I am not able to sleep since 1 week. I always try to sleep on time but I am helpless. I am not getting proper sleep. In a week I slept hardly 3 4 hours only. I have tried each n every possible thing. I am not taking any stress nor any pressure. I have started to drink milk at night also to get better sleep. But all this is in vain. Please suggest me I am fearing that it may be features of insomnia.

It is definitetly insomnia, a significant percentage of population suffer from insomnia. The reasons need to be evaluated before you are treated. Consult a psychiatrist. There are non-dependent medications to treat insomnia as well as sleep hygiene, which will be decided after the evaluation.

Can the Immune System Trigger Alzheimer's Disease

Alzheimer's Disease is a neurological problem that is characterised by a cognitive decline and memory loss. It is a type of neurodegenerative dementia. The symptoms of Alzheimer's Disease include not being able to absorb and retain new information, lack of reasoning and judging, not being able to take on complex tasks, impaired visuospatial abilities, problems in reading, writing and speech, among many others. If a person has at least two of these symptoms in a debilitating manner, then the diagnosis can be made in favour of Alzheimer's Disease. The main causes of Alzheimer's are shrinkage of the brain size and death of the brain cells. The immune system is also said to trigger this neurodegenerative disease.

Let us find out what medical science has found so far:

  1. Connections: Many a times, in Alzheimer's disease, the memory and behaviour of the person changes because the brain is unable to make proper neural connections which can lead to memory loss of how a person behaved and the elements that formed the basis of the patient's cognition. Apparently, the immune system behaves in the same way within the brain and blocks the connection. This happens because there is constant communication between the brain and immune system along neurological lines, which is where the disease first emanates.
  2. Inflammation: The brain is prone to inflammation or swelling that is not the normal kind. This inflammation happens as a result of the activation of the infection fighting neurotransmitters and the chemical changes that happen in the brain when an infection strikes. The inflammation usually happens in the plaques or clumps which the brain tries to protect. These clumps are made up of a protein called Amyloid. The immune system is responsible for creating this inflammation in the brain of the patient.
  3. Pattern Recognition Receptors: Many of these receptors work in different manners and cooperate with each other to create a response in the brain. These PRRs can be found in the brain plaques, and they develop the signs of danger which further fuels the inflammation in the brain as a matter of protection.
  4. Activation of Cells Linked with the Immune System: When the PRRs begin to respond, it basically activates the immune system and the cells of the same. This is the basic reaction that causes the changes in brain which then leads to the attachment of the protein to the tissue that is diseased, in which case Alzheimer's Disease starts. The inflammation that we had spoken about earlier basically happens in the nervous tissue.

It is important to recognise and act on the initial signs of Alzheimer's Disease before it progresses beyond one's control.

Sir, my father (about 88 yrs old) suffered a paralytic stroke 25-26 days back. After that he has lost his confidence and partial memory loss is also there. Migraine - Identifying the Early Signs!

Headaches and migraines can vary drastically depending on their duration, specific symptoms and the person they are affecting. The more you know about your specific type of headache or migraine, the better prepared you will be to treat them—and possibly even prevent them. The two types of migraine are- 

  1. Migraine without aura: The majority of migraine sufferers have Migraine without Aura. 
  2. Migraine with aura: Migraine with Aura refers to a range of neurological disturbances that occur before the headache begins, usually lasting about 20-60 minutes.

Symptoms of migraine vary and also depend on the type of migraine. A migraine has four stages: prodrome, aura, headache and postdrome. But it is not necessary that all the migraine sufferers experience all the four stages.

Prodrome: The signs of this begin to appear a day or two days before the headache starts. The signs include depression, constipation, food cravings, irritability, uncontrollable yawning, neck stiffness and hyperactivity.
Migraine Aura: Auras are a range of symptoms of the central nervous system. These might occur much before or during the migraine, but most people get a migraine without an aura. Auras usually begin gradually and increase in intensity. They last for an hour or even longer and are 

  • Visual: Seeing bright spots, various shapes, experiencing vision loss, and flashes of light
  • Sensory: Present in the form of touch sensations like feeling of pins and needles in the arms and legs
  • Motor: Usually related with the movement problems like the limb weakness
  • Verbal: It is related with the speech problems

Headache: In case of a migraine attack one might experience:

  • Pain on both sides or one side of the head
  • Pain is throbbing in nature
  • Vomiting and nausea
  • Sensitivity to smells, sound and light
  • Vision is blurred
  • Fainting and lightheadedness

Postdrome: This is the final phase of the migraine. During this phase one might feel fatigued, though some people feel euphoric.

