Hello lybrate user, - there might be both muscle and lower back bone involvement, need to avoid heavy lifting works for few days - such heavy lifts may lead to muscle contraction - apply ksheerabala taila daily before bath - take yogaraja guggulu 2-0-2 this will avoid present pain. - heavy lifts, bending exercises need to stop for few days bcz it may lead to slip disc kind of issues. Causes and Diagnosis of Neck Pain

A sudden-onset (acute) bout of neck pain is common. Two out of three of us will have neck pain at some time in our lives. In most cases it is not due to a serious disease or neck problem and often the exact cause for the pain is not clear. This is called 'nonspecific neck pain'. Most are probably due to minor sprains or bad posture. Full recovery occurs in most cases. The usual advice is to keep the neck active. Painkillers are helpful until the pain eases. Persistent (chronic) pain develops in some cases, and further treatment may then be needed.

Understanding the neck

The back of the neck includes the cervical spine and the muscles and ligaments that surround and support it. The cervical spine is made up of seven bones called vertebrae.

  • The first two are slightly different to the rest, as they attach the spine to the skull and allow the head to turn from side to side.
  • The lower five cervical vertebrae are roughly cylindrical in shape - a bit like small tin cans - with bony projections.

The sides of the vertebrae are linked by small facet joints.

Between each of the vertebrae is a 'disc'. The discs are made of a tough fibrous outer layer and a softer gel-like inner part. The discs act as 'shock absorbers' and allow the spine to be flexible.

Strong ligaments attach to adjacent vertebrae to give extra support and strength. Various muscles attached to the spine enable the spine to bend and move in various ways. (The muscles and most ligaments are not shown in the diagram, for clarity.)

The spinal cord, which contains nervous tissue carrying messages to and from the brain, is protected by the spine. Nerves from the spinal cord come out from between the vertebrae in the neck to take and receive messages to the neck and arms. A major blood vessel called the vertebral artery also runs alongside the vertebrae to carry blood to the rear (posterior) part of the brain.

What are the types and causes of neck pain?

Neck pain is common. About 2 in 3 people develop a bout of neck pain at some time in their lives. One survey done in the UK found that, of adults aged 45-75 years, about 1 in 4 women and about 1 in 5 men had current neck pain. Types and causes of neck pain include:

Nonspecific neck pain

This is the most common type. This is sometimes called 'simple' or 'mechanical' neck pain. Often the exact cause or origin of the pain is not known. It may include minor strains and sprains to muscles or ligaments in the neck. Bad posture may also be a contributing factor in some cases. For example, neck pain is more common in people who spend much of their working day at a desk, with a 'bent-forward' posture.

A 'whiplash' jolt to the neck

This is most commonly due to an accident involving a vehicle, such as a car crash. It can cause neck pain. See separate leaflet called Whiplash Neck Sprain for details.

Sudden-onset (acute) torticollis

This is sometimes called 'wry neck'. A torticollis is when the head becomes twisted to one side and it is very painful to move the head back straight. The cause of acute primary torticollis is often not known.

However, it may be due to a minor strain or sprain to a muscle or ligament in the neck. Some cases may be due to certain muscles of the neck being exposed to cold ('sleeping in a draught'). It is common for people to go to bed feeling fine and to wake up the next morning with an acute torticollis. The pain usually eases and clears away over a few days without any treatment.

How can I tell if the pain is due to a more serious cause?

A doctor's assessment and examination can usually determine that a bout of neck pain is nonspecific and not due to a more serious cause. The following are the sort of symptoms that may indicate a more serious problem:

  • If neck pain develops when you are ill with other problems such as rheumatoid arthritis, AIDS, or cancer.
  • If the pain becomes progressively worse.
  • If some function of an arm is affected. For example, weakness or clumsiness of a hand or arm, or persistent loss of feeling (numbness). (As mentioned, some pins and needles may occur with nonspecific neck pain. However, this is mild and usually goes within four weeks.)
  • If you feel generally unwell and have 'general' symptoms such as weight loss or a high temperature (fever).
  • If the neck bones (vertebrae) are very tender (which may indicate a bone problem).
  • If you develop any problems with walking or with passing urine.This may indicate problems with pressure on the spinal cord

Some other treatments which may be advised include:

 

  • A good posture may help. Check that your sitting position at work or at the computer is not poor. (That is, not with your head flexed forward with a stooped back.) Sit upright. Yoga, pilates, and the Alexander Technique all improve neck posture, but their value in treating neck pain is uncertain.
  • A firm supporting pillow seems to help some people when sleeping. Try not to use more than one pillow.
  • Physiotherapy:
  1. Various treatments may be advised by a physiotherapist. These include traction, heat, cold, manipulation, etc. The value of each of these treatments is uncertain, as the results of research studies looking at which treatments work best can be conflicting.
  2. However, what is often most helpful is the advice a physiotherapist can give on neck exercises to do at home.
  3. A common situation is for a doctor to advise on painkillers and gentle neck exercises. If symptoms do not improve over a week or so, you may then be referred to a physiotherapist to help with pain relief and for advice on specific neck exercises.

