Tuberculosis In Relation To HIV - An Overview!

HIV and tuberculosis (TB) are two life-threatening and common diseases in the world. The two diseases are very closely connected so much so that their occurrence is normally labeled as a co-epidemic. In most cases tuberculosis is found to be the most transmissible contagion in HIV-Immunocompromised victims, proving to be the cause of their death.

What is HIV?
Human Immunodeficiency Virus, more commonly known by its abbreviation HIV, is a virus that attacks the immune system, which is the body's natural defense against illnesses. The virus terminates a kind of white blood cells (WBCs) in the immune system known as the T-helper cell also known as CD4 cells. This virus then proceeds to duplicate itself within these cells.

As HIV reduces the T-helper cells, by duplicating at an increasing rate, it slowly breaks down the patient’s immune system. This leaves the individual susceptible to other deadly diseases such as tuberculosis. HIV infection is a permanent disease with three phases of progression. HIV medications can reduce or stop the evolution of the disease from one phase to another. The treatment can also decrease the chances of spreading HIV to other people.

What is tuberculosis (TB)
Tuberculosis is a transmittable disease triggered by the presence of bacterium known as Mycobacterium tuberculosis. It is still considered a deadly disease in most developing countries where treatment is scarce. Generally, TB affects the lungs but it can also affect other parts of the body. Symptoms include – coughing with blood, fever, night sweats and loss of weight.

The relation between TB and HIV
In second world countries, most patients infected with HIV suffer from TB as the initial indicator of AIDS. Tuberculosis can occur at any phase of the HIV infection. The danger and severity of tuberculosis rises rapidly after infection with HIV. Even though tuberculosis can be a fairly primary indicator of HIV infection, it is imperative to observe that the risk of tuberculosis increases as the CD4 cell count reduces along with the progression of the HIV infection.

Treatment
Positive treatment for TB usually entails 6 months of rigorous therapy. HIV patients with TB usually respond well to this therapy, if the regimen comprises INH and a rifamycin for the period of TB therapy and cure. TB generally reappears when the immune system is unable to respond to stop the development of mycobacteria. The cytokine IFN-γ plays a key role in the response of the immune system all through the contagion.
HIV and TB infections area two-directional communication of the two pathogens. TB is one of the main causes of disease and fatality among patients with HIV in Africa and other severely affected regions. With almost a 50% kill rate around the world, raising awareness about this disease and consulting doctors when signs or symptoms appear is vital in reducing the extent of this epidemic.

Rifampicin- HIV treatment are complicated by the fact that one of the key drugs used in TB treatment, rifampicin, reduces blood levels of nevirapine (Viramune) by 30 to 55% and also reduces levels of most protease inhibitors.

Diabetes is a disease that affects the way your body deals with glucose in the blood and the glucose absorbed from the food you ingest. Diabetes is generally of two basic types - Type 1 and Type 2. Apart from these two types, there are other kinds as well including gestational diabetes that some women suffer from during their pregnancy. Type 2 Diabetes is a condition where the cells of the body do not make proper use of the insulin being produced by the pancreas. Living with this kind of a condition may seem debilitating at first, but here are a few things you can do to make life easier despite Type 2 Diabetes.

  1. Weight: Being overweight is one the foremost side effects and sometimes, even cause of Type 2 Diabetes. To make your life easier, you can lose weight so that the complications involved are that much less. Diet and exercise are key for appropriate weight loss. Watch your diet so that you can eliminate high cholesterol as well. This can ensure that unnecessary amount of fat does not stick to your muscles and organs, which in turn will fuel better functioning of the same.
  2. Health Care: Being fit with Type 2 Diabetes does not merely mean staying within your normal weight range. It also means taking care of your medication and insulin doses so that you can function in a more normal way. Make your family and friends your support system as you go about life so that people can help you and recognise symptoms when something goes wrong. Be in constant touch with your health care providers and the general physician you see so that they are in the loop and you know exactly what to do in case there is an emergency.
  3. Exercise: Being active is a great way to keep your blood sugar level low even as you ensure that you heart is in good condition. This will further eliminate the risk of heart disease, hypertension and high cholesterol. Sweating it out also helps the insulin work in a better manner within your body.
  4. Balance: While you may be asked to remove sugar from your diet, there is no need to stay away from proper meals. You can have a fibre rich diet that also has plenty of fruits and vegetables so that you get your due nutrition and maintain a fit body.