Red flags that the patient may be having underlying serious disorder not migraine

  1. Onset of headaches >50 years 
  2. Thunderclap headache - subarachnoid haemorrhage 
  3. Neurological symptoms or signs 
  4. Meningism 
  5. Immunosuppression or malignancy 
  6. Red eye and haloes around lights - acute angle closure glaucoma 
  7. Worsening symptoms 
  8. Symptoms of temporal arteritis

These patients require CT scan / MRI or CSF examination. Most Migraine patients do not need these tests. 

Diagnosis of Migraine: Usually migraines go undiagnosed and thus are untreated. In case you experience the symptoms regularly then talk to the doctor, who evaluates the symptoms and can start a treatment. You can also be referred to a neurologist who is trained to treat the migraines and other conditions. During the appointment the neurologist usually asks about the family history of headaches and migraines along with your symptoms and medical history.

The doctor might advise for some tests like:

  1. Blood Tests: These reveal problems with the blood vessel like an infection in the spinal cord and brain.
  2. CT scan: Used to diagnose the infections, tumors, brain damage, and bleeding that cause the migraines.
  3. MRI: This helps to diagnose the tumors bleeding infections, neurological conditions, and strokes.
  4. Lumbar Puncture: For analyzing infections and neurological damages. In lumbar puncture a thin needle is inserted between the two vertebrae to remove a sample of the cerebrospinal fluid for analysis.

Treatments

Migraine treatments can help stop symptoms and prevent future attacks.

Many medications have been designed to treat migraines. Some drugs often used to treat other conditions also may help relieve or prevent migraines. Medications used to combat migraines fall into two broad categories:

  • Pain-relieving medications. Also known as acute or abortive treatment, these types of drugs are taken during migraine attacks and are designed to stop symptoms.
  • Preventive medications. These types of drugs are taken regularly, often on a daily basis, to reduce the severity or frequency of migraines.

Your treatment strategy depends on the frequency and severity of your headaches, the degree of disability your headaches cause, and your other medical conditions.

Some medications aren't recommended if you're pregnant or breast-feeding. Some medications aren't given to children. Consult a doctor find the right medication for you.

Hi One of the simplest and most effective ways to induce sleep is to rub some oil on the scalp and the soles of the feet before going to bed. Use sesame oil, brahmi oil, jasmine oil, or coconut oil and massage gently for a few minutes. Slightly warming the oil before applying is helpful. 1-1 drop of cow ghee in both nostrils at night time before sleep. Take sarpgandha vati 1-1 tab twice a day. Take pranacharya no tens capsule 1-1 twice a day. Avoid spicy food. Junk food. Fermented food. Tea.coffee. Do pranayama early in the morning.

I feel very weak when I get up in the morning, my hands start shaking when hold something. What should I do to overcome this weakness. I drink enerzal daily and eat food on time as well. What should be done in this case? Awaiting for quick response!

You can feel weak due to lack of essential nutrients, not getting enough sleep, a weak immune system, excessive drinking, skipping meals, emotional stress, and too much physical labor. Persistent weakness can be caused by health problems like anemia, arthritis, hypothyroidism, a sleep disorder, cancer, chronic fatigue syndrome, diabetes and heart disease. In these cases, it is very important to find out the exact cause behind your weakness. To feel energetic and refreshed in life, you must exercise daily for at least 30 minutes, maintain your ideal body weight, follow a good diet, sleep properly, drink water throughout the day to keep your body hydrated, avoid alcohol and stop smoking if you do .

