Quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

You have mentioned your mri findings which is ok but actually it does not correspond to your thigh weakness because thigh nerves are at a higher level. In order to answer your question in a better manner, may I advise you to get: 1) a detailed neurological (motor, sensory, deep tendon reflexes) charting done by a trained physical therapist 2) your mri images and reports in an attachment also please tell me duration of your back pain and mode of onset. I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias (electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. What medications you generally take and what has been your treatment history till now for existing problem. Do not hesitate to contact me if you need any further assistance.

Requires detailed examination and investigation. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Gout is a kind of arthritis caused by a buildup of uric acid crystals in the joints.
Uric acid is a breakdown product of purines that are part of many foods we eat. How Is Gout Treated?

First of all the cause for insomnia needs to be ruled out. So that proper treatment should be planned accordingly For good sleep, just try out these simple ways - 1. Stick to a sleep schedule- Go to bed and get up at the same time every day. Try to limit the difference in your sleep schedule on weeknights and weekends to no more than one hour. Being consistent reinforces your body's sleep-wake cycle. If you don't fall asleep within about 20 minutes, leave your bedroom and do something relaxing. Read or listen to soothing music. Go back to bed when you're tired. Repeat as needed. 2. Pay attention to what you eat and drink - Don't go to bed hungry or stuffed. In particular, avoid heavy or large meals within a couple of hours of bedtime. Your discomfort might keep you up. Nicotine, caffeine and alcohol deserve caution, too. 3. Osteoarthritis: Complications and risk factors

Osteoarthritis is the most common form of arthritis. It affects millions of people around the world and is caused due to the gradual wear and tear of the protective cartilage at the end of your bones. The condition most commonly affects the joints in the hands, knees, hips and spine although it can damage any joint in the body.

The symptoms of osteoarthritis develop slowly and get worse with time, and include pain during and after physical activities like movement. The joints may become tender and may pain even with the application of light pressure. Stiffness and loss of flexibility are some of the other symptoms. The loss of cartilage due to gradual wear and tear causes the condition. It causes the smooth surface of the cartilage to become rough, giving rise to friction.

Some of the risk factors involved that can increase your chances of developing the condition are:

  • Old age - The chance of osteoarthritis increases with a person's age.
  • Sex - A study concluded that women are more likely to develop the condition. Although there isn't an established reason, lesser physical activity is attributed as a possible cause.
  • Obesity - Having excessive body weight can be a contributing factor. This is because it puts added pressure on the weight-bearing joints, such as the hips and the knees.
  • Joint injuries - Joint injuries contracted due to sports activities, accidents or other such factors can increase the likelihood of this condition.
  • Genetics - In some of the cases, the condition is observed to be passed from generation to generation.
  • Bone deformities - Some people who are born with bone deformities and defects can have greater chances of having this condition.
  • Other diseases - Having diseases like diabetes and gout can increase the risk of the condition.

Some of the complications of this disease are:

- Joint pain and stiffness may cause hurdles in daily activities. Daily activities like changing clothes can be painful.
- Some people are unable to work any longer. Joint replacement surgeries are advised by doctors in these cases.

You have to wear corrected arch supported mcr chappals which will definitely be good for you and for knock knees you have to strengthen the knee by doing simple exercises. Knee cap should be worn to correct the knocked knees.

Why does pain occur? The mechanism of pain explained

Pain is an unpleasant and distressing physical sensation caused by disease or injury, which induces hurt and anxiety in those suffering from it.
There are many different manifestations of pain, ranging from short term (acute pain) to long term (chronic pain). Other classifications include pain in the internal organs (visceral pain), injured tissue (inflammatory pain), nerves (neuropathic pain), etc.

The reason for the occurrence of pain depends on the cause and kind of pain being experienced. In most cases, pain acts as a warning sign that the body has been afflicted by a disorder of some sort, which may have arisen due to internal sickness or external wounding. Acute pains are generally cured on their own through rest or simple medication. Chronic pains, on the other hand, are more complicated in nature and the treatment requires more elaborate diagnosis.

The mechanism of pain

Pain is an extremely personal and subjective experience and affects each individual to varying degrees. The pain signals, which can arise in any part of the body, travel through the spinal cord to the brain along thousands of specialized nerves and nerve fibres. In the brain, it is processed in the centres associated with anxiety, emotions, memory, appetite, etc. Signals and pain inputs are then returned from the brain to the spinal cord, which may heighten or diminish pain.

In some cases, pain may also be induced by damage to the brain and spinal cord, which happens after a stroke.
A constant barrage of pain signals may cause the cells at the end of nerve fibres to become over-sensitized. This is known as ‘wind-up’ and is one of the most common causes of chronic pain that occurs even though the root of the problem has been identified and treated.

The causes and effects of pain differ from case to case depending on the signals received by the brain and its interpretation. It affects the individual’s day to day activities and if persistent, can also have a harmful impact on mental health and psychosis. Pain is always handled, diagnosed and treated differently in all patients by drawing out the best possible solution to the problem.

I am a 35 years male have a problem swollen feet and knee pain and have regular pain below left chest sometime just below chest back side and sometime below right chest and now this are days my forehead use to be heaviness most of the time, before five year back I suffer from malaria twice in one year, within eight months gap, after that my stomach use get bloated several times I consult with with my doctor he ask me to take small meals in one hour gap not to eat to much at a time after few months every things was ok but in the year 2015 mid of june I felt sick, that was my first day I went to gym for physical fitness after doing a work out for half an hour or forty five minutes I was on my way back to my flat but suddenly my forehead was spinning I was getting unconscious my eyes was not visualized properly like heavy lights white and feeling vomote any how with the help of security I went to my flat and felt sleep then next day I went to a doctor and he advice for total blood test, the blood test reports were showing positive jaundice. Uric acid. Cholesterol. Alkaline phosphatase high. Uria creatinine. And hbv positive. Then doctor prescribed medicine for three months with a routine check-up after three months again doing all blood test report are showing everything was normal only usg report was showing liver is enlarge, doctor prescribed medicine for two more months with routine check-up everything was fine good, but again now this are days I am facing the problem like my feet are getting swollen stomach is getting bloated I am not feeling sleepy and not feeling hungry and getting pain again sometime below left chest and right chest below also and same table back sometime lower abdominal left and sometime getting senseless foots and hands forehead getting heavyness eyes are getting dizziness. Please doctor help me.

Firstly avoid sitting cross-legged and squatting, do hot fomentation if not having high blood sugar. Take medicines and apply local diclofenac gel. Walk with a tripod stick. If still pain persists then go for joint replacement. Do not delay if there is severe pain.

Go for fasting once a week. You can go for absolute fasting or rely on a liquid diet to digest ama (toxin). Fasting should be followed by hot fomentation or by taking a bath with lukewarm water or steam bath. After fasting and fomentation, mix powdered long pepper, black pepper and dried ginger in equal quantities and take 1 teaspoon of this powdered mix with lukewarm water twice a day. This will help you restore your digestive fire. Take one tablespoon of castor oil before going to bed, once or twice a week. It helps in cleansing ama (toxin) build up and balancing vata. Mix equal quantities of powdered fenugreek seeds, turmeric and ginger powder. Take one teaspoon daily in the morning and evening. For dry fomentation of the affected joints, take a cloth and fold it into four, fill it with hot and sand make a potli (bag) out of it. Use this bundle for the fomentation of different joints. This is very helpful in reducing pain and swelling.
