Dear Lybrateuser, - For your problem take more of fibrous food like fruits & vegetables, beans, whole grains, legumes, sprouts in your diet - avoid fried, processed, junk food, also alcohol, tobacco if you take - for gout restrict non - veg, veg like cauliflower, spinach, fried foods - have 8-10 glasses of water to remove toxins from body - do regular exercises like walking, jogging for 30 min, do yoga, meditation & deep breathing for 10-15 min daily morning - check your blood pressure & uric acid level regularly, take medicines on time.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are many causes for the swelling in leg, it can be postural, decreased proteins due to deranged lft, kft, cardiac causes, vascular causes, hypothyroidism etc. My advise will be to get a basic physician workup done to rule out any medical cause for it, and if you still need guidance, you can then contact me. Meanwhile ask her to keep her limbs elevated, use ice packs, compression bandages till she can go and get tests and consultation done. Hello ... take tab. paracetamol in fever thrice a day after meal. and tab. perinorm -5 when vomiting. drink warm water. take liquid diet, dalia, juice, veg. soups, plenty of water. if symptoms persist then do tests cbc, mp,widal , blood sugar, urine r/m. and consult again. welcome in lybrate.

Dear, if you tired very fast and have headache. Then you will definitely get relief by following these very useful tips. * you should take proper balanced diet first. That means you have to take vegetables, milk, chapattis or rice, butter / cream, cereals (daals) or cheese every day. Very important thing, do not miss or skip your meals at any cost and take meals on time * drink atleast 2 litres of water daily. * proper 6-8 hours sleep is very important. If your sleep is less than 6 hours, then you feel tired very early. So complete your sleep. * do breathing execises daily in morning & evening. Just sit down in peaceful place, close your eyes, take deep and long breaths in and out. Do it atleast for 10-15 minutes daily. You can also do pranayams like anulom vilom for better results. *take a walk, in first walk normally for 10-15 minutes for 15 days. Then slowly walk fast and slowly start running. You can do it on treadmill also. * you can take ashvagandha churn of baidyanath 1 teaspoon with milk daily for 2 months atleast to get your strength back. * check out your haemoglobin level in 3 months, if it is low you get tired early. So get treatment of low haemoglobin (anemia). * avoid all fast foods and habit of smoking or drinking.

CHIKUNGUNYA is caused due to a viral infection (CHIKV) and is transmitted when an infected female Aedes aegypti mosquito carries the virus from an infected human to a healthy person. The mosquitoes mostly attack during the day and are commonly seen breeding around clean water bodies. Chikungunya means ‘to distort’, directly relating to the stooped appearance of the patient due to extreme pain in and around the joints during the disease. The disease is characterised by an abrupt onset of fever frequently accompanied by severe muscle and joint pains. Once bitten, the patient will experience symptoms such as arthritis like pain around the joints, muscle aches, fever, malaise, headache, fatigue, nausea and in some cases vomiting within approximately two to 22 days.
Chikungunya is a self-resolving disease. It gradually decreases in intensity in about a week after infection. But some people may experience arthritic pain and fatigue for a few weeks to a year after it has resolved. In very old and ailing patients and in individuals with a weak immune system, the disease can result in death.
Blood tests can help diagnosis. Doctors can now quickly and accurately identify patients at risk of severe form of chikungunya with the help of a direct biomarker. There is currently no cure for the condition. Medications are given for symptomatic relief from pain, swelling and fever.

