Nephrotic Syndrome: Know Its Causes and Treatment!

Glomerulonephritis is a disease that is caused due to inflammation of the small filters that are present within the kidneys or glomeruli. Glomeruli eliminate the excess waste, electrolytes and fluids from the blood, discharged through urine. A glomerular disease can be either acute or chronic. If the condition arises without a combination of any other disease, it is termed as primary glomerulonephritis. Secondary glomerulonephritis is characterized by diabetes or lupus (an auto-immune disorder) being at the root of the disease. Prolonged or severe inflammation can take a toll on the kidneys.

Causes-

Nephrotic syndrome can be primary, being a disease specific to the kidneys, or it can be secondary, being a renal manifestation of a systemic general illness. In all cases, injury to glomeruli is an essential feature. Kidney diseases that affect tubules and interstitium, such as interstitial nephritis, will not cause nephrotic syndrome.

Primary causes of nephrotic syndrome include the following, in approximate order of frequency:

  1. Minimal­-change nephropathy
  2. Focal glomerulosclerosis
  3. Membranous nephropathy
  4. Hereditary nephropathies

Secondary causes include the following, again in order of approximate frequency:

  1. Diabetes mellitus
  2. Lupus erythematosus
  3. Viral infections (e.g., hepatitis B, hepatitis C, human immunodeficiency virus [HIV] )
  4. Amyloidosis and paraproteinemias
  5. Preeclampsia
  6. Allo-antibodies from enzyme replacement therapy

Nephrotic-­range proteinuria may occur in other kidney diseases, such as IgA nephropathy. In that common glomerular disease, one-third of patients may have nephrotic-­range proteinuria.

Nephrotic syndrome may occur in persons with sickle cell disease and evolve to renal failure. From a therapeutic perspective, nephrotic syndrome may be classified as steroid sensitive, steroid resistant, steroid dependent, or frequently relapsing.

Treatment-

  1. Whether one is suffering from acute or chronic nephrotic glomerular , with symptoms moderate or severe, the treatment lies in treating high blood pressure and other underlying conditions of the disease.

  2. Water pills help control sweating, thus increasing the fluid flushed from the kidneys.

  3. Statins reduce cholesterol level.

  4. Blood thinners such as anticoagulants lower the risk of blood clots.

  5. Corticosteroids regulate the immune system and ease the inflammation that results from kidney disorders.

  6. Incorporating certain lifestyle changes such as opting for lean proteins, cutting on the intake of fat in the diet and consuming lesser amounts of salt can treat inflammation and swelling.

  7. For kidney failure, dialysis can come to the aid in eliminating excess fluids and regulating hypertension.

Dear, first get urine examination done to confirm or rule out infection. If urine is normal then sonography (kub) with full bladder volume, bladder wall thickness on full bladder and post-void residual urine volume. If that is nor mal then anal fissure is to be considered which is usually due to constipation and if so treat constipation. Dr. Kiran mehta.

Dear , first we have to identify why is he not able to pass urine. It could be due the paralysis and/or due to the prostate problem. If it is due to an enlarged prostate you could get a prostatectomy done. However if it is due to stroke then you have to change the ctaheter. Which side is he paralysed on? did he have a spinal injury or astroke if it is a stroke was it a blood clot or a bleed? can you please upload the ctscan and or mri and his ultrasound reports?

Colonoscopy is relatively safe in absence of medical/surgical contradications when done by trained doctor. The only issue you need to know about is about gut perforation the chances of which is very low about 1%.

You should follow these very important and simple things to prevent stone formation & to get sure treatment of kidney stones: 1. First take a lot of fluid daily. The more you drink water and fluid, the less amount of deposit accumulate in kidney. So you have to make your kidneys healthy just only by drinking 2-3 liters (10-12 glasses) of water. 2. Stones are mainly formed of calcium and oxalate. So take very low quantity of milk, cheese, cream and dairy products. 3. Do not take strawberry, plum, spinach and tomatoes. These fruits and vegetables are rich in oxalate. So avoid them to prevent from kidney stones. 3. Stop taking supplementary vitamin d and calcium medication. 4. Avoid animal protein diet- eggs, red meat, fish in diet. 5. Take less amount of salt, pickle and western foods. They have high amount of sodium. So avoid it. 6. Urinary infection are also one of the cause of stone. So take proper antibiotic treatment of urinary infection on time. Even when you feelburning sensation or pain during urination, don't avoid it. Take medication as soon as possible. I hope you follow them in your life andstay healthy. 7. You can take gokshur tablets of himalaya for 3 months. It definitely get relief from kidney stones.

