My grand mom is suffering from cancer. What are the necessary steps in order to relief her pain?

DNB, MBBS
Oncologist, Faridabad
Which cancer and what part is affected. Please specify. For pain relief, if it is bone pain, apart from good analgesics, some times radiation therapy does wonders. We need more details about disease.

Sexual and Fertility Problems- can these be early signs of Diabetes? Be alert.

MBBS (Gold Medalist, Hons), MS (Obst and Gynae- Gold Medalist), DNB (Obst and Gynae), Fellow- Reproductive Endocrinology and Infertility (ACOG, USA), FIAOG
Gynaecologist, Kolkata

Scenario one: Mr PS, 35 years old, married for 5 years, now planning to have a child, suddenly felt that his erection of penis is not as good as it was earlier. initially he tried medicines shown in TV and newspaper and then many treatments through internet. but his problems persisted. Finally when he came to doctor, he was diagnosed to have ERECTILE DYSFUNCTION. Finally after checking, when doctor advised him blood test, it revealed high blood sugar and then he was advised anti-diabetic drugs. After starting this, his sexual performance improved much better.

So, what went wrong? Erectile dysfunction may be an early indication of diabetes. So, if you feel such probems, never ignore it and never feel ashamed to talk to doctor. If you try medicines shown in TV/ newspaper, your problem will persist and the diabetes may go to such point that your life will be threatened. So, never ignore such problem, however embarrasing it might be.

Scenario two: Mrs AM, a 42 years old lady having two children, gradually feeling of low sexual urge. Her husband tried many things but nothing worked. She also had repeated attacks of vaginal infections. After trying many things by themselves, they consulted doctor. And the cause of her LOW SEX DRIVE and VAGINAL INFECTION was found to be uncontrolled diabetes. And the story was same. She was put on insulin therapy and her conditions improved. But if she came earlier, she could have avoided insl=ulin therapy.

So, if any male or female is experiencing low sex desire, he or she should not try various types of treatments by themselves. Proper diagnosis is necessary, so that life threatening diseases are not missed. And as you all know, diabetes can sometimes be life-threatening. So, just imagine, a situation for which you feel embarrassed to talk to doctor, can endanger your life.

Scenario three: Mr MQ, a 30 years old male, is feeling pain and discomfort during intercourse and also sometimes during urination. He thought it may solve automatically after marriage. But it did not. And then he also had bleeding sometimes during intercourse and the pain increased. When he presented to doctor, doctor diagnosed it as a case of tight foreskin or PHIMOSIS. And the next day blood sugar was found to be very high. He was cured by circumcission after controlling blood sugar level with insulin. But after that he was found to have problems in retina and kidney. These are all because of diabetes. had he not ignored Phimosis, he could have avoided these complications of diabetes (retinopathy and nephropathy).

So, if you feeel any problems, particularly infections in private parts, do not ignore them Consult doctor and try to find out the cause.

Scenario four:. Mrs AR, 32 years female, having REPEATED MISCARRIAGE in previous three pregnancies. No apparent cause was found. Then blood sugar test was done properly and she was found to have very high blood sugar. Then she was advised to avoid pregnancy until sugar was controlled. She follwed the advice and then conceived fourth time when blood sugar was normal. Then she delivered a baby who was totally normal.

So, if any couple is experiencing repeated miscarriage, they should test for diabetes first.

So, never feel that diabetes is merely a disease of sugar. It can affect every organ and system. So, if you feel any problems in personal or conjugal life, however embarrasing to you it might be, never feel ashamed, rather consult doctor and have proper diagnosis done. A stich in time saves nine.

Radiotherapy for Lung Cancer

MD - Oncology
Oncologist, Hubli-Dharwad
Radiotherapy for Lung Cancer

Radiotherapy is a kind of treatment that kills the cancer cells by using high energy beams. Specialists use radiotherapy to treat a wide range of lung cancer. For early stage lung cancer, the radiotherapy may get rid of the cancer totally. For the small cell lung cancer you may have radiotherapy only. Then again you may have it with another treatment, for example, chemotherapy or surgery.

