I am suffering from mild PCOD. Today was 1st session of my treatment. I'm consulting homeopathic for this. Is it ok or should I consult an endocrinologist as per my family doc suggestion. What should I do, I'm highly tensed.

MD - Obstetrtics & Gynaecology
Gynaecologist, Mumbai
There is no cure in homeopathy for pcod. Either you have to consult gynecologist or endocrinologist. If you r not married and only have irregular periods, you need to only reduce weight with metformin tablets and exercise and diet. Once the weight is controlled menses will start coming normally. You need to maintain your bmi below 25. Your current bmi is 26.6. So you should reduce your weight by 5 kgs.

I'm type 1 diabetic. Now I am pregnant for 3rd month, so tell me .Can any complication created with me?
Since you are a diabetic, you have to have a strict control of your sugar status. You must be on insulin. Visit your gynaecologist frequently and you drink plenty of water, no food from outside, only home made food, hygiene level should be very high. Get your eyes checked up for retinopathy and blood for KFT. Go for NB, NT scan during 11 to 13 week ofthe pregnancy along with Dual test. Your baby's heart scan should also be done. Basically follow your Dr. advise and get all the tests what everywhere advises you to. Good amount of salad and vegetables.

Fertility Problems

Hello,

I am Dr Tripti Raheja, I am working as senior consultant gynaecology in Max Super Speciality Hospital in Shalimar Bagh. Today, we are going to discuss fertility Problems and their treatment. Inability to conceive or reduce fertility is a very common problem it affects 15% of the couples. This reduce fertility can be due to male factors, female factors or unknown factors. In man we have reduced fertility because of poor quality of sperm or poor sperm count in their semen, women may have reduced fertility because ovaries do not produce eggs regularly or there can be some blockage in the fallopian tube so that the sperms cannot reach eggs in around 25% of couple the reason for reduced fertility remains unknown this is known as unexplained infertility. There are certain lifestyle changes which are likely to improve fertility, these are- stop smoking, reduce alcohol intake to less than 1 to 2 units, not more than once or twice a week, reduce intake of tea coffee or soft drink, do regular exercises, then maintain a healthy body with BMI should be between 20 to 25. It is a well-known factor if you are underweight or if you are overweight you are likely to suffer from infertility related problems. Men are advised not to wear a tight undergarment because they increase the temperature in the scrotum around the testis, so it deteriorates the sperm quality. If a couple is trying to conceive for more than 1 year and if they are unable to conceive so they are offered the certain test. So in men, we use really advice semen analysis to know the quality of sperms and to know the sperm count. If the first semen analysis report come out to be abnormal then again you will be advised to have it again after the gap of 3 months or maybe earlier as advised by your doctor if the repeat test also comes out to be abnormal then you will be advise, some blood test like hormone test and ultrasound of the squadron. So if you have low gonadotropic hormones levels in your blood, you will be given gonadotropic injections to improve sperm count. If you have some blockage in the flow of sperm from the testicles then a surgery may be beneficial for you to correct that blockage. Otherwise, if you have male factor infertility and if you are unable to conceive for more than 2 years it is advisable to go for In Vitro Fertilization or you normally called a test tube baby.

A woman may have reduced fertility because of reduced production of or irregular production of egg in their ovary. So they are advised to go for the test to know the ovulation and this test can be in the form of a blood test to check hormone levels or ultrasonography, this test also helps us to know how well your ovary will respond to fertility drugs. There is 1 more test which is usually, advised to women that is tubal patency test to check whether your fallopian tubes are blocked or then missed and that this can be done by x-ray or ultrasound even the sometimes an operation is necessary, operation laparoscopy is necessary to check the pregnancy of your fallopian tube. So, if there is some problem in the production of eggs by ovaries, you will be given some treatment in the form of tablets usually to stimulate the formation of the egg from the ovary in few women however gonadotropic injection may lead to be given to stimulate the formation of eggs in the ovary. If you have a blockage in fallopian tubes and if the blockage is my mild, then you will be advised to undergo surgery to correct the blockage. But if the blockage is serious, surgery is usually not helpful and in such circumstances, you are advised to go for In Vitro Fertilization. In cases of unexplained fertility, the cause is unknown and the couple is unable to conceive for more than 2 years they are advised usually, to go for In Vitro Fertilization. So in end, I would like to say, that the fertility-related problems, reduce fertility is a very common problem and in most of the cases are easily treatable. So do not lose hope, consult a qualified gynaecologist and after having the proper treatment you should be able to conceive.

If you want to consult me further you can text me via Lybrate you can have a text consultation or you can come to visit me at my clinic in Vijay Nagar, North Delhi. Thank you.

Vitamin D: The Easily Available Sunshine Vitamin

MAMC, MRCPCH, MD - Paediatrics, MBBS
Pediatrician, Noida
Vitamin D: The Easily Available Sunshine Vitamin

Vitamin D is a group of fat soluble nutrient that is an incredibly vital necessity for your body’s proper growth as it helps in the absorption of calcium, magnesium, zinc, phosphate and for multiple other biological reactions of our body.

