Diagnosing the Cause of Chest Pain!

In addition to pumping blood to various organs, the heart also has its own blood supply, through which it receives its oxygen and nutrient supply. In patients with coronary artery disease and atherosclerosis, there is a narrowing of the blood vessels which reduces the amount of blood flow to the target organs, including the heart.

When this happens in the heart, there is chest pain due to overexertion of the heart muscles. There could be two patterns to this chest pain. With regular exertion like exercise, there would be chest pain, and most patients are familiar with this pattern.

This is known as angina pectoris or stable angina. In some people or in some instances, chest pain occurs which is sudden and not of a predictable pattern. It could be related to extreme exertion or stress. This is known as unstable angina and can lead to heart attack and be life-threatening.

Stable angina or angina pectoris has a stable, predictable pattern which most patients get familiar with over a period of time and learn to manage. Read on to know more about the signs and symptoms and management techniques.

Signs and symptoms: Stable angina usually occurs after a round of physical exertion. The patient feels a feeling of tightness in the chest which feels like the chest being squeezed. The pain can gradually spread to the shoulder, arms and even the neck. The pain can also be induced by eating, exposure to cold, emotional stress. It lasts for about 15 minutes and is relieved by rest and sublingual nitroglycerin. The pain intensity does not change with position or coughing. In addition, the patient may also experience shortness of breath, fatigue, profuse sweating, nausea, and dizziness.
The patient may be able to detect signs on further testing including ECG, echocardiography and stress testing. Features like cardiomegaly, altered ejection fraction would be detected based on the severity of the disease.

Treatment: Immediate treatment to relieve the pain includes resting and sublingual nitroglycerin. On an ongoing basis, the treatment would include 3 approaches – lifestyle changes, medications, and surgery.

  1. Lifestyle changes: Regular exercise, smoking cessation, reduced fat intake, reduced alcohol consumption, weight loss, and stress management are some lifestyle changes to be made to improve symptoms.
  2. Medications: A number of medications would be used depending on patient’s symptoms. Aspirin to prevent clotting, medications to control blood pressure and cholesterol and diabetes.
  3. Surgery: In patients with advanced coronary artery disease, revascularization methods may be required, which includes angioplasty and coronary bypass.

In a person with known risk factors, it is advisable to have regular checkups so that the disease progression can be controlled and symptoms managed with minimal intervention.

Dear lybrate-user, you are dependent on alcohol. Alcohol is a ready made sugar/glucose, fully digested. It will difficult for you to control your blood sugar level if you continue drinking. Cholesterol is produced in the body by liver. Probably your liver is damaged causing cholesterol problems. High creatinine level shows your kidneys are under protest. They may give up if you don't get treated. It is better for you to give up alcohol completely. It can not be done by yourself at home. It will have withdrawal symptoms which needs treatment in a hospital for at least 10-14 days. During detoxification itself your sugar, cholesterol and kidneys can be repaired with appropriate drips and tablets. You are still young. I think your children may be still dependent on you. Probably your parents may not be able to support. So there is an urgent need for you give up alcohol in a hospital and get treated for other medical problems. All the best.

CHEST PAIN in your case may be due to 1.acidity. 2. severe anxiety 3. muscular problem A. You can take Tablet Pantocid 40 mg one tablet before food for 3 days B. Local application of Volini gel 1% twice a day for 2-3 day you should go for BP check along with Base line ECG and review with report I am giving you some health tips to avoid acidity 1.Take small frequent meal kindly take 5-6 small meal in whole day 2.Take 6-8 glass of water in a day 4. Moderate physical activity regularly 5.avoid excessive tea, coffee 6. Do Not postpone Breakfast 7. Avoid spicy food Consult physician for further management

Hello, I am high BP problem since last 2 years and I am taking BETACARD 25 mg tablet for it. Now I want to stop this tablet as it may have side effects also. Is there any medicines in ayurveda or homeopathy which will not have any side effect and cure BP problem. Your BP is in control because of the medication. If this medication is leading to side effects, you can request your treating doctor to change it. There are other medications available as well. Also follow diet precautions and exercise daily for 40-45 mins. Prescribing medication online without seeing your records, as well as Blood pressure trend, will not be a good idea, so consult a cardiologist locally and stay connected with them, because BP medication require titration from time to time.

Mr. Lybrate-user, from your description it appears that both diabetes and hypertension are not in control. Bp should ideally be 120 to 130/70 to 80 mm hg. Your bmi is 24.91 kgs/m2, which is marginally high (normal <23 kgs/m2). You have not mentioned anything about blood sugar level and hba1c%. Unless all details are made available, difficult to comment. However, as regards bp you have to initiate treatment immediately. Thanks.