For small cell lung cancer, radiotherapy is used with or after chemotherapy. Chemotherapy makes the tumor in the lung shrink. You may have radiotherapy to your cerebrum, which tackles any cancer cells that may have spread to the brain. This is called prophylactic cranial illumination (PCI). In case of advanced lung cancer, the treatment contracts the tumor and controls it for a while. It additionally decreases symptoms like shortness of breath.

The types of radiotherapy used for the lungs include:

Outer radiotherapy: This kind of radiotherapy comes from a machine and is administered to the body through an external source. It depends on the kind of cancer and the progression of the same. You may have three radiotherapy sittings every day for around 12 days. This sort of radiotherapy is known as CHART ceaseless hyperfractionated quickened radiotherapy. A few people with small cell lung cancer may have treatment twice a day. If you are getting radiotherapy to control the side effects of lung cancer, you may have a single treatment or day by day medications for up to 3 weeks.

Inner radiotherapy: This is called brachytherapy or endobronchial treatment. This will be administered during a bronchoscopy. The radiation is given through a thin tube that the specialist puts inside your aviation route for a few of minutes. Specialists use this sort of treatment to shrink a tumor that is blocking or pushing your breathing track and making breathing troublesome for you.

These are some of the procedures that take place while you are having lung cancer radiotherapy:

  1. Radiotherapy machines are enormous. The machines are fixed in one position or are sometimes ready to go around your body to give treatment from various angles.
  2. Prior to your first treatment, your radiographers will clarify what you will hear and see. The treatment room normally have docks for you to connect to music players. So you can listen to your own choice of music.
  3. You cannot feel radiotherapy when you really have the treatment. It takes anything from one second to a few minutes. It is essential to lie similarly situated every time, so the radiographers may take a short time to get you prepared.
  4. Once you are in the right position, the staff will allow you to sit in the space for a couple of minutes. They watch you constantly on a closed circuit TV screen. They may request that you hold your breath or take shallow breaths during the treatment.

Hello, my query is regarding whether do we need follow up after undergoing surgery or not. Coming to the case history, my sister had undergone surgery for ovarian cancer in the year 2007 and then we had regular follow up for almost 3 years. She was not having any issue later on. Before 2 years she has been diagnosed as diagnosed as diabetic and on insulin from then. We are having regular consultation with diabetologist. Do we need to continue to meet surgical oncologist in regards to her previous case or else just continue what ever we are doing. Thanking you with regards.

MBBS, DNB (Surgical Oncology)
Oncologist, Delhi
Any patient of ovarian cancer has to follow up with the oncologist also irrespective of whether she is having symptoms or not. Because there are many advances in management which evolve over time. For e.g BRCA 1/2 gene testing is mandatory for all patients with history of ovarian cancer. So please follow up with the primary oncologist.

Types of cancer treatment: cancer treatment is now a multidisciplinary management in which different surgical, medical and radiation oncologist plan treatment protocol for best possible chances of cure. Advancement in all these fields and their contribution in comprehensive management results in 25-30% improvement in cure rate over previous decades. Surgery: involves surgical principles to remove tumor with adequate margins and possible draining lymph nodes regions. Improvements and understanding of surgical principles even make resection of distant disease or cure of stage iv disease in some selected cases. Chemotherapy: cytotoxic drugs are given either in injectable or oral forms which acts on uncontrolled cancer cells. Advances in this fields results in drugs with better efficacy and less side effect profile and at same time resulting in better cancer control. Targetted therapy acts on specific pathways of cancer growth. Hormone therapy targets specific hormone receptors for controlling the cancer. Radiation therapy: is delivery of radiation generated by radioactive source over the region of cancer. More and more advances in radiation technologists are emerging like imrt, sbrt to focus radiation over the specific location so as to avoid toxic effect over surrounding area.
