Hello, premature ejaculation occurs when a man experiences orgasm and expels semen soon after sexual activity and with minimal penile stimulation. Men with premature ejaculation often report emotional and relationship distress. Medication: you can take homoeopathic medicine - schwabe's damiaplant/ thrice daily for 4 weeks. Management: regular exercising keeps you fit and healthy, so don't forget to do it. Practice pranayam regularly for half an hour preferably in a garden early in the morning. Not to do these exercises within 4 hours after meals. One can have meals not within 15 minutes after finishing these exercises. These exercises are good for gaining and maintaining good health. Also, practice pelvic floor exercise or kegel exercise, consists of repeatedly contracting and relaxing the muscles that form part of the pelvic floor. It makes the muscles of pelvis toned and strengthened! in order to locate your pelvic muscles try to stop and begin the flow of urine as using the bathroom. You can perform kegel exercises with the empty bladder as exercising pelvic muscle with the full bladder might result in urinary tract infections.

Night fall is common in adolescent male due to involuntary ejaculation during sleep in night / early morning. Night fall in moderation relieves sexual tension. Occasional night falls are nothing to feel guilty about, that's a normal function of body. Excessive fall can cause weakness, fatigue, sexual problem like erectile dysfunction, premature ejaculation, testicular pain, decrease sperm count. Precaution 1. Avoid stress & anxiety. 2. Do regular exercise for min. 30 minutes 5 days a week. 3. Practice yoga, meditation, deep breathing exercise to calm your mind, control emotion and relieve stress. 10 minute deep breathing once or twice a day is helpful. 4. Take good nourishing diet, plenty of green leafy vegetables, fruits (onion, garlic, pomegranate help). 5. Avoid spicy, oily, fatty food and junk food. 6. Avoid consumption of liquid 2 hours before going to sleep. 7. Don't take milk, tea/coffee before going to bed. 8. Go for urination just before going t o bed. 9. Avoid watching pornographic / erotic pictures / movies before going to sleep. 10. Fill your time with enriching, fulfilling activities and avoid dwelling on thoughts of sex. 11. Listen to soothing music which gives peace of mind, read good books before going to sleep. 12. Masturbation once in a week or 2 week will help control night fall. 13. Avoid constipation because upset digestive system can cause night fall. 14. Sleep on your back. 15. Wear loose fitting clothes. 16. Avoid spicy foods and stimulants (coffee and other substances that contain caffeine, especially late at night). 17. Develop a healthy relationship with sex. If you're sexually active, communicate with your partner to maintain a healthy and open sexual relationship that keeps you both fulfilled. 18. Make sure you get enough sleep and exercise, and take time off from your busy schedule to relax and do things that you enjoy. Pick up new hobbies to fill your free time with fun activities and relax when you get the chance to relax. There are good herbal medicines for controlling nightfall. You have eusthesian tube dysfunction. A 10 days course of antihistaminic will help you. Additional nasal drop will give faster recovery. Anyway, visit an ent surgeon for clinical evaluation. Thanks.

Avoid constipation. No fried/ junk/ spicy food no alcohol no smoking walk 30 mts daily do deep breathing exercise for 10 mts take more water for medicine contact on private consultation good luck.

With sedentary lifestyle, refined and processed food habits, obesity and diabetes is the new age epidemic. India, in fact, is being termed as the diabetes capital of the world given the huge rise in the number of cases over the last couple of decades.

There are well established risk factors for diabetes and if these can be managed, then the chance of delaying onset, controlling progress and containing complications are highly possible. Read on to know how simple things can be effective in preventing and managing diabetes.

  1. Weight control: Obesity is the most important risk factor for developing diabetes. A person with normal BMI is 20 to 40 times less likely to develop diabetes than an overweight person. Losing about 10% of the excess weight can help prevent diabetes by more than 50%. Check with your doctor what should be the ideal recommended weight for you and draw up a routine (diet, exercise, etc.) that will help you achieve and stay around that weight range.

  2. Exercise: All methods of exercise help in moving the muscles, which is drastically reduced given our sedentary lifestyle. Moving the muscle (as much and as often as possible) ensures they absorb more glucose and reduce the stress on insulin production. Something as simple as brisk walking for 30 minutes is good enough to reduce the chances of developing diabetes by 50%.

  3. Don’t be a couch potato: If watching television is your favourite pastime, it is time to change it. This puts people at risk of developing obesity, heart disease and diabetes. Also, watching television is usually associated with overeating, further adding to the risk of diabetes.

  4. Fibrous foods: Eating fibrous fruits and vegetables as compared to refined and fried foods helps prevent diabetes.

  5. Whole grains vs processed foods: When you have to choose between a pizza and a bowl of brown rice, go for the latter.

  6. Good fats vs bad fats: Polyunsaturated fatty acids are found in vegetable oils, nuts, and seeds and help control diabetes. Trans fats present in margarine and baked foods are best avoided.

  7. Non-vegetarian foods: If you have a choice, pick fish and poultry versus red meat (processed or otherwise). The fish is rich in Omega-3 fatty acids and helps control inflammation.

  8. Leave the whites: Get off white rice, white refined sugar and white flours. Include brown rice, brown or demerara sugar and wheat flour instead.

  9. Smoking: Quit smoking and you will note how beneficial it is in preventing diabetes.

  10. Alcohol: Moderate amount helps in effective functioning of the insulin.

You have borderline high bp, reduce salt but I am not suggesting a salt free food. Avoid tinned foods as salt is a preservative. Avoid other packed foods also like chips, namkeem, street foods. Use minimum oils for cooking. Eat lots of seasonal vegetables and fruits. Reduce stress by exercise and yoga and meditation. Take a tablet of safe betablocker like metoprolol 25 mg twice daily. Get back here with progress reports only after observations, after 2 weeks. Get back here with reports. You will get more guidance from any of us here on the forum.
