I am 28 year old. I have consistently elevated blood pressure over several weeks. Lowest reading was 155/98mmhg. I am overweight and have diabetes and am treated with metformin. Should my drug therapy be initiated for hypertension?

Mr. Lybrate-user, from your description it appears that both diabetes and hypertension are not in control. Bp should ideally be 120 to 130/70 to 80 mm hg. Your bmi is 24.91 kgs/m2, which is marginally high (normal <23 kgs/m2). You have not mentioned anything about blood sugar level and hba1c%. Unless all details are made available, difficult to comment. However, as regards bp you have to initiate treatment immediately. Thanks.

I am having chest pain an abdominal pain on left side for 15 days. The pain becomes worst when sometimes I miss my breakfast. I am 30 years old and office worker.

Dear lybrate user, -your pain is due to acidity, it may be caused due to long term intake of the analgesic medicine you are taking, -take medicine after food, reduce dose or take only paracetamol, also take an over the counter antacid medicine before meals -aiso do not skip any meals as more acids & juices in stomach increase pain & acidity -have meals regularly & on time, have food 2-3 hrs before bedtime, do not lye down immediately after meals -drink 8-10 glasses of water per day for good digestion -reduce stress, do regular exercises, avoid spicy, fried foods -have snacks like fruit, sandwich, yogurt in between meals.

I am having pre hypertension. I read in net chewing raw garlic 2 3 cloves a day reduces hypertension. One of my friend told it also causes problems in erection. Is it true? Or eating raw garlic increases erections?

Hello- Garlic is known for its lipid/cholesterol reducing property. Hypertension is due to narrowing of blood vessels by the deposition of this harmful lipids in the vessels which decreases the blood flow towards the organ and increases the pressure towards the wall of the vessels. This is the normal pathology of hypertension. Since garlic removes this lipid obstruction in blood vessels and reduces hypertension it also improves the blood supply of organs, and a healthy erection needs a healthy blood supply.

Dear Lybrateuser, - Heart attack occurs when the blood supply to the heart muscle is occluded due to deposition of fats in the blood vessel lining - many factors lead to it like hypertension, diabetes, increase in cholesterol, stress, sedentary lifestyle, obesity, family history, improper sleep, more of unhealthy fats in diet - to prevent it one should lead a healthy lifestyle, have a well balanced diet with more of fibrous foods like fruits, vegetables, whole grains, legumes, pulses, beans, sprouts, nuts - avoid fried, processed & junk foods - have healthy fats as in olive, canola, sunflower oil, seafoods, avocado - do regular exercise like walking, jogging, playing games, swimming for 20-30 min, also do yoga & deep breathing for 5-10 min daily in the morning - sleep well for 7-8 hrs in the night, keep your sleep & wake up time fixed - destress by reading, listening to music, follow a hobby, socialise with friends - get regular BP, sugar, cholesterol check up done.

I am 55 years now. I have undergone Angioplasty balloon surgery 5 years ago. I am taking clopilet, carvidelol, atrovastin, enalpril, vitamin and omega caps daily. For the last 4 months I have been getting pain some times severely at joints, wrists, foot and fingers. If I take paracetamol the pain willgoes off. Why the pain occurs? I am doing normal exercise and works as usual.

Your medicine regime is good. You are to do freehand exercise regularly & mild pain killer like paracetamol is quite well enough. Get following blood test to get your proper diagnosis of pain. 1 blood for tc, dc, esr hb%. Aso titre, rheumatoid factors, anti ds dna serum calcium & vit d ; x-ray bony jts & fingers.

Hi, doctor I feel like I'm rocking all the time. It's in tune with every other heartbeat and makes me feel like I'm on a boat. Everytime my heart beats my body feels like it's moving up, down and sometimes shift left and right. I feel like I'm trying to walk on the deck of a bobbing ship all the time and can no longer work because sitting still makes me ill. I also have a lot of trouble sleeping at night because my body feels like it is constantly in motion.  So tell me how I get rid of this, this is happening from past 8 days. I have no any Other issues also I have done my blood checkup CBC and BP and it comes normal please help.

At 19, good to know that your basic reports like CBC and BP are normal. Excess heart beat can be due to thyroid and stressful life with multitasking, doing lots of work at a time. Get your eyes tested too along with total lipid profile, thyroid profile and ecg, stress test. Do write back with reports and your lifestyle as a student /working girl at 19. You are still a growing up teenager and you have got to do lots in life through education, degrees get back with reports.

