Mr. lybrate-user, Thanks for the query. Fasting glucose 173 mg / 100 ml, is a clear indicator of uncontrolled diabetes. This needs further testing like getting after food glucose and HbA1c% tested, plus lipid profile and kidney functions also. Then initiate appropriate treatment. Once diabetes is confirmed there is a specific need for antidiabetic drugs. Other measures are only supportive. Plus one has to follow diet restriction, regular exercise and weight control. Thanks.

I have been diagnosed T2DM for the past 11 years. She now takes one Tripride-1 in the morning after breakfast and one TENEFRON M 20/500 in the night after food. But her blood sugar remains high at 183 fasting and 276 post prandial. Our doctor has suggested the morning dosage to Tripride-2 one tablet. Is this ok? 2. Cause of T2DM May be (a) low insulin production by beta cells or damage to beta cells (b) normal insulin production, but release inhibited or (c) insulin resistance or (d) high glucagon activity. Are there any methods to diagnose the cause correctly? What is the cause of my illness? Are all the four causes treated by the same medication? Are the medicines now used by me enough to address my problems.

Hello, Thanks for the query. the drugs used to treat T2 diabetes which you are on, do tend to act on various causes that lead to increased blood glucose levels or diabetes. For example Glimepiride stimulates Beta cells to produce Insulin, Metformin helps slow down glucose absorption and reduces glucose load, plus helps reduce insulin resistance. Pioglitazone again helps reduce Insulin resistance. Tenelegliptin acts through Incretin system and helps insulin secretion. Therefore, there is no need for you to worry about the etiology part of diabetes. Type diabetes most often is because of lack of insulin secretion. In an obese or overweight person there is more likely hood of increased insulin resistance. Thanks.

Mr. lybrate-user, Thanks for the query. Your HbA1c% is quite good, which means the present treatment has really been effective, I do not see the reason for changing it. By reducing the dosage, there is a possibility of glucose levels going up again. Thanks.

Mr. lybrate-user, Thanks for the query. For a person not having diabetes, the major precautions to be taken are eat a healthy diet, avoid to much of sugary foods, colas, junk foods, bakery items, deep fried foods and too much processed (packaged) foods. Avoid red meat. Increase the intake of green leafy vegetables, salads, fruits. Always better to use whole grain flour for roti and chapati. Plus eat in moderation. Thanks.

Mr. lybrate-user, Thanks for the query. Your fasting glucose is a bit higher than required. That is suggestive of impaired fasting glucose, a pre-diabetic state. PP is fine. So to prevent development of diabetes you have to maintain normal weight and take a proper planned diet. Thanks.

Sir my father aged 45 has high sugar level. On 31/10 by father's Fasting:- 117.04 mg/dl and PPBS 238.36 mg/dl. After 10 days of ayurvedic medication the sugar level on 11/11 is Fasting 124.75 mg/gland PPBS :- 274.06 mg/dl. For accuracy again on 13/11 the sugar was measured it was Fasting 137.74 mg/dl and PPBS:- 329.36 mg/dl and HbA1c was 11.88. Also in all the above cases the urine sugar was Fasting (+) but PPBS (++++). Also the method use for measuring blood sugar and HbA1c was GOD-POD Enzymatic and Nephelometry respectively. Please reply the test that should I do to find the main cause and please say whether is a diabetic condition or not. I think pancreatic profile and insulin test may help. Please reply.

Mr. lybrate-user, Thanks for the query. All the glucose values (fasting & Pp) and HbA1c% clearly show that you have uncontrolled diabetes. There is no need to do any pancreates related tests. Whith a HbA1c% 11.88, the diabetes is totally not control. You have to initiate appropriate treatment immediately. Please do not waste time. Because uncontrolled diabetes can lead to development of severe complications ealrly. Taking only Ayurvedic drugs for diabetes is unlikely to do any good. Plus there is a need for diet control and regular exercise. Thanks.
