lybrate-user, Thanks for the query. T2 diabetes has become a fairly often seen condition now a days. Following precautions need to be taken to keep a good health: 1) Take appropriate medicines to check the raised blood glucose levels (as suggested by his doctor. 2) Ideally fasting glucose should be < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. 3) Avoid sweets, deep fried foods, junk foods, pastries, bakery items and foods made of maida. 4) Should maintain near normal weight for his height and BMI should be < 23 kgs/M2. 5) Should exercise regularly for at least 1 hour a day. 6) Refrain from alcohol ad smoking. 7) Should consume a good deal of green leafy vegetables, salads and fruits every day. (except banana, mango, chikkoo and grapes). 8) Do a regular check up of blood glucose levels. In other words should lead a modified lifestyle, that will help remain healthy and fit. Thanks.

Urine samples have urine albumin slight trace. Also FBS sugar level is 120. Blood hemoglobin is 11.8, lymphocytes 48, esr 25, creatine level 1. Any issues with albumin slight trace? Is there any serious medical conditions?

Hello, Thanks for the query. Most of the parameters mentioned look within normal limits, except FBG. Blood glucose 120 mg is in the pre-diabetic category. There is no mention of after food glucose level Also no height and weight details are given. Thanks.

Mr. lybrate-user, Thanks for the query. For a patient with diabetes regular foot care is very important, particularly when one has long standing diabetes. Points to be considered are: 1) Regular examination of both feet daily, 2) Keeping the web spaces between the toes clean and dry, after a bath or spending time in water. 3) Observe for any change in sensation and whether there is any swelling or change in colour of the skin. 4) Always use properly fitting foot wear even at home to avoid injuries. 5) Be careful while cutting the nails, do not try to go deep and get an injury. 6) If there is any hardening of the skin of sole, or a corn develops do not try to remove it at home. 7) If fungal infection develops between toes get it properly treated. Lastly always maintain strict blood glucose control. Thanks.

I have hyperthyroidism in 2001 but after taking neo mercizol cured n medicine stopped. Now after 17 years again I found same problem T3 4.36, T4 1.55 TSH 0.01.i am male 60 plus. What should the dose now. My doctor prescribed initial 5 mg twice a day. Will it ok or 10 mg twice a day.

Mr. lybrate-user, Thanks for the query. Start with a lower dose initially, then check TSH levels after 2 months and if level is still too low, then switch to a higher dose. Thanks.

lybrate-user, Thanks for the query. The blood glucose levels (fasting is high & PP is normal) and HbA1c% both show there is a good control. To reduce "belly fat" you will have to do a good deal of abdominal exercises as well as some aerobics like skipping, jogging. Plus increase the intake of green leafy vegetables, salads and permitted fruits. Avoid deep fried foods, junk foods, bakery items, pastries, foods made from maida. Always use whole grain flour for roti and chapati. Take small quantity of food per meal and take 4 to 5 meals in a day. Also use fenugreek seeds (methi) roasted or powdered, before every meal about 2 to 3 spoons. Thanks.

Mr. lybrate-user, thanks for the query. When diabetes is present for many years it causes diabetic neuropathy, where patient feels tingling, numbness and burning sensation in feet. This happens even when blood glucose levels are fairly well controlled. But if the control is bad then symptoms appear much earlier and are worse. Since there are no details about his blood glucose levels (fasting, PP & HbA1c%), present treatment, dosage of drugs being taken, diet, extent of daily exercise, weight and height, there is no way to give any specific suggestions. Hi doctor, I am a 30 year old married female. Before 2 year, I was diagnosed with hypothyroidism. I have been taking Thyronorm 75 mcg. Recently, I had TSH test Latest test on 17 Nov 2017 T3 = 96 ng/dl T4 = 8.1 μg/dl TSH = 0.87 μIU/ml Previous test on 3 Apr 2017 T3 = 105 ng/dl T4 = 3.6 μg/dl TSH = 141.40 μIU/ml Can I really stop the medications? Or Shall I reduce the mcg? When will my thyroid levels become normal. Hair fall also increasing.

lybrate-user, Thanks for the query. TSH 0.87 mU/L, is within the normal range. Which means your present dosage of thyroxine is working very well. There is no reason to alter the dosage. All the symptoms related to hypothyroidism are in check when TSH is within the desired range. But hair fall takes more time to stop. Please do not alter or reduce the dose. Thanks.

