What To Expect From Dilation & Curettage Process?

Dilation and curettage procedure which is commonly referred to as D&C is a minor surgical procedure where the cervix is dilated while a special instrument is used for scraping out the lining of the uterus. It is important to know what you can expect before, after and during the process so that you can stay ahead of unnecessary worries and help the process to be smooth and fruitful.

When do doctors recommend dilation and curettage process?
You may be required to undergo the dilation and curettage procedure for one of many reasons. It can be used for removal of tissues in the uterus during or after an abortion or miscarriage or to remove little pieces of placenta after delivery. This process aids in preventing infection as well as heavy bleeding. On the other hand, it can help in diagnosing and treating abnormal uterine bleeding including polyps, fibroids, hormonal imbalances and even uterine cancer. A sample of the tissues in the uterus is tested under a microscope to check if there is any abnormal cell present.

What can you expect during the dilation and curettage process?
The D&C procedure is a minor one and takes about 15 minutes even though you will have to spend about 4 to 5 hours in the healthcare facility. Before the procedure, your doctor would check complete history, and at this point, you should tell your doctor if you suspect that you are pregnant, you are sensitive to latex or any medicines or if you have a history of bleeding disorders. You will then be given anesthesia so that you don’t feel any pain or discomfort during the procedure. Before this procedure, you will have to empty your bladder.

The D&C procedure comprises two main steps, dilation, and curettage.
Dilation involves opening of the lower part of the uterus or the cervix for allowing insertion of a slender rod. This is done to soften the cervix so that it opens and allows curettage to be performed. Curettage involves scraping of the lining and removal of the uterine contents with the help of a spoon-like instrument known as a curette. This may cause some amount of cramping, and a tissue sample would be taken out for examination in the laboratory.

After the completion of the procedure, you may experience slight bleeding and cramping. In some rare cases, adhesions or scar tissues may start forming inside the uterus, and this condition is termed as Asherman’s syndrome which can cause changes in the menstrual cycle along with infertility. This problem, if arises, can be solved with the help of surgery and therefore, you should report any abnormality in your menstrual cycle to your doctor. The symptoms caused due to herniated disc can be very severe and can also cause a bit of disability. The disc of the spine is like a cushion and separates the set of bones on the backside. The discs are shock absorbers of the spine and are mainly composed of 2 parts, a soft jelly centre called the nucleus and a tough outer covering called the annulus.

Effects of Herniated Disk
A herniated or cracked disc is a severe condition and it seems to happen most commonly in the lower back.  It happens when a fraction of the soft centre gets pushed through the destabilized area due to degeneration, trauma or by putting pressure on the spinal cord. 

Nerves are located precisely at the back of every disc and are responsible for controlling everything in our body. While a disc gets herniated, the external covering of the disc tears and creates a bulge. The soft jelly gets shifted from the centre of the disk to the region where the damage has occurred on the disc. Most commonly, the bulge occurs in areas where the nerve is located and it causes strain on the affected nerve. It has been observed that individuals do not feel any painful sensations even if their disc gets damaged.

When is Surgery Recommended for Herniated Disc?
Surgery for herniated disc is recommended only after options like steroid injections, pain relievers, exercise and non-steroid anti-inflammatory drugs do not work. If the pain persists even after these options, then it becomes important to go for surgery. There are certain risks involved in this surgery like infection, bleeding or nerve damage. 

There are chances that the disc may get ruptured again if it is not removed. If you are a patient suffering from degenerative disc disease, then there are chances that problem occurs in other discs. It is very important that a patient maintains healthy weight to prevent any further complications.

The main factor that increases the risk of herniated disc is excess body weight, which causes a lot of stress on the lower back. A few people become heir to a tendency of developing this condition. Even individuals with physically demanding jobs are prone to this condition.

Ayurveda is a form of medicine that has been practiced for thousands of years and has many effective cure for some of the most complex diseases. Cancer, in this day and age, is one such complex disease and its incidence is increasing rapidly. It is always recommended to consult a Ayurveda specialist for severe cancer. Some of the common ingredients for treatment of uterine cancer through Ayurveda are mentioned here.

  1. Turmeric: Already a staple spice in many Indian dishes, turmeric as a herb in raw form and as a spice in powdered form, is touted the new wonder herb all across the world. It has been used as a traditional healing herb in India since ages. It has also been seen to be a very effective anti-inflammatory agent and thus it is very effective in stopping the growth of cancer cells.