I am 57 yrs old and a mother of two. For the past seven to eight yrs I have been suffering from Insomnia. I hardly sleep for two hours now and it is telling on my health. Suicidal thoughts are indicative of severe depression, kindly consult a psychiatrist immediately .The word depressed is a common everyday word. People might say" i'm depressed" when in fact they mean" i'm fed up because i've had a row, or failed an exam, or lost my job" etc. These ups and downs of life are common and normal. Most people recover quite quickly. With true depression, you have a low mood and other symptoms each day for at least two weeks The common symptoms are (few might be present in a person with depression) Feelings of sadness, tearfulness, emptiness or hopelessness Angry outbursts, irritability or frustration, even over small matters Loss of interest or pleasure in most or all normal activities, such as sex, hobbies or sports Sleep disturbances, including insomnia or sleeping too much Tiredness and lack of energy, so even small tasks take extra effort Changes in appetite — often reduced appetite and weight loss, but increased cravings for food and weight gain in some people Anxiety, agitation or restlessness Slowed thinking, speaking or body movements Feelings of worthlessness or guilt, fixating on past failures or blaming yourself for things that aren't your responsibility Trouble thinking, concentrating, making decisions and remembering things Frequent or recurrent thoughts of death, suicidal thoughts, suicide attempts or suicide Unexplained physical problems, such as back pain or headaches General medical conditions and substances causing depression also needs to be ruled out. Causes of lack of sleep are: • stress • Anxiety • Depression • Pain • irregular schedule of sleep such as sleeping during day time & working during nights. • Caffeine, alcohol • Due to drugs • Due to high blood pressure. • Aging Due to lack of sleep there are many complications arise such as lack of concentration, irritability, fatigue & digestion problem. You can also do foot massage daily it destress you. • Do not take stress. I will suggest you to eat nutritious home made food with adequate fluid intake and few diet tips - not to eat fried food items and can eat all green vegetables, moong, chana, dal, rice etc and to apply ghee over chappati and don't apply oil over chappati and to drink milk mixed with turmeric powder twice a day and if possible eat eggs on alternate days and if possible in breakfast eat atleast one bajra ka rotla and eat seasonal fruit.

Hello Dr. Mere sir me left side drd rehta h 6 month pehle CT scan karwaya tha Khuch ni aaya. Dr. NE migrane btaya but Ab Khuch Dino se mere left eye me Kone me drd hota h sir drd k saath saath. If you have migraine you can take suminat tablet (the specific drug for it. Head ache can be of many reasons and the commonest is visual refractive error of eyes, Checking eyes and correcting errors can cure headaches, similarly the cause can be migraine and tension. Tension relieving methods like meditation, yoga and migraine medication can cure these. Check your eyes first of all and do a blood CBC & TSH and inform me personally. For temporary relief you can take a pain killer like Brufen tablet but need check to assess the real cause and treat.

I want to ask about my cousin. He' s 8 years old. 2 weeks ago he was feeling unconscious and lazy, we took him to sir gangaram hospital, delhi. Their doctors said he might be suffering from meningitis and later they discovered that it was a blood clot. A fluid in the brain was getting collected beacause of which swelling was their in the brain. And for that they did a operation making a hole and taking out some quantity of fluid, and bone from the back, lower part was also cut for some reason. When this child was born he suffered from jaundice because of which his whole blood was changed and because of that he suffered from a problem of listening so a machine was fitted in the brain so that he could hear and speak. And now because of a machine is fitted in the brain, doctors can not do his MRI. He is on ventilator from the first day. There is movement of his right hand, leg and eyes(but don' t know if he is able to see or not) but no left side movement. Doctors are saying his left side is paralyzed. Yesterday another minor operation was done in which now the ventilators air is being given through trachea. If you want I can give you the reports before the operation and after the operation if you want Please tell us what to do

First of all operation not supposed to be done for blood clot. Machine fitted on right side or left side of the brain? kindly send me the detailed reports. Kindly consult neuro physiotherapist and audiologist also. This will help you in this case.

Migraine is a kind of relapsing headache. Its symptoms include - episodic unilateral or bilateral, severe and pulsating headaches, associated at times with nausea and vomiting, relieved momentarily on rest or taking medications and aggravated by precipitating factors like sleep deprivation, loud noises, continued sun exposure, allergy to some agents etc. The treatment of migraine is mainly done for relief from acute headache attacks and later treatments to prevent future relapses. Also treatment of factors which tend to cause relapses is also important. Consult a headache specialist for treatmnet. For time being relief from headaches you may use tab. . 1. It could be due to essential tremor. The cause of the tremor is unknown. In some cases it is hereditary and runs in families. There is no definite treatment unless it is severe 2 other causes of tremor could be stress, anxiety, panic, nervousness, inadequate sleep, physical exhaustion, strong emotion, hyperthyroidism, hypoglycemia, smoking/tobacco/alcohol abuse/withdrawl, excess tea/coffee, drugs like steroid/amphetamine, neurological disorder, cerebellar dis order, brainstem disorde, r multiple sclerosis, stroke, head injury, mercury poisoning, liver failure, parkinsonism etc 3. Investigate yourself to exclude the above causes of tremor after consulting neurologist 4. Avoid stress/anxiety/nervousness/ panic etc 5. Avoid excess tea/coffee/ smoking/alcohol if you have 6. Take good nourishing diet 7. Have adequate sleep. 8. Do a thyroid function test to exclude thyroid problem 9. Consult neurologist for further advice.
