Hello, I gave birth via c sec ; days back.I get breast milk but irrespective of how much time i give feed he is not satisfied and top feed needs to be given. Please help me?

Pediatrician, Delhi
One should give exclusive breast feed to baby till 6mth of age. Milk production is a reflex process, more suking at breast will produce more milk. If u give top feed, baby will stop sucking at breast and milk production will decrease. So avoid top milk and give more of breast milk.

It is not mentioned since howlong baby is crying at night, feeding habbit and day time activities. At6 months baby require more than breast feed. This time for adding semi solids/ solids in his diet. If there is gap of more than 2-3 hours of feeding before 9 pm, give him suitable diet before 9 and last diet about 3 hours thereafter. If it doesn't work, Pls consult pediatrician.

I am 26 year old and mother of 3. I am still breastfeeding my 1 year 8 months old baby and want to stop now. My breast became saggy, soft and sometimes wrinkled. I want to increase my breast size naturally by homeopathy and diet and also want to lift up my sagging breast. I am very depressed looking at it.

M.Ch - Plastic Surgery
Cosmetic/Plastic Surgeon, Indore
U need surgical correction for loose breast. To increase your breast size: no medicine can increase breast size. No peels or massage can ever alter size. Breast implant is most effective and common procedure to increase size of breast. In this procedure silicon implant of desisted size is pushed under breast tissue. Procedure is done under anesthesia so there is no question of pain. Patient can go home in 3 to 4 hrs. You just need to wear supporting bra for some time and can return to work next day. Externally the look and feel of breast is just like natural young breast. Within few hrs you will regain your confidence and have attractive chest figure. In future pregnancy there is no problem in breast feeding. There is no risk of any major illness or disease associated with this procedure. Implants available in different size.

My son is 6 year old, a continuously bed wetting (one or two) by day one. What is the reason, how it goes away.

MBBS, Diploma in Child Health (DCH), DNB (Pediatrics), MNAMS (Membership of the National Academy), MRCPCH, Certificate in GI Endoscopy
Pediatrician, Delhi
Most of the times bed wetting is inorganic. The approach in this case will be to look for organic causes which are those that needs treatment with medication or surgery. These are usually not needed. If the condition is inorganic, behaviour therapy could be instituted.

My 5 months old son passing yellow urine. He is totally formula fed. Should I worry?

M.Sc - Food & Nutrition, PG Diploma In Diabetes Education
Dietitian/Nutritionist, Gurgaon
When a child is on breast feed , initially secreted milk is totally watery that fulfill his thirst and after sometime thick dark milk secretes that completes his nutritional needs. But in formula milk it doesn't happen . So ,if child is passing yellow urine without any sign of infection, offer him sips of water after every 3 hours. While offering water, take care of hygiene.

How to Surgically Treat Cleft Palate

MS - General Surgery, MBBS
General Surgeon, Guwahati
How to Surgically Treat Cleft Palate

Cleft palate or palatoschisis is a common genetic abnormality that leads to a horde of problems and is presently a growing challenge to medicine practitioners. The major developmental stages affected due to this particular irregularity include feeding, speech development, dentition and maxillofacial growth which are rather important to the normal overall developmental pace of an individual. Even though the cleft palate deformity was defined centuries ago, no fixed management algorithm exists for patients suffering from the condition in the present day scenario.

Cleft palate may be successfully fixed using reconstructive surgery. Multiple specialists are involved in the reconstruction surgery including plastic surgeons, otolaryngologists, nutritionists, oromaxillofacial surgeons and speech pathologists. Some hospitals also consider psychological therapy for the patient and the family to help get through the emotional trauma and the issues faced due to developmental backlogs.

The treatment for cleft palate usually begins around 9 to 12 months of age. If left untreated, it may cause major deformities. It takes about some years before the whole procedure is completed although it depends on the type and severity of the deformity.