Home remedies to Control high BP:-1. Bananas are one fruit that people with high blood pressure can eat regularly to control it. Bananas are a rich source of potassium, which lessens the effect of sodium.So, try to eat one or two bananas daily. Along with bananas, you can try dried apricots, raisins, orange juice, spinach, baked sweet potatoes. 2. Eat one or two crushed garlic cloves daily. You can simply crush them with your hands. Crushing garlic cloves creates hydrogen sulfide, a compound that promotes good blood flow, removes gas and reduces the pressure on the heart.If you do not like eating raw garlic or if it causes a burning sensation, then take it along with a cup of milk. You can also mix five or six drops of garlic juice in four teaspoons of water and take it twice a day. Dear, Heart attack symptoms typically follow a classic pattern. This list includes several common signs and symptoms of heart attacks. Chest Pain The most common and regularly documented symptom of a heart attack is chest pain.The pain can feel like anything, but usually is dull or aching.Heart attack chest pain is usually very steady. It doesn’t typically increase or decrease with movement or palpation (touching or pushing on the chest). It likely will come on suddenly, without warning. Chest pressure Pressure or heaviness in the chest can be independent of chest pain, but often goes together. Pain in the arm, neck or jaw Shortness of Breathing Most often, pressure in the chest makes the person feel as if he or she is being suffocated.Heart attacks can also cause fluid in the lungs, which leads to severe shortness of breath. Sweating Heart attacks cause stress on the heart and it tells the brain, which leads to the “fight or flight” stress response. Nausea Like sweating, nausea is a very common response to stress. Fatigue The same decreases in blood flow that can lead to passing out or trouble breathing also cause fatigue. Syncope As a heart attack progresses, damaged heart muscle leads to a loss of blood pressure and decreased blood flow to the brain.

Hello, Pain in left side of chest could be cardiac, bony, muscular or gastric in origin, need to rule out the possibilities. Get yourself examined thoroughly from a nearby hospital. Get an ECG done. Management: We need to make few changes in our life to stay fit and healthy (if there is no medical condition): - Just walking alone is a very beneficial exercise which can help you stay in shape, feel more energetic, and get healthy. Always take stairs rather than taking an elevator. -Standing while you work can help your body get more activity than if you were just sitting down. Spend at least some of your work day standing instead of sitting in order to help your blood flow and maintain your muscles. -Aim to exercise for a total of 30 minutes per day and you’ll easily be able to maintain your fitness level. -Some exercises will be more efficient than others, and choosing these more efficient exercises means that the small amount of time that you do spend exercising will really count. Try to choose exercises that focus on your core muscles, since your legs and arms can get activity through daily activities like lifting bags of groceries and walking a longer distance from your car. Try squats, planks, burpees. -Get indulge into some activity at home like riding a bicycle, swimming, playing active games with your kids. -Eating right. Cut out unhealthy fats and take in healthy fats. The easiest way to identify unhealthy fats is to look for “solid” fats, or fats that are solid at room temperature. Get plenty of protein. Protein will help you maintain your muscles and also give you the energetic feeling that you may be missing now that you’re getting less exercise. Good protein sources include skinless chicken, turkey, salmon, anchovies, sardines, oysters, almonds, walnuts, peanuts and peas. Eat properly portioned, balanced meals. Generally, about ⅓ of your plate should be fruits and vegetables and ⅓ of your plate should be grains. The final third should be a mix of protein and low-fat dairy (cottage cheese, eggs, non-fat milk, etc). -Drinking enough water also has the added bonus of keeping your hunger level down, helping you to maintain the lower calorie count that you’ll be getting now that you’re not exercising as much. -Keep a regular check on your B. We recommend intake of slow-release carbs (low gycemic index (GI) foods) to keep blood sugar levels even because they are digested more slowly. High GI foods spike the blood sugar rapidly, while low GI foods are absorbed gradually. We encourage eating regularly at set times. Eat smaller meals at regular intervals (6 to 7 times a day) as both overeating and starving could result in fatigue as well as cause blood sugar levels to fluctuate. The body is better able to regulate blood sugar levels—and the weight—when an individual maintains a regular meal schedule. We recommend moderate and consistent portion sizes for each meal or snack. •Reduce saturated fat and cholesterol from your diet. •Trim any visible fat off meat before cooking and remove the skin before cooking. Consume fish or chicken instead of red meat. •Instead of frying, choose to grill, broil, bake, or stir-fry. •Avoid using heavy cream, use low-fat milk. •Eat more foods rich in fiber, such as fruits, vegetables and whole grains. •Consume a low-fat balanced diet consisting of whole-grain cereals, low-fat/ skimmed dairy, fruits, vegetables, sprouts. •Avoid stimulants such as too much caffeine. •Drink 10 to 12 glasses of water everyday. •Lose weight, if overweight. Aim to maintain a healthy body weight. •Increase your level of physical activity (walk at least 45 minutes to 1 hour daily). •Meditation and relaxation techniques can help calm the body and mind, bringing them into a better state of balance. •Get enough sleep. A good night’s sleep helps reduce overall fatigue, anxiety and stress.