CAUSES
Chikungunya is caused due to a viral infection (CHIKV) and is transmitted when an infected mosquito carries the virus from an infected human to a healthy person. They are commonly seen breeding around clean water bodies and mostly attack during the day.
Usually confused with dengue and in some cases malaria, chikungunya is a self resolving disease (the condition gradually decreases in intensity in about a week after infection) that does not have a cure. As an exception, some people experience arthritic pain and fatigue for a few weeks to a year after it has resolved.
SYMPTOMS
A condition that is spread by the female Aedes aegypti mosquito, ‘chikungunya’ means ‘to distort’, directly relating to the extreme pain the patient experiences in and around his/her joints during the disease. Once bitten the patient will experience symptoms such as
Arthritis like pain around the joints
Muscle aches
Fever
Malaise
Headache
Fatigue
Nausea
Vomiting (within approximately two to 22 days of infection, in some cases)
Here are how the symptoms of chikungunya are different from malaria and dengue.
There are very subtle differences between the three mosquito borne diseases and one of the most clear difference is the type and frequency of fever a patient experiences.
In the case of dengue a patient suffers from high fever that appears and subsides quickly only to reappear with rashes and in the case of dengue haemorrhagic fever, along with bleeding.
In malaria, the patient suffers from high fever, most predominantly in the evenings with chills followed by sweating in quick succession.
In the case of chikungunya the patient suffers a high fever with severe pain in the body especially the joints along with all the symptoms mentioned above. In all three conditions the patient will experience a temperature of about 102o .
DIAGNOSIS
Most doctors do tend to diagnose the condition using a process of elimination – by testing for dengue and malaria first then followed by tests for chikungunya.
The diagnostic test for the condition is a simple blood test where the sample is tested serologically using the ELISA test (Enzyme-linked immunosorbent assays) or PCR (polymerase chain reaction) to check for the presence of the anti-chikungunya antibody in the patient’s blood.
If the patient is infected, the amount of immunoglobulin is the highest in the first two weeks after the onset of symptoms and continues to be present in the body for about two months after that.
TREATMENT
There is currently no treatment for the condition and doctors prescribe medication to give symptomatic relief to the patient. Drugs to reduce the pain, swelling and fever that appear with the disease are usually the first line of treatment. That being said there have been a number of research studies to identify a possible vaccine and treatment for the condition.
A study published in the American Journal of Tropical Medicine and Hygiene in 2000 said that they had developed a vaccine that showed 98% efficacy in its patients. This vaccine is still not available in the market. Other studies on a possible DNA vaccine for the chikungunya vaccine is still underway.
Although this condition has no known complications it can cause death in very old and ailing patients.
If you do think you might have the condition it is extremely important to get diagnosed early, since the symptoms overlap with conditions that have serious complications like dengue and malaria.
PREVENTION
Here is what you should do to prevent from getting infected with the disease:
Since infected mosquitoes, bite during the day, it is advisable to use a mosquito repellent cream while stepping out, especially in endemic areas.
Use repellants within the house and use them as per the recommendations on the packaging.
Keep all water reservoirs like buckets, small tubs, mugs etc. covered at all times. It is essential to empty out all the containers regularly and wash them well.
Declutter your home and air our all the rooms once a day. Remember to close all your windows and doors by mid morning.
If you have too many mosquitos in your area, ask your area municipal corporation to get the area sprayed to rid it of mosquitoes.
Finally, what you need to remember is that if you experience symptoms like fever, joint pain, malaise, fatigue and vomiting make sure you visit a doctor at the earliest. Do not self medicate because it could be something more serious

1. Take paracetamol 500mg, 1 tablet as and when required after food up to a maximum of 3 tablets daily, 2. Drink plenty of water 3. Take adequate rest 4. Have adequate sound sleep in the night for 7-8 hours if you have recurrent attack of headache1. It could be a tension headache due to inadequate sleep stress and strain, low bp/high bp, migraine, prolonged use of cell phone/computer, chronic anaemia, refractive error, chronic sinusitis, organic brain lesion 2. Avoid stress, physical and mental exertion, avoid pronged use of cell phone 3. Go for regular exercise 4. Practice yoga, meditation and deep breathing exercise to calm your mind, control your emotion and relieve stress 5. Check for refractive error, sinusitis hemoglobin, bp 6. Consult neurologist to rule out other causes of headache

Hi do regular walking/exercise for atleast 45 minutes daily. Eat lot of green leafy vegetables and also get back to me with the type of medication as there are some diabetic drugs which will cause weight gain.

  • Floss at least once everyday
  • Use a soft bristled toothbrush
  • Remove your dentures and clean them regularly. Do not sleep putting them on
  • Quit smoking, chewing tobacco and chewing gums with sugar in them

Hi, Please follow these tips to stop snoring: - Reduce your weight if you are obese. - Perform pranayama (breathing exercises) everyday. - Quit smoking which irritates the mucosal layer of the nose and throat and causes blockage and results in snoring. - Quit alcohol and sedatives which relax the throat muscles and results in snoring. - Sleep on sides instead on back. - Elevate the head by using a thick pillow which eases the breathing. - Clear the nose before going to bed. - Establish regular sleep pattern. - Keep the bedroom air moist by using humidifier; as dry air irritates the mucosal membrane in the nose and throat. - Perform throat exercises like repeating the vowels a, e, i, o, you loudly for 2-3 mins few times a day or keeping the tongue behind the teeth and moving it backwards - 3-4 times a day. - Anti snoring appliances can also be used.

I am 70 years old. I am recently observing acidic smell after passing urine at first time in the morning, no burning sensation. No color. While walking some times I am getting burning sensation in lower part of the feet so I got blood tested and found sugar first time earlier it was normal at free and post lunch. I was learning car driving and under went some tension. I thought that may be the reason (stress diabetes)

Sir, it is better if you get a detailed blood test for diabetes, which includes fasting, after food sugar levels, hba1c% and lipid profile and also routine urine examination and inform the results. With those details I can guide appropriately. If diabetes is present whatever type it may be, treatment has to be started. I await your details.
