For ankylosing spondylitis the best suggestion to do general aerobic exercises which includes breathing and all the other exercises which involves the total movements of the proximal joints ie. Neck, shoulder, elbow in the upper limb and in the lower limbs, trunk, hip, knee movements which will facilitate to improve the normal range of motion of the joints thereby improving the blood circulation. You can also try swimming activity, tread mill, cycling etc.

I have pain in my legs because I have certain with milk what will we its treatment?

Kindly show me digital x rays of knee &leg. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Ibuprofen 200mg od & sos x 5days bio d3 max 1tab od x10 do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. Do reply back for detailed treatment plan. Do not ignore. It could be beginning of a serious problem.

For leg pain, please follow the below advise: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

You have not mentioned the age and weight of your mom which is very important for knee pain. I think she must have arthritis along with osteoporosis. You need to chek her bone density and consult a good orthopaedician in your town.

Hi Thanks for your query and welcome to Lybrate. I am Dr. Akshay from Fortis hospital, New Delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - To maintain proper posture of your back while working and sleeping - If pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - Physical therapy initially under supervision of a trained physiotherapist and then to continue at home - Adequate calcium & vitamin d intake if levels are low in body - Ice packs can applied if your pain is acute, then hot fomentation can be done at home - Analgesic spray for local application can be used and is easily available. - Avoid lifting heavyweight/ acute forward bending in mornings etc We will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Get x ray done. Wear knee cap open patella. Start with physiotherapy sessions and learn exercise for knee joint. Take calcium and methylcobalamine supplement with glucosamine for three months minimum.

Dear lybrate user, Most of the time the clicking or snapping is due to everyday use and changes that occur naturally within our joints.  As we age this causes some of the surfaces to roughen —thus when they rub against one another they will snap, click, or pop within the shoulder.    In the majority of circumstances snapping or clicking of the joints is not an issue to worry about.    Painless clicking and popping usually does not require any treatment at all.

It requires detailed investigation and examination. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5days, inform.

Apply hot fomentation twice daily. Avoid bending in front. Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang asana -- lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core strengthening exercises- straight leg raised with toes turned outward, repeat 10 times, twice a day.