Matra Basti: Matra Basti is a special treatment in traditional ayurveda for chronic constipation, low back pain, rheumatoid arthritis, obesity, and various neurological disorders like multiple sclerosis, epilepsy, dystrophy and atrophy of nerves and muscle. In this treatment, certain herbal oils and herbal extracts are applied through the rectum on a daily basis. Matra Vasti has a lubricating, balancing, nourishing, strengthening and pacifying effect. It also works as rejuvenator, immune-modulator and nutrient and subdues elevated vata dosha. The treatment lasts for a period of 3 to 7 days.
  2. Pichu Dharanam for Soothing at Advance Stages of Uterine Cancer: It is a special treatment modality in Ayurveda where sterile medicated cotton swab is kept inside the vagina for a specific period of time. This comes under local treatment modalities specific to gynaecological diseases in females and is also done in ante natal care. Pichu means “cotton ball”. Yoni pichu is the application of a sterile cotton swab soaked in medicated oil or ghee in the vagina.
  3. Abhyanham: Abhyanga is a form of Ayurvedic medicine that involves massage of the body with large amounts of warm oil. It’s a complete body massage and really effective. The Abhyangam massage therapy is very beneficial and prevents degeneration and ceases the aging process. This therapy needs more positions to massage and comparing to other massage therapy, Abhyangam is very special. It usually involves oils prepared with specialized Ayurvedic herbs and essential oils for lymph drainage, detox, and relaxation. It has seven positions and it treats the whole body. It assures new healthy physical changes. 
  4. Nasyam: Nasyam or errhine therapy is an Ayurvedic treatment excellent remedy for chronic sinusuitis, headache, throat diseases, epilepsy, catarrh, migraine, voice constraint, eye diseases and cervical spondylitis. In Ayurveda, the application of medical oils or powders through the nostrils is called Nasya. It is said in classical texts that the nostrils are the doorway to the brain. Nasya is an important method of treatment for illnesses of the cranial region. Nasya treatment - the forehead and neck regions of the patient are gently massaged with a suitable oil to induce sweating. Thereafter the patient is made to lie down on his back with his head bent slightly back. Warm oil is then dripped into both nostrils which the patient draws in.

  4. Minimally invasive neuroendoscopy: Similar to endonasal surgery but small incision is made.

Risks:

  1. Anaesthesia risks like breathing difficultyallergic reaction to medications, excessive bleeding or clots and infection.
  2. Risk related to the brain surgeries are seizurescoma, swelling of brain, infection to brain or meanings, surgical wound infection that intrudes to the brain structures, abnormal clot formation and bleeding.
  3. General risks include muscle weakness, disturbances in memory, speech, vision, coordination, balance and other functions that are controlled by the brain.

Breast cancer is a very common form of cancer that develops in the cells of your breast. The cancer typically forms in either the ducts of your breast (the pathway through which milk reaches the nipples from the glands) or the lobules (the glands which produce milk).

Breast cancer can affect both males and females. However, it is more commonly observed in women aged between 45-55 years.

Look out for these symptoms!
Signs and symptoms of breast cancer include the following-

  1. Discharge of blood from the nipples
  2. A lump near the breast or the armpit
  3. Scaling or peeling of skin around the breasts
  4. Sore nipple
  5. Breast pain
  6. Swelling of lymph nodes in the armpit or on the neck
  7. Change in the shape and size of the nipple or the breast
  8. An inverted nipple

What are the risk factors?

  1. Age- Breast cancer usually strikes at an older age. Women aged over 55 years are most often diagnosed with invasive breast cancer.
  2. Family history- If you have someone in your family who’ve had a history of breast cancer, then you too may develop this disease later in life.
  3. Inherited genes- Mutation of certain genes- BRCA1 and BRCA2- can put you at a higher risk of developing breast cancer.
  4. Obesity- People with Body Mass Index (BMI) higher than or equal to 30 are at risk of developing breast cancer.
  5. Alcohol consumption- Aggressive or frequent alcohol consumption can make you prone to breast cancer.
  6. Early menstruation- Those who’ve had their first menstrual cycle before the age of 12, are more likely to get affected by breast cancer in the later stage.
  7. Postmenopausal therapy- Women undergoing hormonal therapy or medications combining progesterone and estrogen to treat menopause have greater risk of having breast cancer.

What is the treatment for breast cancer?
After a thorough evaluation of your overall health condition, your doctor will determine the options to treat your breast cancer depending on the type, size and stage of the cancer. The usual methods of curing breast cancer are-

  1. Lumpectomy- This is a minimally invasive surgical procedure whereby smaller tumors are removed from the lumps of your breast. Your doctor will only remove a portion of the cancerous lump, leaving the remaining portion of the breast intact. This allows for better symmetry of your breast. However, you may need to undergo chemotherapy or radiation therapy following a lumpectomy to make sure that all the cancer cells are destroyed.
  2. Mastectomy- This surgical procedure is performed to remove all of the breast tissues including the ducts, lobules, nipples and areola.
  3. Sentinel Node Biopsy- In order to determine whether or not the cancer has metastasized to other body parts including the lymph nodes, your doctor will first discuss with you the importance of removing the lymph nodes which are likely to be in the receipt of the lymph drainage from the tumor.
  4. Breast removal- In certain cases where people have a greater risk of developing breast cancer due to family history or a genetic susceptibility, removal of both the breasts are prescribed by doctors.

The rate of survival for breast cancer has increased in recent times owing to modern treatment techniques. Nearly 90% of breast cancer patients survive for at least 5 years after the treatment is done.