Now the major issue about Vitamin D is that our dietary sources are inefficient to fulfil the Vitamin D requirements of our body since foods do not contain much of it. Sunlight is the only major natural source of Vitamin D for a human being. Our skin synthesizes the vitamin through complex organic reactions with the help of the UVB radiation of the sunlight.

How is Vitamin D deficiency harmful to your body?
The most important function of Vitamin D is to help the body absorb various other nutrients from our diet and incorporate it into our bones and other necessary parts. Thus, due to Vitamin D deficiency bones become weak, soft and brittle, and results in conditions such as Rickets and Osteomalacia. Recent researches have shown that Vitamin D deficiency is also linked to prostate, breast and colon cancer. Reducing the inorganic mineral content of our body Vitamin D deficiency takes a severe toll on the body from childhood.

How to combat the deficiency among Indians?
Vitamin D deficiency is now an epidemic in the South Asian countries, and India is no exception. About 65-70% Indians are vitamin D deficient despite the abundant sunlight they receive. It is essential to understand the contributing factors that lead to this situation to remedy the problem.

Ignorance
The first and primary issue is that Indians are reluctant to go out into the sun. From households to professionals, everyone prefers to stay away from the only major source of Vitamin D, and even when they go out, they cover up themselves. Lack of knowledge and fear of getting their skin tone darker seem to be some of the nastiest causes that keep the everyone under the shade. Along with these problems, use of sunscreens and completely vegetarian diets are also responsible for the epidemic.

Melanin composition
A majority of Indians have a darker skin tone, which means the melanin content of their skin is comparatively much higher. Melanin prevents the formation of Vitamin D from sunlight. As a result, the Indians need much-extended exposure to sunlight which they do not get.

Obesity
Obesity in Indians is another primary issue that causes the excess fat in the body to soak up the necessary Vitamin D of the body, causing its deficiency.

Learning from the stated causes we see that the only ways to prevent this disease are, much more exposure to sunlight, consuming Vitamin D rich diets and obviously fighting obesity. In cases where the deficiency is observed doctors would prescribe supplements to ensure proper functioning of the body. If you wish to discuss about any specific problem, you can consult a Pediatrician.

Anorectal Abscess - 10 Signs That Indicate It!

An anorectal abscess can be a combination of various conditions such as a rectal abscess, perianal abscess, anal abscess etc. It is a condition where pus and stool ooze out from areas of the rectum and the anus. Some of the possible causes of an anorectal abscess include infection in the anal area, sexually transmitted infections and blocked glands in the anal region. Disorders such as diverticulitis and Crohn’s disease can also lead to a deep rectal abscess.

What are the risk factors for this disease?
Some of the common risk factors of this condition include sexual activities involving the anal region, certain medications such as chemotherapy drugs, uncontrollable diabetes, a weak immune system due to conditions such as AIDs and HIV. An anorectal abscess can also be witnessed in toddlers if there exists a family history of this disease.

What are the common symptoms of this condition?
Some of the common symptoms of this condition include

  1. Frequent constipation
  2. Discharge of pus from the anal region
  3. Nausea and fatigue
  4. General malaise
  5. Formation of a nodule or lump in the rectum area,
  6. Night sweats
  7. Hardened tissue
  8. Redness and swelling of the lump near the anal area
  9. Frequent discharge of pus
  10. Painful stool, etc.

Diagnosis and complications:
A rectal exam conducted by the doctor can reveal the condition. In case of any doubt, a doctor often prescribes a proctosigmoidoscopy to rule out other conditions. The doctor might still prescribe tests such as MRI scan, CT scan, ultrasound etc. to get a detailed picture of the area where pus is getting collected near the rectal region. Complications of an anorectal abscess includes scars, anal fistula, constant pain, sepsis and recurrence of pus.

What are the treatment options available in Ayurveda?

  1. Curcumin: Curcumin is the pigment extracted from turmeric. This pigment is considered to be the most powerful naturally occurring anti-inflammatory agent. Its medicinal effect can cure many diseases. For patients suffering from an anorectal abscess, curcumin can control pain and inflammation in the anal region. Furthermore, it ensures that the condition does not aggravate to the next stage.
  2. Triphala: Triphala is a combination of three herbs- harara, bahera and amla. These three herbs should be consumed in a powdery form twice a day to clean the wound and balance the doshas. Triphala can speed up the process of recovery for patients suffering from an anorectal abscess.
  3. Guggul: Guggul is a gum resin that is extracted from the tree known as the Commiphora Mukul. It is used to treat a wide range of medical conditions. Although it seldom cures the condition alone, it cures many ailments when used in union with Triphala and other herbs.
  4. Neem: Neem leaves have many medicinal properties. It is anti-inflammatory in nature and its application in the powdery/paste form can pay rich dividends in curing an anorectal abscess. Furthermore, neem leaves when boiled and applied near the rectal region helps to ease the muscles and remove the blocked pus in the rectum.