The process involves the administration of anaesthesia after which the palate repair closes the inner, middle and final layers and at the same time realigning of the palatal muscles in a technique called anintravelarveloplasty is conducted. This ensures that the muscles are adjusted in a normal position which facilitates the best functioning of the palate during feeding, swallowing and speaking. It is possible that the child might require more than one surgery to completely close the palate.

Some of the risks involved during the process include:
1. Abnormal reactions to the medications
2. Bleeding
3. Problems in breathing
4. Need for more surgery
5. Infection

Although complicated and time consuming, cleft palate must be given immediate attention to avoid serious developmental issues. The reconstruction surgery and therapy combined ensures a normal development for the child in the longer run, given the surgery was done at the correct time. The child would be required to remain at the hospital for about 5-7 days. Complete recovery takes a time period of 4 weeks. Keeping the wound of the surgery clean is of the utmost importance and it should not be strained. If you wish to discuss about any specific problem, you can consult a specilized pediatrics and ask a free question.

I want to know about krill oil. Can I give my daughter krill oil as she is adhd and how can it help.

C.S.C, D.C.H, M.B.B.S
General Physician,
Krill oil is oil from a tiny, shrimp-like animal. Baleen whales, mantas, and whale sharks eat primarily krill. In norwegian, the word" krill" means" whale food" people extract the oil from krill, place it in capsules, and use it for medicine. Some brand name krill oil products indicate that they use antarctic krill. This usually refers to the species of krill called euphausia superba. Krill oil is used for heart disease, high levels of certain blood fats (triglycerides), high cholesterol, high blood pressure, stroke, cancer, osteoarthritis, depression, premenstrual syndrome (pms), and painful menstrual periods. How does it work? krill oil contains fatty acids similar to fish oil. These fats are thought to be beneficial fats that decrease swelling, lower cholesterol, and make blood platelets less sticky. When blood platelets are less sticky they are less likely to form clots. High cholesterol. Developing research shows that taking 1-1. 5 grams of a specific krill oil product (neptune krill oil, neptune technologies & bioresources, inc) daily reduces total cholesterol and? bad? low-density lipoprotein (ldl) cholesterol, and increases? good? high-density lipoprotein (hdl) cholesterol in patients with high cholesterol. Higher doses of 2-3 grams daily also appear to significantly reduce levels of triglyceride, another type of blood fat. Osteoarthritis. Early research shows that taking 300 mg of a specific krill oil product daily reduces pain and stiffness in people with osteoarthritis. Premenstrual syndrome (pms). Early research shows that taking a specific krill oil product might reduce pms symptoms. Rheumatoid arthritis. Early research shows that taking 300 mg of a specific krill oil product daily reduces pain and stiffness in people with rheumatoid arthritis. High blood pressure. Stroke. Cancer. Depression. Other conditions. More evidence is needed to rate the effectiveness of krill oil for these uses. Krill oil is possibly safe for most adults when used appropriately for a short amount of time (up to three months). Research on krill oil has not adequately evaluated its safety or possible side effects. However, it is likely that krill oil can cause some side effects similar to fish oil such as bad breath, heartburn, fishy taste, upset stomach, nausea, and loose stools. Special precautions & warnings: pregnancy and breast-feeding: not enough is known about the use of krill oil during pregnancy and breast-feeding. Stay on the safe side and avoid use. Seafood allergy: some people who are allergic to seafood might also be allergic to krill oil supplements. There is no reliable information showing how likely people with seafood allergy are to have an allergic reaction to krill oil; however, until more is known, avoid using krill oil or use it cautiously if you have a seafood allergy. Surgery: because krill oil can slow blood clotting, there is concern that it might increase the risk of bleeding during and after surgery. Stop using krill oil at least 2 weeks before a scheduled surgery. The specific utility in adhd is not certain. Ask privately

Myth - Small breast can't produce sufficient milk

MS - Obstetrics and Gynaecology, MBBS
Gynaecologist, Agra
Breast size has nothing to do with the quantity of milk produced. Large breasts just have more fat tissue, which is unrelated to milk production.