Besides genetic factor involved, it is also a life style disease. Weight reduction, active life style & proper diet having less of carbohydrate, high fiber diet. Fasting & feast not recommended in diabetics. Never do heavy exercise on empty stomach. It can lead to low sugar levels, which can be very dangerous. Always keep glucose biscuits & glucose available for low blood sugar. Watching your lifestyle is the main thing you need to do if you are suffering from Type 2 Diabetes.

When to Check, When to Worry About Fever!

Fever is not a disease! It's a known fact that normal human body temperature is 98.6 F (37 C). If temperature goes beyond that, then it's called fever. But one must not worry every time if there is a rise in the temperature. 

Why?
The reason is that immune system is designed to raise temperature as a defence mechanism if there is any attack by a foreign body (such as bacteria). So body temperature may fluctuate between 97 and 100.4. But there is no need to worry.

When to Consider Checking?
Routine check up is unnecessary. You may consider to check when:

  • You feel the warmth while you touch
  • You feel very tired, dizzy and lose appetite
  • You feel nauseated, see rashes on the body, experience loose motion, sore throat, muscle pain, headache, etc.

Is it Time to Worry?
Normally, fever is not dangerous. However, if you see the following conditions, then it's time to worry and call a doctor. 

  • If the body temperature is more than 103 F 
  • If fever lasts for more than five days
  • If you feel difficulty in breathing
  • If there is severe vomiting or diarrhoea
  • If you feel stiffness in the neck and have headache
  • If you feel excessively sleepy and lethargic
  • If you see redness in skin, rashes or blood in stool.

If the condition worsens, it is time to rush to a hospital. There could be following:

  • Meningitis: It's life-threatening and highly transmissible in case of bacterial infection. A combination of a fever, extreme headache and stiffness in the neck is an indication that you must rush to the emergency department of a hospital near you.
  • Difficulty in breathing or chest pain along with fever should prompt you to call for emergency medical assistance.
  • If someone has fever along with blood in stool, urine or mucus, it's one of the reasons to seek for emergency medical help.
  • If one has fever and is quite disturbed or puzzled unnecessarily, then the person should be taken to the emergency department.
  • A person with low immune responses (such as one who is suffering from cancer or AIDS) needs doctor's help in case of fever development.
  • Hyperthermia: it's a condition of very high fever, i.e., more than 104 F. The person seems to be confused and may not respond to verbal stimuli or commands. It's time for medical emergency.

What should You Do?
For mild fever, even home remedy can help. Aspirin shouldn't be the first choice and an absolute no for individuals less than 18 years. Other drugs that people usually tend to consume are ibuprofen or acetaminophen. One should also drink plenty of water. In case of a heat stroke, body needs to be cooled.

Other severe causes such as viral infection should lead you to a doctor. If you wish to discuss about any specific problem, you can consult a General Physician.

Most people listen to music or their favourite videos while on the move, and they spend a considerable amount of time plugging in the headphones or earphones. If you are concerned about the pros and cons of the headphones and earphones, or rather which one is better, you will have to consider various aspects. These include noise isolation, frequency response, noise cancellation and comfort of wearing.

Earphones are devices which are worn on the pinna or the internal part of the ear. This device is also referred to as earbuds, in-ears, canal phones, buds, in-ear headphones, IEMs.

Headphones are equipment worn on or around the ear. These are not considered as handy as the earphones and are also known as ear speakers. Let’s evaluate the pros and cons of these two devices and choose the one that is better.

