I am a 27 years old male, I have migraine for the past 7 years. What should I do?

MBBS, MD - Consultant Physician
General Physician, Ghaziabad
Welcome to lybrate. We are here to help you. Such pattern of headache is seen in migraine. Migraine headaches are usually unilateral, associated with throbbing, vomiting sensations, photophobia and phonophobia. They aggravate on stress. They also aggravate on watching tv, loud noises, sleep deprivation. A clinical examination specially neurological examination, fundus examination are must to rule out any other neurological problem which might also require neuroimaging. I would also advise you to get your eyes tested. You should see your doctor. He will classify the headache and if it is migranous, treatment would be with the drugs like flunarizine, propanolol, amitryptilline. The drugs are decided on the basis of severity, frequency and duration of headache. I hope it helps. Stay healthy.

RISK FACTORS OF DEMENTIA

MBBS, DPM (Psychiatry)
Psychiatrist, Thrissur
RISK FACTORS OF DEMENTIA

RISK FACTORS OF DEMENTIA

Risk factors
Dementia usually affects older people. While it is possible to develop dementia early in life, the chances of doing so increase dramatically with age. One in 50 people between the ages of 65 and 70 have a form of dementia, compared to one in five people over the age of 80.
Mind your head There is no fail-safe way to prevent dementia, no magic medicines or vitamins that do this, but there is now good evidence that a healthy lifestyle and diet could reduce your risk.
Causes of Alzheimer’s diseaseIt is unlikely that there is a single cause of Alzheimer's disease. Researchers believe that many factors, including age, genetic background and lifestyle, work together and lead to the onset of the disease. Information sheet Am I at risk of developing Alzheimer's disease? Information sheet Aluminium and Alzheimer's disease.
Causes of vascular dementia Many factors can affect whether a person develops vascular disease or vascular dementia, including lifestyle, diet, and drinking and smoking habits. Some types of vascular disease are hereditary. People with high blood pressure, a high level of fats in their blood or diabetes are at an increased risk of developing vascular disease.
Learning disabilities and dementia The prevalence of dementia in people with other forms of learning disability is also higher than in the general population. People with Down’s syndrome are at particular risk of developing dementia.Information sheet Learning disabilities and dementia

Dear Sir, I had suffered epilepsy attack about 5 years back, I was advised by the neurosurgeon to go for Valance 500 twice, but after 1& half year I again suffered an epilepsy attack when I contacted my Dr. Harinder Singh he advised me to take valance OD500 mg2 tablets in the morning & 2 in the evening as per my weight, which is 79 Kg, but due to this higher medication my hands tremor, kindly advise.

MBBS, MD - Psychiatry, MBA (Healthcare)
Psychiatrist, Davanagere
Good Afternoon, Tremors are a significant side effect of the medications and I believe that you will do well with taking the higher dose since you have not had a recurrent of epilepsy. However if the tremors persist, you may need to have a revised list of medications without this side effect and there are many in the market that can cater to your needs. I believe you understand and hope you do not have a recurrent of seizures or epilepsy. I can certainly help with other medications as the need may be. Look forward to hearing from you. If you need help with booking an appointment online with me on this website either text, audio or video appointment for confidential and private consultation you may do so by contacting the help desk of Lybrate.

Few Warning Signs of Parkinson's Disease You Should Know!

Few Warning Signs of Parkinson's Disease You Should Know!

Parkinson's disease is a neurodegenerative condition that affects your brain cells which produce dopamine and thus hampering all kinds of motor functioning and other emotional responses. After the disease is diagnosed, medication and therapy can help to provide relief from the symptoms to a large extent but a permanent cure is not possible.

Causes:

The disorder does not have a definite set of causes. A small percentage of the cases have been related to hereditary factors and others have a range of environmental causes associated with them.

1. Genetic factors:

Specific gene mutations and gene variations have been proved to cause Parkinson's, such as SNCA, PARK2, PARK7, PINK1 and LRRK2. These make or recycle proteins which are linked to the disease.

Research shows that men are at a 1.75 times greater risk of developing the condition than women are. Also, age plays a role with most cases occurring after the age of 60.

2. Environmental factors:

Regular exposure to inorganic insecticides, pesticides, herbicides and heavy metals or other toxins may cause the disorder. In rare cases, Parkinson's disease occurs in the aftermath of a severe head injury or immune disorders.