Mr. lybrate-user, Thanks for the query. Diabetes does affect the small blood vessels in the body. Thus affecting blood supply to penile muscles. Even when blood glucose is well controlled, this effect can be seen leading to erectile dysfunction. However, strict blood glucose control is helpful in reducing the problem. The tablet being used by you is the type of medicine recommended for overcoming the problem. There is no other option. Thanks.

lybrate-user, Thanks for the query. There are some studies going on since many years for developing a vaccine for T1 diabetes, however, still there no success. Now the expectation is by 2018 there may be a possibility of introduction of such a vaccine. For T1 DM but at present there is no vaccine. Plus for T2 diabetes there is possibility of that at all. Hello, Thanks for the query. The glucose levels mentioned are fairly good. Ideally fasting should be < 100 mg & PP 150 to 160 mg. These levels can be achieved by altering your diet and increasing the amount of daily exercise that you are doing. There is no need to change the medication. Thanks.

lybrate-user, Thanks for the query. Your thyroid test results are absolutely normal. TSH range is 0.3 to 5.5 mU/L. In some labs it is shown as 0.4 to 4.5 mu/L. Therefore 1.47 mU/L is within the normal range. There is no reason to worry. Thanks.

Doctor Ji, Blood sugar range as per lab, fasting is 100 mg % (Normal Rage 60-110 Mg%) and Postprandial is 144 mg% (Range as per report 80-150 mg%. And as per electronic sugar checker is showing pasting 154 post 165. This is for my mother's report aged about 74 yrs. Dosage is Melmet SR 500 mg Tab, 1 tab for morning and 1 tab for night. Is it required to continue the same dosage or any advise for the same.

lybrate-user, Thanks for the query. The blood glucose levels (fasting 199 mg & PP 144 mg) are quite good. The other levels mentioned (fasting 154 mg) is high but PP 165 mg is good. The blood glucose levels from venous blood are about 11% lower than those from capillary blood (glucometer). Please check the HbA1c%, it should be <7 % or so. Which means her diabetes is under good control. There is no need to change any medication. Thanks.

lybrate-user, Thanks for the query. Since you are on treatment for hypothyroidism, it is essential to maintain TSH level around 3 to 4 mU/L to be free from all the symptoms. Hence, it is clear that present dosage of thyroxine (75 mcg) is not adequate. Please increase it by 12.5 mcg and then check TSH after 6 to 8 weeks, to see if the desired level is reached. At that level all the symptoms will disappear. Thanks.

lybrate-user, Thanks for the query' When on treatment for hypothyroidism, it si necessary to maintain TSH around 3 to 4 mU/L, then only all the symptoms are in check. Since there is no mention of the TSH level, it is difficult to comment. As regards the elevated levels of liver enzymes it may be due to fatty liver. So please let me know your lipid profile, thyroid hormone levels, and whether you exercise daily as well as your dietary pattern. Thanks.

Hello, Thanks for the query. TSH 10 mU/L, is showing he has hypothyroidism which needs to be treated with adequate dose of levothyroxine. It is necessary to maintain TSH around 3 to 4 mU/L, to be free from all the symptoms of hypothyroidism. He should avoid soy beans, soy containing products and should consume a good deal of green leafy vegetables, salads and fruits. Avoid bakery items and foods made of maida. Plus exercise daily for 1 hour. Thanks.

Please explain what particular condition are you referring by saying "thyroid disease" Because there are two major conditions: hypothyroidism and hyperthyrpoidism. There is no specific diet for thyroid disease. Only if someone has hypothyroidism then soy beans and soy containing products should be avoided. Plus one should consume more of green leafy vegetables, salads and fruits. Always use whole grain flour for chapati and roti. Should avoid maida and products made from it. Thanks.

Mr. lybrate-user, Thanks for the query. First and foremost it is necessary to conrol blood glucose very strictly, with fasting < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. The second point, it looks like fungal infection of the glance or prepuce. It needs to be treated with antifungal drugs. Thanks.

lybrate-userji, Thanks for the query. Weight loss in a diabetes patient occurs when blood glucose is uncontrolled and there is excess glucose lost through urine. Hence, primary need is to achieve a strict blood glucose control with fasting < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. Once having achieved this then the efforts to add some weight can be successful. Since there are no details given like weight, height, fasting, PP glucose levels, dietary pattern and extent of daily exercise and drugs with dosage, no specific suggestions can be made. Thanks.