Treatment may vary and you should go back to see a doctor:

  • If the pain becomes worse.
  • If the pain persists beyond 4-6 weeks.
  • If other symptoms develop such as loss of feeling (numbness), weakness, or persistent pins and needles in part of an arm or hand, as described previously.

Other pain-relieving techniques may be tried if the pain becomes persistent (chronic). A pain management programme may be offered to help you control and live with your pain. Chronic neck pain is also sometimes associated with anxiety and depression which may also need to be treated.

Hi good evening lybrate-user, uric acid level 8.8 means you are suffering with a condition called gout. In which there will be lack of some enzymes which prevents the formation of protein end products the unprocessed products will get deposited in your joints and resulting pain, treatment you have to visit your doctor for anti gout medication and monitor your uric acid levels on regular intervals. Prevention by decreasing the intake of high protein diet like red meat, thank you bikram.

I am having frozen shoulder and very painful at night. . What should I do? any otc which will help me?

Frozen shoulder or adhesive capsulitis is a condition in which shoulder movements get progressively decreased, it is commonly associated with diabetes although association with other conditions is also known. Treatment includes intensive physical therapy, short course anti inflammatory medications according to one' s suitability and maintenance of calcium/ vitamin d metabolism. Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily – Lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana? Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day.

Shoulder advice. Regular stretching of shoulder muscles. Warm up and cool down is a must. Hot fomentation. Neck. Neck exercises. Neck stretching. Postural correction. Shoulder shrugs frequent breaks at work use cervical pillow use back support. Self massage the back of neck.

Apply prasarini oil or pranacharya restopain oil on your affected part then give hot fomentation twice a day. Take mahayograj guggul 1-1 tab twice a day. Take maharasnadi kwath 2-2 tsf twice a day. Avoid tea. Coffee. Junk food. Fermented food. Spicy food. Rice. Walk for atleast 30 minutes a day. Do pranayama early morning. Sleep for atleast 7 hrs per day.

I am 66 and weighs 96 kgs. Ht is only 5' 5. Suffering from rheumatic arthritis and osteo arthritis. Is there any proven treatment for this illness? tried all branches of medicines.

Kindly lose weight. Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful do hot fomentation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again. Do not ignore. It could be beginning of a serious problem.

I am 25 and from past 1 month, I feel very exhausted after the office as if there is no energy left in me.Also, I don't feel fresh when I wake up. And shoulder pain occurs when I am in ac environment.

You need to be clinically examined by musculoskeletal physiotherapist or orthopaedician asap to rule out nerve deficit for pain you can have hot water and cold water fomentation thrice a day for 10 days of 15 minutes duration each time at home.

My mother is aged 54yrs. she is having pain on both hands elbow. On the joints of the hands inside there is swelling. Why it is happening like that. What is the treatment for that?

I have undergone tkr for both knees and also an operation where my ball in the hip joint was damaged in an accident 2 years ago. I am 60 years. Obese (95 kg). I cannot bend my knees less than 45 degrees and cannot climb steep staircases. Should I still do physio? I am walking for half an hour daily and also do pranayaama. Kindly advise.

You must reduce weight. To loose weight: Step 1-stop taking sugar & fat e.g. Ghee/fried stuff. Step 2 take a plate of salad before meals Reduce no. of chapatti gradually. Continue with Yoga.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Foot pain especially in the region of soles is commonly due to inflammation of plantar fascia. I would like to ask you especially because you are a diabetic that are there any s/s of inflammation or infection like local warmth, redness of skin, swelling etc? if not, then you can start with - ice packs - anti-inflammatory medication which suits you. - physical therapy and special stretching exercises of your plantar fascia if you do not get improvement with these measures after around a week to 10 days, then get back to me. Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young and if you have not sustained any significant shoulder injury, then this pain can be due to weakness in your muscles, or some bit of soft tissue strain or inflammation. If there are no s/s of inflammation like swelling, skin redness or local warmth etc, then you can start with gentle anti inflammatory medications, ice packs for local application, analgesic spray after fomentation and see how it goes in the next 2-3 weeks, and if it does not improve, then we will have to get some investigation done for evaluation. 3 years back I was residing in delhi after that I moved to noida, when I was in delhi or earlier my skin was so fair (face, neck, hand, legs) but since I moved to noida it's being 3 years my skin going dark day by day, I didn't change anything in my life style, but still affected, I would like to know is it water in noida because in noida water is quite sour other than that nothing is changed, please help give me right suggestions, I want my fair skin back thanks.

Water may not be the only cause for skin tanning. Try best to avoid sunexposure during peak sun. Limit the use of sun exposure as far as possible or use sunscreen. Get your hormonal profile evaluated,
