Reasons Behind Sciatica Pain!

Sciatica is a very common neuropathic pain that is caused by damage to the sciatic nerve, a large nerve that runs from the lower back region down the end of each leg. It is the largest single nerve in the body.
This condition affects around 4 out of 10 people at some point of their lives. The nature of the pain varies from a mild ache to a sharp stinging sensation and may cause feelings of numbness and extreme weakness.

Causes of Sciatica
The causes for the onset of Sciatica are numerous. Some of them are as follows:

  • Slipped lumbar disc
  • Muscle inflammation
  • Internal bleeding
  • Pregnancy
  • Obesity
  • Awkward sleeping position
  • Uncomfortable footwear
  • Spinal injury

How can physical therapy help?
There are many ways of treating Sciatica, such as medication, acupuncture and surgery, which can provide relief. The most common and easy treatment method however is physical therapy. A progressive and controlled exercise routine that is specifically tailored to the nature of the condition in consultation with a physical therapist acts as an effective remedy in treating the problem. It alleviates pain and also eliminates the chances of relapse.

Physical therapy exercises used to treat Sciatica are of many types. They are a combination of core strengthening, muscular stretching and aerobic conditioning routines, which aim at correcting the root of the problem.

The different types of exercises employed to treat Sciatica include the following:

  1. Strengthening exercises: These aim at strengthening the spinal column and the pelvic region, which includes the hips and buttocks. They help in making the muscles and ligaments in this region more robust and allow for improved flexibility and support.Lunges and squats are some of the examples.
  2. Stretching exercises: These provide pain relief through improved agility and flexibility. The most important aspect of these routines is stretching and exercising the hamstring. Hamstring, stretches, calf stretches and tricep stretches are a part of stretching exercises.
  3. Aerobic conditioning: These are cardiovascular exercises of low intensity, which provide pain relief through the release of endorphins and the facilitation of fluid and nutrient exchange. Examples of these are swimming and brisk walking.

Investigations advised - hb, tlc, dlc, esr, sugar f & pp, uric acid, vit d3 urine r/e x ray of the affected part. Anyway you may try- sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomentation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. If no relief in 4_5 days, then inform again.

When the density of the bone decreases, it is known as osteoporosis. The bones also start to get weak and fragile and their strength decreases. It causes the bones to become porous, more like a sponge. This can also lead to frequent fractures in the bone. Here are some of the factors that might increase your risk of getting osteoporosis:

  • Thin and small body frame
  • Family history of osteoporosis
  • Smoking
  • Excessive alcohol consumption
  • Lack of exercise
  • Calcium deficiency
  • Poor nutrition
  • Lack of certain vitamins
  • Bowel diseases
  • Low estrogen in women
  • Low testosterone in men
  • Chemotherapy because of the toxic effect it has on the ovaries
  • Menstrual cycle complications in women
  • Chronic swelling due to rheumatoid arthritis or any liver disease.

Osteoporosis can be treated in a number of ways. The main goal is to prevent the bones from getting fractured or becoming weaker. Timely treatment can surely decrease the risk of fractures in the future. It is very difficult to completely cure or rebuild the bones however one can easily prevent it from happening further. Following are some of the treatments that can help in preventing osteoporosis and make a person fit to a certain extent:

  1. Changes in the lifestyle of a person are very important. These include, not smoking and refraining from alcohol consumption.it is very important for a person with osteoporosis to exercise regularly and take a healthy and balanced diet.
  2. A good amount of Vitamin D and calcium is required in the body. These can be consumed straight from certain foods or in the form of supplements.
  3. Medicines like alendronate, calcitonin, ibandronate and relaxofine can turn out to be extremely helpful if other conventional home cures do not pan out well.
  4. Exercising has a huge impact on the bones. It helps strengthen the muscles that support the bones in your body. Although exercise cannot bring back the bone’s density, however, it decreases your risk of falling since it improves the muscle balance.

What type of exercise might be best for osteoporosis has not been determined yet, however, your physiotherapist can help by assigning you certain exercises that can help you with your problem. Be careful of what you eat and drink and what activities you do. If you are sure that your muscles are too weak for you to be playing any kind of sport that might make it easier for you to get injured, you need to avoid them.

Consult a doctor to chalk out a good diet plan and an exercise or workout routine that is neither too exhausting nor very lenient. I am over 50 male, in my right leg at the top of Ankle the skin has dotted black spots and in some parts of ankle it covers the ankle, it is not harmful or gives pain but it look odd and dirty, on my left lg ankle it is not so black spotted. Blend one tablespoon of lemon juice and yogurt with two tablespoons of gram flour powder. Add a pinch of turmeric to this mixture and blend them well into a paste. Apply this pack on the affected area and massage it for 3 minutes. Allow it to dry for 20 minutes and wash the area using lukewarm water.

Ankle pain. Our suggestion would be to do hot water fermentation along with Cold (ice) therapy. You shall also keep a pillow under the knee while sleeping, next is you can keep ice in the painful area for about 5--10 minutes.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have a 3 day history of back pain which is not long and it could be just a muscle strain if you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you and you can also add a muscle relaxant like tab myoril for 5-7 days - physical therapy initially under supervision of a trained physiotherapist and then to continue at home (only modalities and no exercises unless you become pain free) - adequate calcium & vitamin d intake if levels are low in body - ice packs to be applied 3-4 times in a day for 5-7 days - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation.
