surgeries may also be performed in relation to brain tumor treatment:

  • Laser surgery
  • Photodynamic laser surgery
  • Ultrasonic aspiration
  • Fluorescent-guided surgery
  • Electrophysiological monitoring

Respected doctor, my son is 3 1/2 year old and is suffering from epilepsy, as now at this stage he sits properly, but not standing or walking and not talking as little bit he is trying now but not up to that level, the treatment is going through" neurosuregon" as epilepsy medicine is being given to him the response is good. Please guide me what are the chances & how many days it will take & what are the medicines & excercises for him.

Is there some history of delayed cry or perinatal anoxic ischaemic insult as he is having delayed milestones. It seems that he is having cerebral palsy. Needs evaluation by neurologist.

My father (45 years) is having fits. He has dat problem since childhood but its vry sad n depressing to see my father when he gets it. He had one yesterday n even today which lasted for 2-3 minutes. Hello lybrate-user, Fits or seizures is due to several reasons. If he has it since childhood and the MRI brain is normal, it is likely generalized epilepsy. Epilepsy means disease where a person has tendency to have seizures or fits. Usually it is fairly controlled with medications. Sometimes when it does not respond to medications, patient has to be on 2-3 seizure medications to decrease the frequency of the seizures. Few of the seizure medications need some monitoring of blood electrolytes but there are many safe seizure medications available in market. I will also get a brain wave test called EEG. Any alternative treatment for pituitary macroadenoma of size 4.5×4.7cm? I would not like to go for surgery.

Pituitary macroadenomas often require surgical intervention for cure. The exceptions to this rule are the macroprolactinomas, which usually have an excellent response to medical therapy. Dear sir madam I did sex with a prostitute 3 and half month ago. I have been getting numbness and cramps in leg and hand and tingling and loose stool twice a day. Hello lybrate user, the symptoms you are describing are similar to what we call claudication. In common language, it is development of symptoms of pain etc in the lower limbs after one does some previously accustomed activity ie one was able to do those activities easily but are causing symptoms now. It occurs due two causes. Firstly, vascular problem causing decreased blood supply to the muscles of the lower limb which is now inadequate to maintain the demands of the muscle while doing activity. Secondly, neurological problem in which there is stenosis in the spinal canal which transmit the nerves from brain to the lower limbs. This stenosis causes decreased space for the nerves and their vessels causing pain in the lower limbs. With the details you have provided, and your profile, it looks more like a spinal canal stenosis. I would suggest you to see a spine surgeon or an orthopaedician for a detailed assessment. I would also suggest you forward bending exercises and exercises to strengthen your abdominal muscles. Hope this helps.

Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 1 wk, contact me again. Do not ignore. It could be beginning of a serious problem.

Hi doctor, I'm very fearful while picking up the client call and strange people around me. My voice tremble like anything. Though, I know the answer to the client call, I couldn't explain to them comfortably. I feel ashamed when I finished call as I had a bad call. You are just afraid of people and their judgments of you. This is closely related to the fear of public speaking, which everyone has, including me. But you will not notice it because we use the fear t prepare and deliver the talk properly. You can imagine that nobody is there and just speak naturally. You are obviously good but have a low self-confidence and esteem. It is all about trusting people. Do you know why you do not trust people? Have you had a bad experience? If not, you will still need to meet people and live with them. The ultimate test of a personality is in the social context. The greater test is when we meet with the opposite sex. And the final test is when we have to bed and board with a member of the opposite sex in marriage. If you wait for that time to resolve it, it might be too late and traumatizing. Even if you are afraid of people, face your fear and see what happens. They are certainly not going to kill you. So what is the worst that can happen? Really nothing but it will feel great to have friends and lots of companions in life. We are after all social animals, gregarious by nature and need each other. So don’t handicap yourself on this front. Attend a personality development class, personal grooming class, public speaking class, and conversation classes. It will help to do a lot of this with a close friend, if you have one. If not, find one. We are into relationships all the time. It is near impossible to live without people. All research has shown that living with someone gives us good health and long life too.

Migraine is a type of headache which can occur in varied intensities; the pain resulting from a migraine is pulsating and throbbing. This sort of a headache is usually accompanied by extreme sensitivity to sounds and light as well as severe nausea. One feature that significantly differentiates between migraine and any other type of headache is that the headache resulting from migraines usually develops on any one side of the head.

Causes of Migraine

The exact cause behind migraine is yet to be fully understood. Migraines are thought to be a result of combination of genetic and environmental factors.In majority of cases, migraines tend to run in families . There are few well known trigger factors for migraine. These are – stress, exertion, gastric disturbances, eating heavy food stuff, taking alcoholic drinks, coffee intake, hormone changes in females.

Symptoms of Migraine

The most characteristic symptom of migraine is one sided headache mostly of violent, throbbing and pulsating in nature. The accompanying symptoms include nausea, vomiting, blurring of vision, flashes of light before eyes, double vision, light/sound sensitivity, confusion and weakness.

Homeopathy is not in the favour of using temporary treatments like popping a painkiller to alleviate the throbbing, pulsating and often intolerable pain caused from migraine. Homeopathy aims at going deep down into the root of the problem and entirely removes the tendency of any patient to be shot with migraine headaches every now and then. Glonoine for migraine with congestive headache: When you are struck with congestive headache, a rush of blood reaches the head region causing redness in face and head area with intolerable throbbing pain. You may even feel shocks around the head which get aggravated when exposed to heat or sunlight. It becomes impossible to tolerate any kind of warmth, and you may feel a bit relieved when you uncover the head. If you are affected with congestive headache, then consuming Glonoine can be of immense help.
  3. Natrum Mur for migraine from grief and stress: This medicine is widely used for alleviating migraine caused from stress and grief. Natrum Mur is also effective in alleviating anaemic headaches, which mostly affect young women. It often leads to temporary sightlessness, and the pain feels as if thousands of small hammers are being throbbed within the brain. The pain tends to rise up after through the day, and the patient may feel better after sunset. If the pain gets triggered when the eye is strained, then Natrum Mur is the medication for you.

Cerebral atrophy is a common feature of many of the diseases that affect the brain. Atrophy of any tissue means a decrement in the size of the cell, which can be due to progressive loss of cytoplasmic proteins. In brain tissue, atrophy describes a loss of neurons and the connections between them.
