DNB ORTHO, MNAMS (Membership of the National Academy) (General Surgery) , MS ORTHO, MBBS
Orthopedist,
Ossteoarthritis is the most common form of arthritis, affects almost everyone. It occurs when the protective cartilage wear off. Although osteoarthritis can damage any joint in your body, the disorder most commonly affects joints in your hands, knees, hips and spine. There is no Cure for Osteoarthritis. But staying active, maintaining a healthy weight and other treatments may slow progression of the disease and help improve pain and joint function. Symptoms Osteoarthritis symptoms often develop slowly and worsen over time. Signs and symptoms of osteoarthritis include: 1) Pain. 2) Tenderness. Light pressure gives pain. 3) Stiffness. 4) Loss of flexibility 5) Grating sensation 6) Bone spurs. These extra bits of bone, which feel like hard lumps, may form around the affected joint. Causes Osteoarthritis occurs when the cartilage that cushions the ends of bones in your joints gradually deteriorates. Cartilage is a firm, slippery tissue that permits nearly frictionless joint motion. In osteoarthritis, the slick surface of the cartilage becomes rough. Eventually, if the cartilage wears down completely, you may be left with bone rubbing on bone. Risk factors Factors increase risk of osteoarthritis include: 1) Older age. The risk of osteoarthritis increases with age. 2) Sex. Women are more likely to develop osteoarthritis, though it isn't clear why. 3) Obesity. Carrying extra body weight contributes to osteoarthritis in several ways. It puts added stress on weight-bearing joints, such as your hips and knees. In addition, fat tissue produces proteins that may cause harmful inflammation in and around your joints. 4) Joint injuries. Injuries, such as those that occur when playing sports or from an accident, may increase the risk of osteoarthritis. 5) Certain occupations. If your job includes tasks that place repetitive stress on a particular joint, that joint may eventually develop osteoarthritis. 6) Genetics. Some people inherit a tendency to develop osteoarthritis. 7) Bone deformities. Some people are born with malformed joints or defective cartilage, which can increase the risk of osteoarthritis. 8) Other diseases. Having diabetes or other rheumatic diseases such as gout and rheumatoid arthritis can increase your risk of osteoarthritis. Complications Osteoarthritis is a degenerative disease that worsens over time. Joint pain and stiffness may become severe enough to make daily tasks difficult. Some people are no longer able to work. When joint pain is this severe, doctors may suggest joint replacement surgery. Tests and diagnosis Clinical Examination, X Rays and Blood tests. Treatments and drugs 1) Medications A. Acetaminophen. B. Nonsteroidal anti-inflammatory drugs (NSAIDs). 2) Therapy A. Physical therapy. B. Occupational therapy. C. Braces or shoe inserts. D chronic pain class – Training in group 3) Surgical and other procedures A. Cortisone shots. B. Lubrication injections. C. Realigning bones. D. Joint replacement. 4) Lifestyle and home remedies A. Exercise. B. Lose weight. C. Use heat and cold to manage pain. D. Apply over-the-counter pain creams. E. Use assistive devices. 5) Alternative medicine (I do not believe but people use) A. Acupuncture. B. Glucosamine and chondroitin. C. Avocado-soybean unsaponifiables. D. Tai chi and yoga.

Which brand of calcium is best for the healing of bone patient is suffering from femur fracture.

Which brand of calcium is best for the healing of bone patient is suffering from femur fracture.
Dear lybrate-user, Calcium taken externally has no role in bone healing unless you have a very low calcium level and are osteoporotic, So if you have a normal calcium level which it seem you will have looking at your age, do not be fooled by any calcium brand, the time the fracture will take to unit is not going to be affected by it.

I am a 20 years old male and have backache for last 9 months. What should I do? please sir help me.

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS

I am a 20 years old male and have backache for last 9 months. What should I do? please sir help me.
Hi, thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. Also 9 months is a long time to be having continuous pain, so it is advisable for you to get few investigations done and follow up after results. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc and with no other associated constitutional symptoms like evening rise in temperature, loss of weight, excessive sweating at night etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

Hi sir!
My mother is suffering from knee pain from 5 years
When she take ayurvedic medicine then she is fine for some...
From her symptoms it seems like she has developed osteoarthritis (oa) of knee. A heating pad or cold pack can feel really good on achy joints. Use whichever one feels better unless you have an injury, in which case cold is best for the first couple of days. You can apply cold or heat several times a day. Just remember to cover the cold pack with a towel and keep your heating pad at a low setting to avoid burning your skin. Whether you use heat or cold, remove it after 20 to 30 minutes. Physical activity is essential for people with osteoarthritis, whether it means walking around your apartment. Activities that cause pain should be avoided temporarily. -if the knee is swollen or sore, the following positions and activities you should avoid until knee pain and swelling resolve: squatting kneeling twisting and pivoting try to take the dietary supplements glucosamine for knee pain. Try using a cane to help her walk can take some of the weight off a painful hip or knee. It might even slow the damage from knee osteoarthritis. Please see a physiotherapist for detail assessment and management of knee pain. Best wishes.

Here most of every one commented on back pain. I know it is commonest problem. And it needs to be addressed.

Exercise and advice what all physios have posted are excellent.

List out as below:-
Muscle strength
aerobic conditioning
Stretching muscle
Posture
Weight loss to change angle of the spine
Dynamic and static stretching
Important of all neural stretch to maintain neural flexibility as most of the sedentary people develop stiff muscles and that leads to decrease neural mobility with due to pinching or getting compressed within the muscles or entry and exit between the muscles. Main nerves for neural stretch is sciatic nerve.
that is the commonest affected. Please do consider in your practices as it will provide a very good outcome for back pain.

and this has to be learned and practice under physiotherapist suprvision

thanks
