Causes And Symptoms of Exocrine Pancreatic Insufficiency

Exocrine Pancreatic Insufficiency or EPI is also known as pancreatic deficiency, a disorder where the pancreas is unable to produce sufficient amount of enzymes that are required to digest food. The pancreatic enzymes help to break down and absorb nutrients from the food in the small intestine. So, this disease causes nutritional deficiencies.

Causes of Exocrine Pancreatic Insufficiency :

This condition is generally caused as a complication of other diseases because EPI develops only as a result of severe damage to the pancreas. The following reasons can cause EPI:

  1. Inflammation of the pancreas: After surgery in the pancreas, stomach or the intestines, there may be inflammation in the pancreas as a post-surgical complication. A high content of triglyceride fat in the blood can also cause pancreatic inflammation and hinder the secretion of the enzymes.
  2. Chronic Pancreatitis: In this disease, the pancreatic ducts are swollen and blocked and so the digestive enzymes cannot be passed into the small intestine. This condition is often caused by a heavy consumption of alcohol.
  3. Cystic Fibrosis: The digestive fluids and enzymes become thick and sticky and block the passageways of the pancreas and other organs like the lungs and the kidneys. This can obstruct secretion and passage of enzymes afterwards.
  4. Shwachman-Bodian-Diamond syndrome or SBDS: SBDS is an autosomal recessive genetic condition where enzyme producing pancreatic cells is not formed properly. This rare disorder causes a number of associated disorders like bone marrow diseases, skeletal defects and exocrine pancreatic insufficiency.

Symptoms of Exocrine Pancreatic Insufficiency:

Symptoms of EPI often appear in the middle stages of the disease when the process of absorption of nutrients has already been affected. The common warning signs are:

  1. Digestive problems like gas, diarrhea and indigestion because the food remains mostly undigested.
  2. Severe and frequent stomach pain in the lower abdominal region
  3. Greasy stools due to the excretion of undigested fat
  4. Rapid loss of weight and body mass due to malabsorption of nutrients
  5. Constantly feeling bloated and full even if you have not eaten anything
  6. A general sense of fatigue and exhaustion
  7. Excessive bleeding from small wounds because protein deficiencies hamper blood clotting
  8. Pain in the muscles and bones
  9. Increased susceptibility to infections of the body system
  10. Anemia
  11. Joint pain
  12. Abnormal swelling of the limbs or edema

Hello lybrate user. Gas trouble is due to irregular diet habits. You can take the help of allopathic med. For emergency, but not on regular basis. Taking these medicines for long time have definitely many side effects. So better you try to take healthy meals at regular time. Don't skip meals or replace it with junk foods. Take aprox 10 - 12 glasses of water per day. Even after following these things if you are suffering a lot of gas trouble you can take help of homoeopathic med. You need physical exercise and diet control avoid fried, oily food avoid sugar brisk walk at least 20 min daily for gas you can take syp digene (if no drug allergy), one tsf before food twice a day for 3 days take small frequent meal kindly take 5-6 small meal in whole day avoid excessive tea, coffee do not postpone breakfast consult me/ physician for further management.

Avoid outside spicy and fried food. Walk about 3-5 kms daily. There should be gap of 2hrs after dinner and sleep. Small and frequent meals is of help chew your food properly at time of lunch and dinner. Keep your blood sugar controlled. Raise your bed about 4" from headend side. Get your gastroendoscopy done. Tab nexpro-l 1 tab empty stomach.

Hello doctor, I am suffering from lower abdominal pain for 3-4 days. When I am sneezing it pains a lot. I thought of it as gastric problem but after taking pan-D in morning it is not gone. Gas could be due to indigestion or incorporation of air during eating or side effects of any medications (if any). If the cause is indigestion, try taking small meals at regular intervals, avoidpulses, cruciferous veggies, high fibre foods. Donot speak or drink water while eating. This should subside your problem.

Hi Dr. I am nandini ,41 yr. Gastric problem. Its starts from early morning. With lot of burps. I do hv fast 9 am n lunch 12 to 12.30 and correct hours. Hi, Inadequate sleep, untimely intake of food, intake of very hot and spicy food, mental worries and tensions, excessive usage of pain killers and antibiotics are all causes of acidity. Please follow these tips apart from taking medicines to avoid recurrence: - Do not keep your stomach empty for long durations, - Eat in small quantities frequently. - Avoid hot, spicy and oil fried food - Drink plenty of water in between your intake. - Keep away from mental stress and tensions, - Perform physical exercises daily and meditation helps to relieve mental stress and tensions. - Consume lot of fibre like raw vegetables, oats, fruits, sprouts, etc - Avoid excess intake of coffee or tea , intake of medicines on empty stomach. - Be careful about the food while eating outside. - Carry a small pack of biscuit or some fruits along with you for emergency. - Whenever you suffer from acidity, take the medications prescribed by the doctor for 2-3 weeks continuously until the wounds in the stomach gets healed completely. - Sleep at least for 8 hours a day. - Restrict the usage of pain killers and antibiotics frequently.

Hi, it seems you are suffering from 1. Gastritis or hepatitis 2. Take an antacid 3-4 tsf 8 hrly 3. Get a blood investigation - liver function test done 4. Avoid fatty, oily meals 5. No fast food, no momos and samosas 6. No tea and coffee 7. Lots of water 8. Walk after meals 9. Eat less at nights 10. No soft drinks, hard drinks and no smoking.

It is called reflux disease (gerd). Avoid heavy meals especially in the night. Do not eat at least 2 hours prior to going to bed. Avoid liquids at bed time. Raise head end of the bed bt at least 9-10 inches. In case there is no relief then she needs medical treatment.
