Modern Day Targeted Chemotherapy & Cancer Care!

Modern Day Targeted Chemotherapy & Cancer Care!

In the past, cancer was a disease that could not be cured. It was a threat to mankind. But as science has developed tremendously through all these years, today it is possible to fight cancer.
Cancer therapies have evolved with time. Until the late 90s, most of the drugs worked by killing the cancer cells. Some of the normal cells were also killed by them, but it was acceptable due to the huge effect on cancer cells.

In modern days, targeted therapies are being performed. Targeted therapy is a process of cancer treatment where instead of the cancer cells, the changes in the cells are targeted. Changes that help the cells grow, divide and thus spread cancer have been observed and so researchers can diagnose the right treatment that can block these affects and prevent cancer. This therapy uses drugs and it is different from traditional chemotherapy.

There are two types of Targeted therapy.

  • Small-molecule drugs: These drugs are small so that they can enter cells easily and target the changes inside the cells. They are taken as pills. They work by keeping the tissue around the tumor and starve it by stopping blood vessels that bring it nutrients.
  • Monoclonal antibodies: These are used to target the outer surface of cancer cells. They work like a plastic plug in an electric socket. They help chemotherapy and radiation therapy to cure cancer cells.

Fields:

Targeted therapies are used in the fields below
Breast cancer
Lung cancer
Colorectal cancer
Melanoma
Kidney cancers

How It Works:
Targeted therapy can be used as a treatment option for cancer in the following ways:

  1. Destroy cancer cells: Cancer cells hide well from the immune system. This therapy points them out so that the immune system can destroy them. It also boosts the immune system to work better.
  2. Stop cancer cells growth: This therapy interferes with the proteins that divides the cancer cells and thus controls their growth.
  3. Kill the tumor: Blood vessels feed the cancer tumor. This therapy surrounds the tumor and stops the blood supply so that the tumor does not grow and remains small. And if the tumor already has blood supply, then it kills the blood vessels and causes the tumor to shrink.
  4. Deliver cell-killing substances: It delivers the cell-killing substances to the cancer cells. They are the monoclonal antibodies that are combined with chemotherapy drugs, toxins and radiation.
  5. Cause cancer cell death: Cancer cells avoid the dying process. These therapies cause them to go through the process.

Side Effects:
Targeted therapies are less toxic than standard chemotherapy. So it is a better option but it still has some side effects.

  1. Diarrhea
  2. Liver problems
  3. Skin problems
  4. Problems with wound healing
  5. Difficulty with blood clotting
  6. High blood pressure
  7. Gastrointestinal perforation

Even though it has some side effects, it is still way better and much more effective than traditional chemotherapy. So, cancer patients are advised to go through this modern-day therapy.

Is der chances of cancer to come back once treated or it is gone forever. If yes any precautions which could be taken.

MS ( General Surgery)

yes cancer can recur, so regular check up is very important by your treating doctor. Few investigations will be done if your doctor suspect recurrence of cancer to treat immediately.

I am 75 years old mail suffering from secondary metastatic lung lesions and CA 19.9 is 740. I had 12 doses of chemo but the problem is not completely cured. Since it is told that frozen lemon helps in controlling cancer I want to know how much frozen lemon shall I take for better result per day and also whether I can take it in water because vegetables etc. Can not accommodate much. Shall be grateful for your advise.

DNB, MBBS
Oncologist, Faridabad
You have carcinoma pancreas with lung metastases. Vegetables and frozen lemon have protective role but it can not be considered as therapeutic. For treatment chemotherapy is the only option but ofcourse other supportive food do help in enhancing immunity and increase tolerance to treatments.

Prostatitis And Its Causes!

Prostatitis And Its Causes!