  1. Frequency response curve: This helps in assessing how much distortion is brought about by the devices which relate to the clarity. They should be able to respond equally well to all the pitches without transmitting any frequency more loudly than another. This suggests that the frequency you are receiving should be exactly similar to what has been fed in regardless of the frequency ranges. Typically, the earphones have bad frequency response while the headphones tend to have better frequency response. If you are primarily concerned about the noise distortion, then you should opt for headphones.
  2. Noise isolation: By this, it is understood how well does the gear help in isolating other noises than what you are trying to listen. Generally, higher noise isolation is preferred unless you are willing to hear the sounds of the ambience. Better isolation also ensures that your audio would not bother others surrounding you. A rightly fitted pair of earphones would be able to give you the best noise isolation whereas good over-ear headphones would not be able to seal the sound from going out.
  3. Noise cancellation: Noise cancellation is entirely different from the noise isolation. In the process of noise isolation, the atmospheric sounds which intrude in are reproduced and inverted. This is an excellent feature especially when you are travelling through public transportation or walking down the street. When it comes to earphones, the noise cancellation feature is are not as effective as headphones unless you buy any of the high end pieces. But the headphones are far better in cancelling the noises that are constantly playing in the backdrop.

Cervical Spondylosis - How to Treat It?

Cervical spondylosis is fast becoming the bane of modern life. An age-related wear and tear problem affecting the spinal discs in your neck, cervical spondylosis is increasingly affecting the young because of the use of cell phones, laptops and the like.

Cervical spondylosis is a general term used to define shrinkage of disc in between vertebrae in the neck region. Along with disc degeneration, bony projections also form in the area and are called bone spurs.
Other causes of cervical spondylosis are dehydrated discs. Discs are thick, pad-like cushions in between vertebrae that act as shock absorbers. They are made of a gel- like material that can dry over time. This causes the spinal vertebrae to rub together causing pain. The discs also crack which allows the internal gel-like material to spill out and impinge on spinal nerves causing symptoms.

There is a narrowing of the space required by the spinal cord and nerves that emanate in the upper spine to go to various parts of the body. Pinching of these nerves can cause alarming symptoms like

  1. Numbness and weakness in your arms, hands, legs and feet
  2. Lack of coordination and difficulty in walking
  3. Loss of bladder or bowel movement
  4. Stiffness and pain in neck

Treatment and management
Usually, cervical spondylosis doesn’t cause any symptoms. But when you start experiencing pain, stiffness and weakness of muscles, it’s best to go to a doctor who will diagnose the problem and then treat it.
Treatment for cervical spondylosis depends on the severity of your symptoms. Its goal is to get rid of pain, allow you to carry on your daily activities and prevent any permanent damage to your spinal cord and nerves.

Medicines

  1. Anti-inflammatory drugs are prescribed to reduce pain and inflammation. Example Ibuprofen.
  2. Corticosteroids are oral medications to get rid of the severe pain. If these don’t work, your doctor can suggest steroid injections.
  3. Muscle relaxants medicines relax the muscles in the upper part of the spine and help in curing neck pain and stiffness
  4. Anti-seizure medications are also prescribed to dull the pain of damaged nerves in the spine. These include epilepsy medications, such as gabapentin and pregabalin.

Physiotherapy

  1. Non surgical treatments are usually very effective in treating cervical spondylosis and surgery is usually not required. Physiotherapy is a bulwark for patients as it helps manage the condition very well.
  2. Neck exercises that help stretch and strengthen muscles in your neck and shoulders can help manage debilitating symptoms of cervical spondylosis.
  3. Few patients also benefit from traction, which can help provide more space for nerve roots within the spine.

Surgery

  1. Surgery is the last resort for treating cervical spondylosis.
  2. It is suggested when non-surgical treatments fail and the pain and neurological signs such as weakness in your arms or legs get worse.
  3. It is done to create more room for your spinal cord and nerve roots.

Cervical spondylosis surgery typically involves removing:

  1. Herniated disc
  2. Bone spurs
  3. Part of a vertebra

Pneumonia is a type of lung infection that may affect one or both lungs. In this condition, the air sacs, also known as alveoli, fill with pus or fluid, which makes it difficult to breathe. Moreover, pneumonia is known to cause inflammation in the air sacs. This disease may range from mild to life-threatening and the severity depends on the cause of inflammation, the organism fostering your infection, your age and general health. 

