Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are actually quite young to have a prolonged back pain. Can you tell me what was the initiating factor for your pain? apart from pain, do you feel any other symptoms like weakness, numbness or paresthesias in legs as well? is there any localized swelling or any associated systemic symptoms like fever, night sweats, weight loss etc? if you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you, you can also add a muscle relaxant to relax muscle spasm - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

Hi, I am having ligament pain issue in my right leg ankle from past 6 months. Consulted Arpho and physiotherapy but did worked out. Please help

BPTh/BPT
Physiotherapist, Hyderabad
Ligament injuries usually take 8-14 months to be healed completely provided you don't disturb the process by excessively using that part of the joint. Use of ankle brace and ample rest should give temporary relief. Ultrasound therapy sittings 3 times in a week over a period of 3-4 months should help with healing of the ligament.

Back exercises. Many people go to physiotherapists with back pain. Here are two exercises that help to relieve pain and reduce stiffness in the back: •Dear, 1. Lie on your back with both of your legs straight. In this position, bring your left knee up close to your chest. Hold this position for 10 seconds. Return your leg to the straight position. Repeat with the right leg. •2. Stand up with your arms on your side. Bend to the left side while slowly sliding your left hand down your left leg. Come back up slowly and relax. Repeat with the right side of your body.

I am 23 years old male and recently encountered a fracture in my palm. The bone connected to my little finger got fractured in angular manner. Shall I go for a POP or a surgery where a wire will be inserted?

Fellowship in Neurological Rehabilitation, BPTh/BPT
Physiotherapist, Gurgaon
Hi, all dislocations or fracture dislocations should to be put back in joint (reduced) and held in joint until healed. Sometimes the injury will pop back into joint and be stable enough to hold in a plaster cast. If so they need careful supervision with repeat x-rays for 2-3 weeks from injury to make sure they do not slip out again. If the joint is not that stable it will need to be held with 1-3 wires passed through the skin known as k wires. These usually protrude through the skin so they can easily be removed in the outpatient's clinic. Some surgeons will bury the wires under the skin. Only occasionally is open surgery required i. E. A formal cut is made on the back of the hand over the injury. The wires typically remain in place for 4-5 weeks. During this time the fracture and wires will need support in a plaster cast either a full cast or a partial cast. Any fingers or parts of fingers not held by the plaster should be moved to reduce stiffness. Most patients do very well following these injuries. After an immobilization period, rehabilitative exercises should be performed to regain strength and mobility and to minimize chronic pain and stiffness. Thanks.

MPTh/MPT

Shoulder exercises are essential for you but you need to rule out first why the pain is occurring. Is there any pain in while you are pulling off your parse from your hip pocket? Is the pain is more in the initial range of movement? Is there any pain while sleeping? Can you sleep turning towards the affected side? Please let us know for better guidance.

Hi Thanks for your query and Welcome to Lybrate. I am Dr Akshay from Fortis Hospital, New Delhi. Actually pain at calf/Leg region can be due to many reasons , commonly it is muscular in nature but it can also be due to Tendons, Nerves, Blood vessels etc. My advise to you for initial 2-3 weeks will be : - Ice packs application intensively with elevation of your limbs at night - Take a short course of anti-inflammatory medication as per your body's suitability - Stop your physical activity till pain completely subsides - Intensive physical therapy, training and proper muscle conditioning once pain has subsided - Slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - We will get some blood tests for you - Serum Vitamin D and serum Potassium levels, ESR and CRP Once you have all these reports then you can contact me as i can advise you for proper supplementation Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation. Thanks & Regards Dr Akshay Kumar Saxena Consultant Orthopaedics Fortis Hospital, New Delhi

Sometimes, the veins of my right hand in the lower portion become swallowing and even I do not carry small things. I have consulted with orthopaedic doctors about 4 months back. But pain killer tablet did not relieve me. On the contrary I felt very uneasy immediately after consuming the dose. Therefore I stopped consuming tablets. Please advice.

U should go for an X-ray of your elbow as sometimes due to weak bones pathological fractures may occur, or it may be due to low calcium or vit d3 deficiency or due to sprain or due to remote effects of past injury or it may b due to orhervorthopaedic causes. In all the conditions Homoeopathic medicines are very beneficial if selected on the basis of symptom similarity.

May be she is having knee Osteoarthritis. Osteoarthritis can occur at any time of life but the incidence increases with age. It usually affects people over 40 years of age. There is no cure for osteoarthritis but the progression of the disease can be slowed and pain and disability reduced. Treatment options include: Healthy body weight: Maintaining a healthy body weight will reduce stress on the arthritic joints. Nutritional supplements Supplements such as glucosamine have been reported to relieve the pain of osteoarthritis and halt, or even reverse. So please try to take this supplement once a day. Exercise Exercise can assist with the maintenance of muscle strength and the mobility of arthritic joints. Activity at a level that does not cause pain is not thought to worsen osteoarthritis. Non weight-bearing exercises such as swimming or aqua jogging in a heated pool may be beneficial. Range-of-motion and stretching exercises will help maintain the maximum possible movement in the joint. Stretching will help maintain or restore flexibility to the joint’s muscles and tendons. You can apply cold or heat several times a day to reduce your knee pain. Just remember to cover the cold pack with a towel and keep your heating pad at a low setting to avoid burning your skin. Whether you use heat or cold, remove it after 20 to 30 minutes. If the knee is swollen or sore, the following positions and activities you should avoid until knee pain and swelling resolve: Squatting Kneeling Twisting and pivoting See a Physiotherapist for detail exercises programme and manangement.