Prostatitis is a very common infection of the prostate. However, it is worth to note that prostatitis can also be an inflammation of the prostate without infection. Only 5 to 10 percent of prostatitis is caused by bacterial infection. Prostate cancer does not normally have its chances increased by prostatitis. There are several forms of prostatitis, including acute bacterial prostatitis, chronic bacterial prostatitis and chronic nonbacterial prostatitis (which is also known as chronic pelvic pain syndrome ).

The exact cause for prostatitis is not known, but here are some factors which increase its risk, particularly acute bacterial prostatitis.

Possible causes:

  1. Medical instrumentation: Putting an instrument like a urinary catheter may well cause prostatitis.
  2. Rectal intercourse: This is basically another name for anal sex.
  3. Abnormal urinary tract: The urinary tract comprises of the bladders, kidneys, ureters and urethra. If any one of these organs gets infected, then prostatitis is much more likely.
  4. Bladder infection: A bladder infection may well spread to the prostate.

Symptoms
Prostatitis has a variety of symptoms. Here are the most common symptoms of prostatitis.

  1. Constant need of urination: This is one of the most common symptoms of prostatitis.
  2. Difficulty when urinating: Just like the constant need of urination, difficulty urinating is also a sign that you may have prostatitis.
  3. Pain while urinating: This is because the prostate gland is a part of your urinary tract and if it does not work properly, there will be pain.
  4. Chills and fever: This is a rarer symptom, but may indicate prostatitis if it is coupled with the other symptoms.
  5. Pain in perineal area and genital organs: If you are experiencing pain in genital organs, than it may indicate prostatitis.
  6. Painful ejaculations or relief of perineal pain after ejaculations: If you are having painful ejaculation or the pain arises after ejaculation, then it may be due to prostatitis.
  7. Hematospermia: Pinkish or brownish semen.

If these symptoms are persistent or bothersome, then a proper consultation is required. Evaluation includes physical examination along with few simple tests. This can be followed by proper treatment for cure or relief of symptoms.

The normal size of a prostate is around 20 gms. The size of a prostate is not dependant on the size. That means even small prostates can be severely symptomatic. Surgery is the only method of treating prostates and exercise has no role in treatment. You must consult the doctor for full assesment.

I am a rectum cancer patient. I underwent surgery and removed colon and rectum. The biopsy report says my cancer stage is T3N1. My doc advised me to eat low fiber foods. I still have difficulty passing urine and motion. What all food I must include in my diet? What other treatment I have to undergo after this?

Hi lybrate-user. I appreciate you seeking help. Following is what you can include in a low fiber diet - (a) poultry - tender meat cuts, chicken (well cooked and soft), scrambled egg whites. (b) dairy - fresh yogurt, cottage cheese, custard, ice cream, soups etc all (c) breads & cereals - white breads, white rice, plain pasta or noodles (d) vegetables - fresh and well cooked tender vegetables without any seeds, skin, stems etc all. Vegetable soups and juices (without pulp) can be taken too. (e) fruits - small amount of soft cooked fruit without skin and seeds, clear fruit juice, well ripened banana in small amounts, (f) apart from the above-mentioned, he can have dark chocolates, mayonnaise, plain gravy, honey, jellies etc all too subject to his health condition foods that he needs to avoid are as follows - high fiber foods including peas, beans, lentils, legumes, nuts, processes meats, tough meats, brown rice, dried fruits, raisins, wheat germ, canola, coconut, bran, whole grains and whole wheat products, steamed vegetables, corn, cabbage, cauliflower, onions, potatoes with skin, berries, marmalades, pickles, potato chips, olives, horseradish, cucumber etc all are best avoided. The above-mentioned is subject to your consulting doc's concurrence too. He may have to drink lots of clear fluids unless advised otherwise by his doc. Your consulting medical oncologist can best advise on the next steps post surgery. However, an integrated therapy that extrapolates the best of conventional medicine and naturopathic/ alternative medicine to further treatment shall be a desirable approach to treat, going forward. You may connect with me should you require me to complement appropriately. Hope this helps. Take care and all the very best.
