I have got a severe lower back and shoulder pain and pain on the heel and toe please advice.

I have got a severe lower back and shoulder pain and pain on the heel and toe please advice.
Massage with your thumbs from the mid points of the shoulder blades on both sides towards the lower back of your head up to lower level of the ears. Use oil or water for massage. Rotate your head clockwise and anticlockwise, forward and down, left and right a few times. Do for 5 to 10 times morning and evening. Give pressure the dipping point on the portion between the thumb and the index finger on the back of the palm. Press and release for 15 times where the pain is more. This will solve the problem. Further do some wrist rotation exercise. Consult after 5 days. For further guidance and treatment.

I have knee pain from last 1.5 yr. I feel it when I use stairs and during running. I am 21 year old male. I have visited some doctors but there is no relief. I have read some online blog. I think there is damaged cartilage. Whay should I do?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are quite young to have persistent knee pain and as you have mentioned it could be due to some softening of knee cap cartilage but pain could be due to certain reasons but common ones are chondromalacia, pain due to knee instability, inflammation etc. If there is no history of any injury, and no s/s of inflammation or infection like swelling, skin redness or local warmth on touching, then I will like to advise you for conservative management initially in form of: - intensive physical therapy with focus on strengthening of quadriceps and stretching of other group of muscles - gentle anti inflammatory medications like tab etoshine or tab paracetamol can be taken in small doses for 5-7 days - adequate calcium and vitamin d supplementation as per requirement - ice packs at home for 3-4 times in a day and see how it goes for around 2 weeks time. If your condition does not improve, then we will have to get certain investigations done. Do not hesitate to contact me if you need any further assistance. Hi please thanks for your query and welcome to lybrate. I am dr akshay from fortis hospital, new delhi. Please since your pain is only 15 days old and you have also not mentioned any radiation of pain to limbs or any other associated neurological symptoms, I advise you: please - to maintain proper posture while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. Torn Cartilage - Is Surgery Required For It?

Torn or damage cartilage is not just a problem for the sportsmen. It can occur to anyone. It is generally caused due to a direct hit on the body parts. Its treatment is far more complicated than a simple bone injury. The best sign to know whether or not you have injured your cartilage is to check out your activities. If you have hurt your joint and don’t notice major symptoms, like if you are still capable of putting weight on and move the joint, you might have had a cartilage injury. It doesn’t hurt you a lot in the beginning, but might get worse later. 

Here are the initial care and self-treatment procedure: 

  1. Protection: It is the first thing that has to be kept in mind. You must protect the affected area from any further injury. Thus, providing an external support, for example, use a knee brace. 
  2. Rest: Provide complete rest to the affected joint. The rest is must during the first two or three days. Crutches may help the situation if you’ve got an injury around your knee or ankle. It is then advisable to return back to light activity over the next few days or weeks, as prescribed by the doctor. 
  3. Ice: It is best to apply an ice pack to the injured area for about 15 to 20 minutes in every two to three hours. This must be repeated in the first two to three days. 
  4. Compression: Use a bandage to provide the needed compression to the injured area to avoid any swelling and movement. This will not let things go beyond control. 
  5. Elevation: You must keep the injured area raised. Keep it supported on a pillow to reduce swelling. 

If there is an unbearable pain, take painkillers such as paracetamol or non-steroidal anti-inflammatory drugs (NSAIDs). If your condition doesn’t improve after the initial therapy, you must consult a doctor immediately. 

  1. Physiotherapy: It can be helpful in treating the affected joint. If there is any difficulty in moving the affected joint, you must opt for physiotherapy. A professional will be able to teach you exercises that will strengthen your muscles and provide support to the joint. This will help you in getting rid of the pain. It can be also beneficial at a time when you are recovering from any surgical procedure. 
  2. Surgery: If physiotherapy doesn’t work, it means you have severe cartilage damage. In that case, you might be advised to take up surgical procedures to set things in proper shape. Surgery is done either by opening the joint or by Arthroscopy. There are various types of surgery which can be done. 1st the old and successful surgery wherein damaged part of the cartilage is debrided and filled with normal cartilage from the non-weight bearing part of the joint. 2nd is ACI (Autologous Chondrocyte Implantation) is a 2 stage procedure wherein chondrocyte is grown using stem cell and implanted in defect part in 2nd stage. 3rd is multiple drilling of the area usually done in childrens.

Sir mere knee ke niche dono legs mein pain hota hai. Pehle yeh nahi hota tha actually 1 month pehle maine morning mein slow running start ki thi, jis se Maine 5 kg weight loose ho gaya. Phir maine running stop kardi to pain theek ho gaya. Maine doctor ko bhi dikhaya tha x-ray or uric acid report bhi normal thi. Par ab maine dobara slow running start ki 1 week ke bad phir pain start Ho gaya. Aisa lagta hai jaise legs joint se nikal jayegi. Running stop karne ke baad normal chalne se pain nahi hota tha, ha par jada chalne se thoda pain hota tha. Doctor pls advise me.

Get your vitamin D levels done Please visit a sports orthopaedic surgeon. An MRI of your leg may be needed and you maybe suffering from stress reaction in your leg bone (tibia) Presently you have to rest for 3 weeks and the slowly progress in activity levels. Do not start jogging and running on day 1.

Try few things for your back pain: - Apply Ice and Heat Heating pads and cold packs can comfort tender trunks. Most doctors recommend using ice for the first 48 hours after an injury -- particularly if there is swelling — and then switching to heat. You can use whichever you find comforting as long as your skin is protected. - Limit Bed Rest Studies show that people with short-term low-back pain who rest feel more pain and have a harder time with daily tasks than those who stay active. -Keep Exercising Activity is often the best medicine for back pain. Simple exercises like walking can be very helpful. But remember to move in moderation, Stay away from strenuous activities like gardening and avoid whatever motion caused the pain in the first place. -Maintain Good Posture most people have poor posture when going about their daily activities, putting unnecessary strain on their backs. -Strengthen Your Core Most people with chronic back pain would benefit from stronger abdominal muscles. If the abdominals are weak, other areas must pick up the slack. When we strengthen the abdominals, it often reduces the strain on the lower back. - Improve Flexibility Too much tension and tightness can cause back pain. By increasing flexibility you need to put an equal load throughout the body from the feet all the way up to the head. -Sleep the Right Way The amount of rest you get is important, and so is the position you get it in. Sleeping in a bad position or on a mattress without support can cause back pain. Back sleepers should put pillows under their knees. Side sleepers should place pillows between their knees to keep their spine in a neutral position. Stomach sleeping causes the neck and head to twist and can put undue stress on the back. -Use Relaxation Techniques Research shows that practices such as meditation, deep breathing and yoga, which help put the mind at rest, can do wonders for the back. -See a Specialist Developing an individualized exercise plan is essential to managing chronic back pain.