I am aged 50. I am a vegetarian. Recently, I checked normal yearly routine check up. My creatinine level is 1.4 and urea is 20. Dr. Says that it is a border line. Hence, need not worry much. U can control it naturally by avoiding salt and taking coriander, green tea, fresh fruits and veggies. Is that ok? Or will I have take serious and go for further tests.

At the age of 50 blood level creatining and urea of 1.4 and 20 respectively are not much to worry about. They are borderline but going on a healthier diet (as suggested by your physician) shoul keep them under control. And you don't need to take any further test for this.

Lemon water ------------- hi dear lemon water produces cold in body, so it is of use in summer season. Piles is localized engorgment of blood vessels in the anal area. Initially it is limited to inside the anal canal and gradually they tend to pout outside. They may or may not cause bleeding. They may just cause irritation or some anal discharge. Piles tend to get aggravated by chronic constipation. Yes, what you perceive as piles may not be piles because there is an overlap of symptoms with certain other conditions like fissure, fistula, polyp and occasionally more serious problems like anorectal cancer. Occasionally in patients of cirrhosis liver you may get secondary piles as a consequence of portal hypertension. That would require a different kind of management. It is important for you to know that anal fissure & piles improves with general measures but may require surgery. The general measures I would suggest are 1. Take lot of vegetables & fruits in your diet 2. Avoid chillies & spices 3. Take lot of fluids 4. Avoid constipation 5 periodically do anal exercises (alternately contracting & relaxing the buttocks, anal area & upper thighs) 6. Take sitz bath 3-4 times a day. This means sitting in a tub of warm water & doing anal exercises 7. Atleast once get it examined by a general surgeon.

Dear lybrate user. I can understand, Burning in anus while passing stool could be due to anal fissures and dryness due to the fissures. Dryness of your anus is the key and you should concentrate more on your food. You should take fiber rich food and you should drink lots of water. These combined should give your relief. Take care.

There are multiple disorders which might produce the above mentioned symptoms, it could be Fissure/coccydynia or even a pilonidal sinus. It is suggested to get clinically examined by a surgeon for a confirmed diagnosis and therapy for the same. However though the few symptoms which you have mentioned are not alarming.

Kidney is the most vital organ in the body as it eliminates the waste material from our blood. When kidneys fails to carry out its work effectively the waste materials gets accumulated in the blood and end up with many serious disease condition that affects the other vital structures in our body. The function of the kidney is carried out artificially by an external device called dialyser to purify the blood and remove the waste products from it. This procedure is called as dialysis. There are two types of dialysis they are haemodialysis and peritoneal dialysis. In Haemodialysis, the waste products are removed from the blood and there by the fluid and electrolyte balance is maintained.

Things a person should follow if they are undergoing haemodialysis

  1. Love your heart: Patients undergoing dialysis are at more risk of getting cardiac problems. So it is best advised to follow few of the following measures to take care of their heart
    • Keep your blood pressure and cholesterol levels under control
    • Quit smoking
    • Reduce stress
    • Have healthy low fat and heart friendly diet
    • Get regular physical activity but avoid excessive strainers activity that may require extra effort from your heart
    • Maintain a healthy weight
  2. Diet: Dialysis not only removes the waste matters from your blood but sometimes also the essential electrolytes and nutrients. Protein, potassium sodium and phosphorus are the major things that are to be noted.
    • Encourage the patient to take high quality protein rich foods like meat, fish, milk, egg and chicken which is essential to improve your muscle strength and immunity.
    • Have a check of your sodium level and take sodium rich foods accordingly, as excess sodium may increase the fluid intake.
  3. Limit your fluid intake: Use ice cubes instead of water or use small cups of water. Make sure you measure every sip of water you take.
  4. Don't Overeat: Slow down while eating and stop eating when you feel full. don’t over load your stomach. If you wish to discuss about any specific problem, you can consult a nephrologist.
