If your hands are shaking almost any time. Then its a matter of concern. Consult a neurologist to treat the same. Stress, fatigue, and too much caffeine can worsen it. Hi Sameer Excess fat deposit on tummy area is due to insulin and glucagon hormone imbalance. To lose tummy fat you should avoid starchy and sugary food exm. chapati, rice, bread, potato. U can use high protein diet exm. pannier, egg, chicken. Take lipolyzer tummy tablet after lunch and dinner. Microfruit and cytofresh tablet after breakfast.

To lose weight, you have to maintain an input-output ratio. Whatever food you eat contains the input calories which gives you the output calories in the form of energy. If your input calories is too much, you are likely to put on weight. The best way to lose weight is to exercise or spend the additional calories accumulated in your body in the form of fat. Hi Insulin pump would be ideal for brittle diabetes. Those who can't afford, can manage with insulin and proper diet which mostly is small frequent meals with a little knowledge about carbohydrate count which will be of much help in insulin dose adjustment. You need continuous blood glucose monitoring for few days to understand your dips and surges in blood sugar which inturn is very useful in adjusting your insulin dosage. De-stressing also plays an important role. Avoid alcohol, make sure insulin needle is changed after every fourth prick (if it's a flex pen) and is taken properly. Stay fit and strong, as fitness level rises, diabetes risk reduces.

Surgical Sperm Retrieval (TESA/PESA/MESA)

The main methods of surgical sperm retrieval available include:

  • PESA: percutaneous epididymal sperm aspiration.

  • MESA: microsurgical epididymal sperm aspiration.

  • TESA: testicular sperm aspiration. This includes testicular fine needle aspiration (TFNA).

  • TESE: testicular sperm extraction.

  • Microdissection TESE.

  • Perc biopsy: percutaneous biopsy of the testis.

Which method is used depends on the nature of the problem in the male partner, which needs to be explored carefully first.

  • Tests required before surgical sperm retrieval

A man that produces no sperm in his semen is said to have azoospermia. This may be because of a blockage in one of the tubes that carry sperm from the areas of the testes where they are produced, out to the penis during ejaculation. Obstructive azoospermia can be caused by testicular cancer, as the tumour presses on the vas deferens. This type of cancer is common in young men and can be treated successfully. It can, however, lead to infertility, so surgical sperm retrieval may be performed to store some sperm before treatment begins.

Other conditions cause non-obstructive azoospermia, including having an abnormal cystic fibrosis gene. Men with this condition may not show all the symptoms, but they often have no vas deferens. Surgical sperm retrieval is possible but there is a 50:50 chance that the embryos produced by subsequent ICSI and IVF will have the same genetic abnormality. Options then include using a sperm donor and intrauterine insemination (IUI) or IVF, or having pre-implantation genetic diagnosis (PGD) performed on the embryos to select ones that carry the normal gene.

If the problem that underlies poor sperm production is physical rather than genetic, or if a couple wants to have children after the male partner has had a vasectomy that cannot be reversed, surgical sperm retrieval can go ahead

  • Surgical sperm retrieval techniques used when obstruction is the problem

When the release of sperm is prevented by a blockage in the vas deferens, or by a vasectomy, several techniques can be used to retrieve the large numbers of sperm that remain inside the testes. The first three involve aspirating sperm using needles or tubes placed through the skin of the testis and are carried out under local anaesthetic. The fourth requires open surgical sperm retrieval and is usually carried out under general anaesthetic.

Some small studies have been done to compare success rates after the different types of surgical sperm retrieval. These concluded that MESA gives the highest number of sperm, with a hundred times more sperm being recovered compared to TESA and perc biopsy.

MESA also produced sperm that were better swimmers and therefore more useful for infertility treatments, including IVF and ICSI.

  • Surgical sperm retrieval techniques when there is no obstruction

Men who have no sperm in their semen, despite having clear tubes in their testicles, usually have a problem with the process of sperm production. It is unlikely that sperm are present in large numbers, so the surgical sperm retrieval techniques required are more invasive

  • TESE: Testicular sperm extraction. This involves opening up the scrotum and taking a large volume of testicular tissue, perhaps from several regions of the testicle. Sperm are then retrieved using a microscope to identify individual sperm.

    • Microdissection TESE: A similar technique but a microdissecting microscope is used to pinpoint the tissue to be removed. This aims to cause less damage to the structure inside the testicle, and to therefore have fewer after effects such as blood supply problems caused by tiny blood vessels being cut. It also appears to increase the number of sperm that can be retrieved.

Steam inhalation carefully by steam inhaler twice a day for 2-3 days frequent/ continuous cough cold may be because of many reasons like: 1. Any ent problem like dns polyp sinusitis 2. Low immunity 3. Allergic condition so you need to consult ENT doctor for proper examination and further management.

Causes of lack of sleep are : • stress • Anxiety • Depression • Pain • irregular schedule of sleep such as sleeping during day time & working during nights. • Caffeine , alcohol • Due to drugs • Due to high blood pressure . • Aging Due to lack of sleep there are many complications arise such as lack of concentration ,irritability, fatigue & digestion problem. So these are some instructions & methods for good sleep : • Frequent oil massage of head with oil relax your mind and enhances sleep .You can also do foot massage daily it destress you . • Do not take stress . Do meditation and long breathing pranayams to relax the mind. • Do not take caffeine, tea & alcohol in excess and these are stimulant ,(activate) your brain result in lack of sleep. • Avoid drinking alcohol & smoking. • Avoid sleep during day time as it makes it harder to fall asleep at night. •Try to sleep early at night . Do not think during sleep and avoid use of laptop during night . Do exercise and long walk daily . • Avoid to take sleeping pills because these are habit forming drugs and these drugs disturb your normal sleep pattern. • Take a bowl of curd daily in afternoon and one glass of milk at night daily .

Follow the following tips:- · Pay attention to your health and food intake. · To increase your appetite, have a glass of lime water or 1 cardamom (elaichi) before your meal.  · Have minimum 4 meals in a day. · Drink 1 glass of milk twice a day. · Try to enjoy your meals and gradually increase the quantity you eat every time. · A balance diet should have all essential nutrients in it. Breakfast is the most important meal of the day. It should include combination of Both Carbohydrates and Proteins for e.g. Oats with Milk or Poha/ Upma with a glass of milk. If you do not like milk have 1-2 egg whites in the morning along with your breakfast. · You need to focus on your protein intake : Increase your protein intake e.g. milk, dairy products- cheese, paneer, lentils (dals), nuts ( soya, peanuts, dry fruits ), pulses e.g. rajma, chole, kidney beans etc. sprouts. If you are a non-vegetarian, then you can have proteins by also eating eggs, chicken, fish. Make sure you are including at least 2 serving of any of these in your daily meal. · Lunch and dinner again should be a combination of Proteins, Carbohydrates and Fats which could have dal, roti, sabji/chicken, salad.  · Please do have minimum 4 almonds or a small bowl of peanuts every day. · Please ensure that you drink at least 3 litres of water every day.
