If a person have 3 block 75%,80%,90%. What is preferable CABG, MIDCAB OR CARDIAC CATHETERIZATION. I am asking as a general question as many people get confused about what to do?

MBBS, Fellowship From the College of General Practioners
General Physician, Mumbai
Do not try to become a cardiologist & start advising people on blocks in the coronary arteries! the choice of treatment in these cases depends on the symptoms of the patient. Location of the blocks. Its % & many other factors. Best, leave it to a sincere cardiologist to decide.

Hi lybrate-user, I appreciate you seeking help. First and foremost, I would advise you to consult a medical practitioner (allopathic or alternative medicine) for a clinical evaluation, assessment and treatment thereof. You may require to control the same through BP medications should it be an essential hypertension with no specific cause (idiopathic). Should it be secondary, underlying cause, if any, needs to be understood and remedied appropriately to control the rise in BP. Simultaneously, following are a few remedies that you can additionally try to control your hypertension - (a) Restrict salt. Sweets, greasy, oily, fatty and high calorie foods including fast foods are best avoided. (b) Take fennel seeds (1 tsp) post meals. Also, have a clove of fresh garlic twice daily (after food). (c) Streamline your diet & lifestyle. Take a Mediterranean diet mostly. Fresh cold water fish prepared in extra virgin olive oil can be consumed twice a week. In case you are a vegetarian, you may take omega-3 supplements daily. (d) Exercise for at least 30 minutes on a daily basis, walking being the safest. Daily de-stressing and relaxation either through yoga, meditation, listening to soothing music, socializing etc all should be part of the daily routine. (e) Realistic plan your day to day activities, and execute them one by one seamlessly. Avoid a burnout by all means. (f) Try having an uninterrupted & sound sleep of at least 7 - 8 hours on a daily basis. Your bedroom should be kept dark. Try avoiding night bulbs as they are likely to interfere with quality of sleep. Should you have difficulty falling asleep, take medical help immediately. Hope this helps. Do take care and all the very best. Sincerely

Hello, I want to know about my father. He is 62 years old. in 2011 he was attached with brain stroke. Now he is taking tablet" telvas am "for controlling bp. But now a days bp is 103/61. So can you please tell me is it harmful for him in future or not? Thanks,

MBBS, MD-General Medicine, DM - Cardiology
Cardiologist, Secunderabad
Hi, brain stroke patients have poor autoregulation mechanism. So it's necessary to keep the BP in a range. Although controlling high BP is important, keeping at too low may be harmful also. I will be comfortable if it's in a range of 110 to 130 systolic. You may consult your doctor to adjust his medication accordingly.

I am experiencing chest pain, discomfort on left side (front and back chest, and left upper arm). So far, doctors have performed Echo (clear, no issues indicated), TMT (when the technician stopped it, it showed Max HR 152 bpm 83% of max predicted 179 bpm. Max BP 160/100. Remarks say -" Good effort tolerance. No Angina or Arrhythmia. No Significant ST-T changes found), Endoscopy (didn't show any ulcers or any issues, normal mucosa), and Ultrasound scan of abdomen (only issue as per report is Fatty Liver measuring 17.5 cms). However, I have been to emergency room 4 times during last 1 month due to chest pain, sweating, etc. Everytime, they did ECG and Troponin tests and then sent me home after giving Pantoprazole injection. Doctor says I don't have any heart related condition. According to him, I have" Non-ulcer Dyspepsia" that is being worsened by my anxiety and panic. He has suggested - Pantodac DSR (twice a day), Atorvastatin 10 mg (at night), and Alprazolam 0.25 (at night). My cardiologist says that since TMT and Echo were normal, there is no possibility of a heart attack. I asked him whether I should undergo any further tests like CT Angiography to rule out any blockages etc. He told me very clearly that I am panicking for no reason and I should consult a psychiatrist. He says that my pain is result of anxiety and panic. I tried explaining to him that I do get anxiety and panic but it happens after I get chest pain, sweating, and breathlessness. What to do now? Should I consult another cardiologist? Is a clear TMT indicator that there is no risk of a heart attack? Is a clear Echo indicator that there are no other heart issues?

MBBS
General Physician, Mumbai
Dear lybrate user, -you have undergone most of the tests & since they were normal the cardiologist has not advised further testing so do not panic & take the prescribed medicines, sometimes acidity also causes similar symptoms. -engage yourself in various activities like walking for 20-30 min daily, yoga, meditation & other day to day activities. -avoid spicy & fried food -give sometime to the medicines to take effect -if you donot find any improvement then go for a second opinion.

Sir. I underwent open heart surgery at my 62 years of age i. E. In the year 2000. I was alright and take ecosprin 150 every day. Recently I observed that when I take my food or tiffen, I feel some pain while walking even a bit distance. This remains for half an hour so then I will be alright. Please. Advise me the reason for this and any suggession by way of medicine etc. Thanking you sir.

DM Cardiology
Cardiologist, Delhi
Dear , chest pain occurring on walking is definitely angina. You should start taking tab angispan tr 2.6mg bd, and tab nikon 5 mg bd and see if the pain improves. If it does not improve, get a coronary angiogram done, there could be new blockages.

When dealing with low blood pressure, it is usually suggested to increase your salt and fluid intake. Before increasing your salt consumption, do check with your doctor. Also, cut down on alcoholic beverages and high-carbohydrate foods such as bread, rice, pasta, and potatoes.