Hi I 40 years old suffering from chest pain tmt echo ecg all are normal , CT angio done normal but with long segment myocardial bridging in MId LAD. BP and resting heart always normal . Sometimes heart beat increase when sudden activity pls suggest.

Dear Lybrate user, As long as you make sure that the patient is eating more than 60% food comprising of fresh vegetables and fruit: all fresh food is good. Make sure the lady chews her food properly, drinks plenty of water, eats on time and of enough volume (not too much and not too less). Reduce dependency on rice, non veg, breads, cakes and artificial foods. Pomegranate, grape, alma, aelovera juices will be very good for health. Avoid packaged foods, fried and spicy. It is not harmful if you cook in a pressure cooker because of intense heat inside. Plus after cooking we take out and keep in other vessel. All mental diseases are very rare it does not affect everybody.

I am hypertensive but recently I noticed that after acute exertion for several days when I felt restless and tired, the bp was on lower side. 114/79. Why? Causes?

Hello Lybrate user according to ayurveda, Hypertension is referred to as Rakta Capa Vriddhi and it sees the vitiation of vata and pitta doshas as the main cause. Home Remedy- Mix 1 teaspoon of onion juice with an equal amount of honey. Take once a day for 1 week. Upon noticing improvement, continue for several more days. Treatment- 1-take sarpgandha vati twice a day. 2-yogendra ras. Akik bhasm works very good in case of hypertension. 3-Take pranacharya no tens capsule twice a day Diet-avoid salty diet. Oily food. My heart rate at rest is approx 50 bpm. Is it ok. I started going for a walk daily for the last one month. I'm taking a daily dose of multivitamin. Does that effect heart rate.

In medical terms you have sinus bradycardia. That is common in young and active adults, especially if you are doing regular exercise. Don't worry, you are perfectly alright.

Since last 5 days I am experiencing a savior pain on my left side of chest For 10 to 15 secs at bed time. Please advice what to do ?

Get an ecg done to rule out whether the pain is related to your heart. If there are any variations, then meet a cardiologist for further management. But if it is normal, then the pain may be due to gastritis for which you need to be treated with medications for 2-3 weeks.

Coffee Drinking Is Good For You?

It appears that, in large populations of people, drinking coffee may (on average) confer health benefits. In particular, the risk of cardiac death and of overall mortality appears to be lower in coffee drinkers, with the optimal benefits seen in people who drink 3 - 4 cups per day. Whether caffeine has anything to do with such health benefits is not entirely known. Coffee contains a host of active substances aside from caffeine - substances that are thought to act as antioxidants and anti-inflammatory agents, for instance - that may be contributing to this beneficial effect.

Epidemiological studies like this one, no matter how well they are performed, cannot possibly answer whether a specific individual will realize health benefits from coffee drinking. They can only tell us about the average response, across a large population.

We know that some individuals (probably determined by their genetic makeup) are particularly sensitive to caffeine, and may suffer severe sleep deprivation, jitteriness, or palpitations after drinking coffee (or any other caffeine-containing food or medication). We also know that caffeine sensitivity can change dramatically at various periods in one?s life (especially during pregnancy, when sensitivity to caffeine almost always increases). Drinking coffee is clearly not a good idea for everyone.

As a general rule, then, coffee drinking appears to have health benefits. But at least until routine genetic testing is available that might tell us about our own individual sensitivity to caffeine, whether coffee drinking is a good idea for us is something we?re just going to have to figure out for ourselves.

Cardiac mortality: Compared to people who drank no coffee, people who drank one cup per day experienced an 11% reduction in cardiac mortality.This mortality
benefit ?peaked? (at about 20%) for people who drank 3 - 4 cups per day, and then began to diminish as the amount of coffee consumption increased beyond that level.
Cancer mortality: Several reports over the years have suggested that coffee might reduce the risk of certain cancers. In this large study, however, there was no difference in cancer mortality between people who drank no coffee, and people who drank from 1 to 6 cups of coffee per day.

Overall mortality: Compared to people who drank no coffee, coffee drinkers had a reduced mortality rate. Again, this mortality benefit seemed to peak at between 3 - 4 cups per day (at about a 15% reduction in mortality).