Symptoms:

The symptoms can be classified into two broad categories-

1. Motor symptoms:

  • The hand tremors when in resting position
  • The grip loosens
  • Walking slows down
  • Facial muscles freeze partially
  • Speech is slurred
  • Balance is disturbed
  • Swallowing is painful
  • Urination is abnormally frequent
  • Dementia-like symptoms are seen
  • There is involuntary twitching of the fingers and toes
  • Repetition of the same movement in quick succession is nearly impossible

2. Non-motor symptoms:

  • Dental problems
  • Vision impairment
  • Skin disorders
  • Panic attacks
  • Excessive secretion of saliva
  • Extreme weight loss or gain
  • Sexual problems
  • Loss of sense of smell
  • Lowering of the pitch of the voice
  • Fatigue

If you wish to discuss about any specific problem, you can consult a Neurosurgeon.

Treating Spinal Stenosis the Right Way

MS - Orthopaedics, MBBS
Orthopedist, Hyderabad
Treating Spinal Stenosis the Right Way

What is spinal stenosis: The narrowing of the gaps in the spine or the backbone is called as spinal stenosis. This leads to pressure on the nerves and the spinal cord. 5% of cases of spinal stenosis are seen in the low back of the lumbar spine. In some cases the narrowing of the spine leads to the compressing of the nerve root, which leads to pain in the leg.

Causes of spinal stenosis: It can be caused by aging, arthritis, trauma to the spine, spondylolisthesis and tumor of the spine

Symptoms of spinal stenosis: There is constant pain in the back and legs as stenosis pinches the nerve responsible for sensation and muscle power. Other symptoms are numbness and tingling in the legs, frequent falling and pain while walking.

Treatment of spinal stenosis:

  1. A routine program with exercises and physical therapy are the best treatment for spinal stenosis. The exercises are not a complete cure but they are beneficial in keeping the patient active. A physical therapy can be supervised and once the period of the same is over people easily transition to the exercise program. The key for exercise is starting slowly in order to build strength.
  2. Generally, the patients are counseled not to perform strenuous activities as that might worsen the symptoms of spinal stenosis. In case of lumbar stenosis, patients find it comfortable when they are flexed forward. The doctor might recommend walking while being bent over and leaning when using a walker if not walking upright; sitting in recliners is also recommended instead of straight back chairs.
  3. Injections of cortisone are usually given in the epidural space. This is believed to relieve the symptoms of spinal stenosis temporarily. Though there have been studies which question the use of these injections as they lead to the risk of osteoporosis related fractures. This is still being pondered over by the doctors as a suitable treatment option as it has its own benefits and risks.
  4. Medications such as the anti inflammatory medicines like ibuprofen or aspirin are helpful in controlling the symptoms of spinal stenosis. Narcotic drugs might also prove to be helpful if used with care and for a short while. This is helpful for severe pain related to the nerves. Muscle relaxants like gabapentins might also be recommended. In some case antidepressants might also be given to provide relief from pain.

Though side effects are a concern when using these medications and the patient should let the doctor knows about their allergies.

Respected Sir, I am Sudip Goswami from West Bengal. My father suffering with Parkinson's disease from last 3 years. We had visited several hospital india including NIMHANS, AOPLLO (Chennai, Bangur (W. B) Now please suggest me the way to recover from this situation. Also suggest me the best treatment for P. D in india.

DM - Neurology
Neurologist, Hyderabad
Dear friend, unfortunate to know your father suffering from parkinson's disease. It is a disease of brain in which brain cells in particular area of brain degenerate spontaneously so the patient develop problems in body movement like tremor, shaking of limbs and walking difficulty and movement become slow and body became stiff. There is no treatment to permanently cure the disease or stop the disease till now. But patients symptoms can be controlled by medication for a significant years. But he will be symptom free as long as he uses the medication, when he stops the medicine symptoms will reappear. Some centers doing stem cell therapy for parkinson's disease, but it is in experimental stage. Depending on patient's response to symptomatic therapy, a brain surgery is done called bds in which patient may get symptomatic relief without medication for many years. But it is available in selected centres, costly, helpful in some specific parkinson patients.

Recently read that researchers believe certain foods might cure Alzheimer’s disease. Is this true? If so, which foods do this and how much would you have to consume to get the benefits?

MD - Psychiatry, MBBS
Psychiatrist, Solapur
There are certain foods that can help in avoiding onset of alzheimers dementia. Foods containing high levels of anti oxidants and omega acids are beneficial in general for nervous system. But in case of advanced cases of alzheimers, along with healthy lifestyle and good food habits, medications are necessary. Currently there is no evidence of complete cure of alzheimers either by medications or with use of any particular food items. Currents medications however prolong the progress of illness and improves quality of life for the patients.