Symptoms:

Most common symptoms associated with the condition are:

  • Chest pain
  • Difficulty in breathing
  • Shaking chills
  • Rapid heartbeat
  • Fever
  • Rapid breathing
  • Dry cough
  • Vomiting
  • Wheezing
  • Nausea 
  • Muscle aches
  • Bluish skin tone due to oxygen deficiency 
  • Blood in sputum that is coughed up mucus
  • Labored breathing
  • Confusion 
  • High fever

Causes

  1. Bacteria: One of the most common causes of pneumonia is the bacteria named as Streptococcus pneumoniae. This type of pneumonia may occur after you have had flu or a cold or maybe even on its own. Moreover, it may affect only one part of the lung i.e. the lobe and is also known as lobar pneumonia. 
  2. Viruses: Viruses are one of the main reasons why children below 5 years of age suffer from pneumonia. Viruses that cause pneumonia may also cause cold or flu. Viral pneumonia is usually mild and may be treated at home itself. However, in some cases, it may be very severe and life-threatening. 
  3. Mycoplasmas: Mycoplasma is a type of bacteria that doesn't have a cell wall around the cell membrane which is why they are usually unaffected by most antibiotics which target cell wall synthesis. The symptoms of pneumonia caused by mycoplasmas are mild and the patient does not require bed rest. This type of pneumonia is known as 'Walking Pneumonia'. 
  4. Fungi: People with weakened immune systems or chronic health problems usually develop pneumonia due to fungi. You may also develop fungal pneumonia by inhaling large doses of it mainly found in soil or bird droppings.

Prevention against Pneumonia

It is important for all of us to prevent ourselves from pneumonia and for that we can follow the below mentioned steps:

  1. Know about the symptoms of pneumonia first: It is difficult to detect or diagnose pneumonia, especially in people over the age of 65. In older patients, the common symptoms such as fever, chills, cough may not be experienced. You should watch out for non-respiratory symptoms like weakness, dizziness, delirium and confusion. It is even more difficult to diagnose pneumonia in people who are suffering from preexisting conditions.
  2. Practice good hygiene: Common respiratory Infections, influenza and cold may cause pneumonia. You should follow hygienic habits. Wash your hands regularly before meals and use hand sanitizers in order to prevent the illness from spreading. You should also maintain proper oral hygiene as several oral infections may also lead to pneumonia. You should also keep away from people who are affected by pneumonia or any other illness like flu, cold and serious diseases such as measles or chicken pox. All these factors may cause pneumonia.
  3. Get vaccinated: It is recommended for people of all ages who may be at a risk of getting pneumonia to get vaccinated and immunized against pneumonia causing virus called pneumococcal pneumonia. This is a one-time vaccine, which prevents and reduces the severity of pneumonia. A booster vaccine after every five years may also be prescribed. Elderly people should be vaccinated against all diseases which cause pneumonia.
  4. Quit smokingSmoking accounts for being a major cause or risk factor for pneumonia. It increases a person’s chance of getting pneumonia as the lungs’ ability to defend themselves from the infection is reduced. By quitting smoking, you will be able to prevent pneumonia.
  5. Maintain a good general health: Maintaining an overall good health and following healthy habits will help you in preventing pneumonia. This is because your immune system stays strong and is able to fight efficiently against pneumonia-causing infections. A proper diet with all essential nutrients is also recommended along with regular physical exercise and proper rest.

Severe cases of pneumonia are capable of making you suffer and you may be admitted to a hospital. According to studies, it has been estimated that more than 60% of people over the age of 65 need to be hospitalised because of pneumonia. If you wish to discuss about any specific problem, you can consult a pulmonologist.

How much water should we take daily to full fill our body requirements and keep our skin glowing.
Around 2 litres in winter. 2& 1/2 litres in summer. Roughly 8 to 10 glasses. During fever, diarrhea & repeated vomitings also water requirment increases.
